產(chǎn)婦與嬰兒脊髓灰質(zhì)炎中和抗體水平變化特征分析

【摘要】:目的評價脊髓灰質(zhì)炎中和抗體在產(chǎn)婦及其嬰兒體內(nèi)消長規(guī)律,比較不同免疫程序下嬰兒抗體水平變化。方法選取2013年7月—2014年4月在廣州市荔灣區(qū)婦幼保健院進行分娩的產(chǎn)婦及其新生兒作為研究對象,采用血清流行病學(xué)方法對產(chǎn)婦及產(chǎn)婦所生嬰兒0(新生兒)、3、6月齡脊灰中和抗體水平進行分析;并根據(jù)嬰兒基礎(chǔ)免疫程序?qū)⑵浞譃榧顾杌屹|(zhì)炎減毒活疫苗(OPV)組、脊髓灰質(zhì)炎滅活疫苗(IPV)與IPV+OPV 3組,比較疫苗接種后的抗體水平。結(jié)果共采集179位產(chǎn)婦外周靜脈血,嬰兒0、3、6月齡時分別采血176、149、62份。新生兒較產(chǎn)婦I、II、III型脊灰中和抗體水平有所下降,3、6月齡時抗體水平均大幅上升,6月齡時各型分別高達607.0(95%CI=146.0~2 523.1)、239.6(95%CI=80.4~713.6)和235.9(95%CI=56.0~994.8)。OPV、IPV、IPV+OPV組在嬰兒各個時間段脊灰I、II、III型抗體幾何平均滴度倒數(shù)(GMRT)和抗體陽性率差異均無統(tǒng)計學(xué)意義(均P0.05)。產(chǎn)婦脊灰抗體水平與新生兒脊灰Ⅱ、III型母傳抗體呈顯著正相關(guān)關(guān)系(tII=5.953、P=0.000,tIII=7.260、P0.001)。結(jié)論新生兒脊灰胎傳抗體水平較母體下降。IPV全程接種、IPV與OPV序貫免疫對嬰兒均有較好的免疫原性,中國現(xiàn)階段可大力推進IPV與OPV序貫免疫程序。
[Abstract]:Objective to evaluate the changes of neutralizing antibodies against poliomyelitis in puerpera and their infants, and to compare the changes of antibody levels in infants with different immune procedures. Methods from July 2013 to April 2014, the pregnant women and their newborns in Liwan District Maternal and Child Health Hospital of Guangzhou City were selected as the research objects. Seroepidemiological method was used to analyze the level of neutralizing antibody against poliomyelitis of puerpera and their newborn infants at the age of 3 months and 6 months, and to divide them into two groups according to the basic immune program of infants: (OPV) group with attenuated live poliomyelitis vaccine. The antibody levels of inactivated poliomyelitis vaccine (IPV) and IPV OPV were compared. Results Peripheral venous blood was collected from 179 puerperas. The level of neutralizing antibody against poliomyelitis type III in newborns was lower than that in puerpera. The level of antibody increased significantly at the age of 3 to 6 months. At the age of 6 months, the antibody levels were as high as 607.0 (95%CI=146.0~2 523.1) / 239.6 (95%CI=80.4~713.6) and 235.9 (95%CI=56.0~994.8) .OPVV IPV / IPV OPV respectively. The geometric mean titers of II-IIIII antibody were inverted in the infants at all time periods. The number of (GMRT) and antibody positive rate were not significantly different (P0.05).
