【摘要】：目的研究骨科住院患者感染病原菌種類及其耐藥性,為臨床合理使用抗菌藥物提供參考。方法采用細菌分離培養(yǎng)和藥敏試驗方法,對某醫(yī)院骨科病房1 460份送檢標(biāo)本進行細菌鑒定及藥敏試驗。結(jié)果從1 460份標(biāo)本中共檢出病原菌860株,檢出率58.9%。在檢出的病原菌中,革蘭陽性球菌占66.0%,其中金黃色葡萄球菌,表皮葡萄球菌、腐生葡萄球菌居前3位;革蘭陰性桿菌占30.0%,主要包括銅綠假單胞菌、大腸埃希菌、鮑曼不動桿菌等;真菌占4.0%。骨科臨床分離革蘭陽性球菌對萬古霉素和替考拉寧敏感,革蘭陰性桿菌對亞胺培南敏感。結(jié)論骨科住院患者感染病原菌主要是革蘭陽性球菌,臨床分離的病原菌對常用抗菌藥物普遍耐藥,根據(jù)藥敏試驗結(jié)果選擇抗菌藥物是有效控制感染的關(guān)鍵。
[Abstract]:Objective to study the types and drug resistance of infectious pathogens in orthopaedic inpatients, and to provide reference for rational use of antimicrobial agents in clinic. Methods bacterial isolation, culture and drug sensitivity test were used to identify 1 460 specimens from orthopaedic ward of a hospital. Results A total of 860 strains of pathogenic bacteria were detected from 1 460 specimens, and the detection rate was 58.9%. Among the pathogens detected, gram-positive cocci accounted for 66.0, among which Staphylococcus aureus, Staphylococcus epidermidis and Staphylococcus saprophyticus ranked the top 3, Gram-negative bacilli accounted for 30.0%, mainly including Pseudomonas aeruginosa and Escherichia coli. Acinetobacter baumannii and so on; fungi account for 4.0. Gram-positive cocci isolated from orthopaedics were sensitive to vancomycin and teicoplanin, and Gram-negative bacilli were sensitive to imipenem. Conclusion Gram-positive cocci are the main pathogens in orthopedic inpatients. The pathogens isolated in clinic are generally resistant to common antimicrobial agents. The key to control infection is to select antimicrobial agents according to the results of drug sensitivity test.
