【摘要】：目的探討剖宮產(chǎn)術(shù)后發(fā)生切口感染的危險因素,制定對應的護理干預對策,以降低感染發(fā)生率。方法回顧性分析2009年2月-2013年2月收治的1 473例剖宮產(chǎn)產(chǎn)婦臨床資料,模型篩選采用Stepwise法,采用非條件logistic回歸分析危險因素。結(jié)果 1 473例剖宮產(chǎn)產(chǎn)婦術(shù)后共發(fā)生切口感染42例,感染率2.8%;單因素分析顯示,切口感染的危險因素包括高齡30歲、合并基礎(chǔ)疾病(糖尿病)、體質(zhì)量指數(shù)25kg/m2、術(shù)中出血量300ml、手術(shù)時間60min、侵入性操作5次,其感染率分別為6.6%、12.3%、7.3%、5.8%、5.8%、7.0%,差異有統(tǒng)計學意義(P0.05);多因素分析顯示,合并基礎(chǔ)疾病的危險因素最大(P=0.009),其余危險因素依次為侵入性操作頻繁(P=0.012)、手術(shù)時間較長(P=0.018)、體質(zhì)量指數(shù)較高(P=0.021)等。結(jié)論剖宮產(chǎn)術(shù)后發(fā)生切口感染與多種因素關(guān)系密切,臨床可采取針對性的護理干預對策預防感染,確保母嬰安全。
[Abstract]:Objective to explore the risk factors of incision infection after cesarean section and to make corresponding nursing intervention measures to reduce the incidence of infection. Methods the clinical data of 1 473 cases of cesarean section from Feb 2009 to Feb 2013 were analyzed retrospectively. The model was screened by Stepwise method and the risk factors were analyzed by non conditional logistic regression analysis. Results there were 42 cases of incision infection in 1 473 cases of cesarean section, the infection rate was 2.8%, univariate analysis showed that the risk factors of incision infection included 30 years old. Combined with basic diseases (diabetes mellitus), body mass index (BMI) of 25kg / m2, intraoperative bleeding volume of 300ml, operation time of 60 min, invasive operation of 5 times, the infection rate was respectively 6.6 / 12.3and 5.8cm / 5.8cm, the difference was statistically significant (P0.05), the multivariate analysis showed that, The risk factors associated with underlying diseases were the largest (P0. 009), and the other risk factors were frequent invasive operation (P0. 012), long operation time (P0. 018), high body mass index (P0. 021), and so on. Conclusion incisional infection after cesarean section is closely related to many factors. Clinical nursing intervention can be taken to prevent infection and ensure the safety of mother and child.
