【摘要】：目的分析纖維支氣管鏡清洗消毒機(jī)消毒失敗原因,提出針對性改進(jìn)措施,達(dá)到全自動內(nèi)鏡清洗消毒機(jī)消毒效能最佳化。方法 2013-2014年按國家規(guī)范對內(nèi)鏡內(nèi)腔面采樣,針對7例陽性標(biāo)本進(jìn)行細(xì)菌學(xué)鑒定,從內(nèi)鏡預(yù)處理、設(shè)備維護(hù)管理、消毒劑管理、人員培訓(xùn)4個(gè)方面查找失敗原因,分別從管理、培訓(xùn)、標(biāo)準(zhǔn)流程的制定、設(shè)備清洗保養(yǎng)等方面提出針對性改進(jìn)措施。結(jié)果通過根因分析,發(fā)現(xiàn)主要原因?yàn)槿藛T培訓(xùn)不到位、操作人員對設(shè)備過度依賴、未實(shí)施有效預(yù)清洗、全自動清洗消毒機(jī)自身清潔消毒不到位、未建立標(biāo)準(zhǔn)化操作及維護(hù)保養(yǎng)流程等;通過建立內(nèi)鏡首洗負(fù)責(zé)制、設(shè)備預(yù)防性維護(hù)計(jì)劃、標(biāo)準(zhǔn)化操作及管理流程、強(qiáng)化人員培訓(xùn)4項(xiàng)改進(jìn)措施,進(jìn)行針對性管理,纖維支氣管內(nèi)鏡清洗消毒后期采樣合格率達(dá)標(biāo)。結(jié)論全自動內(nèi)鏡清洗消毒機(jī)運(yùn)用過程必須加強(qiáng)預(yù)處理、設(shè)備的維護(hù)保養(yǎng)、注重人員的培訓(xùn)和標(biāo)準(zhǔn)化流程的建立,才能確保清洗消毒效果。
[Abstract]:Objective to analyze the causes of failure in disinfection of fiberoptic bronchoscope cleaning and disinfecting machine, and to put forward corresponding improvement measures to optimize the disinfection efficacy of automatic endoscope cleaning and disinfection machine. Methods Endoscopic endoscopes were sampled according to the national standard in 2013-2014. Seven positive specimens were identified by bacteriology. The reasons of failure were found from four aspects: endoscope pretreatment, equipment maintenance management, disinfectant management and personnel training. Training, standard process development, equipment cleaning and maintenance and other aspects of targeted improvement measures. Results through root cause analysis, it was found that the main reasons were that the personnel training was not in place, the operators were too dependent on the equipment, the effective pre-cleaning was not carried out, and the automatic cleaning and disinfecting machine itself was not in place. No standardized operation and maintenance process has been established, through the establishment of endoscopic first wash responsibility system, equipment preventive maintenance plan, standardized operation and management process, strengthen the personnel training of 4 improvement measures to carry out targeted management, the establishment of the first wash responsibility system, equipment preventive maintenance plan, standardized operation and management process, The qualified rate of sampling in the later stage of fiberoptic endoscopy cleaning and disinfection was up to standard. Conclusion Pretreatment, maintenance of equipment, training of personnel and establishment of standardized flow should be strengthened in the operation of automatic endoscopic cleaning and disinfecting machine in order to ensure the efficacy of cleaning and disinfection.
