【摘要】： 目的:通過調(diào)查武漢市成年人胃腸疾病的患病率,探討胃腸疾病的危險(xiǎn)因素。 方法:以隨機(jī)分層整群抽樣的方法選擇調(diào)查地點(diǎn),在選取的城鄉(xiāng)調(diào)查點(diǎn),以單層隨機(jī)抽樣方法從所選調(diào)查點(diǎn)的居民名單中抽取18到80歲成年人共3600人。調(diào)查問卷為簡化的ROME II組合問卷和返流性疾病問卷。按照ROME II標(biāo)準(zhǔn)對(duì)各種胃腸疾病進(jìn)行診斷。計(jì)算胃腸疾病患病率并用多元Logistic回歸分析探討胃腸疾病的危險(xiǎn)因素。資料經(jīng)Epidata3.02建庫并用SAS8.2統(tǒng)計(jì)軟件進(jìn)行統(tǒng)計(jì)學(xué)分析。 結(jié)果:1、調(diào)查合格問卷3567份,既往胃腸疾病患病率為:胃食管反流病2.16%、消化不良6.57%、吞咽困難0.73%、胃炎11.21%、消化性潰瘍4.74%、膽囊疾病2.97%、腸易激綜合征0.20%、炎癥性腸病2.16%和食管癌胃癌0.06%。 2、ROME II標(biāo)準(zhǔn)下胃腸疾病患病率為17.41%,其中GERD的患病率為2.94%,反芻綜合征患病率為1.26%,功能性消化不良患病率為5.07%,吞氣癥患病率為2.47%,IBS患病率為4.12%,功能性腹脹患病率為4.29%,功能性便秘患病率為7.04%,功能性腹瀉患病率為0.73%。 3、不同性別、地區(qū)和年齡間胃腸疾病患病率存在差異。女性患病率高于男性(18.90%15.57%,P=0.0092),農(nóng)村患病率高于城市(20.74%13.95%,P0.0001),胃腸疾病的患病率有隨著年齡升高而升高的趨勢(P0.0001)�；加袃煞N以上胃腸疾病的人占患有胃腸疾病人群的37.38%。 4、在城市,學(xué)歷、總的健康狀況、從事日常生活的能力、精神狀態(tài)和家族遺傳史是胃腸疾病的危險(xiǎn)因素,調(diào)整OR值分別為0.861、1.610、1.254、1.292和1.427。在農(nóng)村,性別、年齡、總的健康狀況、工作中的勞動(dòng)強(qiáng)度、精神狀態(tài)和胃腸疾病家族史是胃腸疾病的危險(xiǎn)因素,調(diào)整OR值分別為1.370、1.154、1.312、1.121、1.186和1.558。 結(jié)論:武漢市羅馬Ⅱ標(biāo)準(zhǔn)診斷的胃腸疾病患病率為17.41%,男性15.57%,女性18.90%,城區(qū)13.95%,農(nóng)村20.74%�；疾÷逝愿哂谀行�,農(nóng)村高于城市。胃腸疾病的患病率有隨著年齡升高而升高的趨勢;胃腸疾病有疾病重疊現(xiàn)象;胃腸疾病各病種的患病率低于我國其它地區(qū);胃腸疾病患者的就診情況不容樂觀;居民的生活行為方式與胃腸疾病的發(fā)病密切相關(guān),胃腸疾病的發(fā)生是多種因素共同作用的結(jié)果。
[Abstract]:Objective: to investigate the prevalence of gastrointestinal diseases among adults in Wuhan, and to explore the risk factors of gastrointestinal diseases. A single-layer random sampling of 3600 adults aged 18 to 80 from a list of residents at selected sites was carried out. The questionnaire consisted of a simplified rome II questionnaire and a reflux disease questionnaire. Various gastrointestinal diseases were diagnosed according to rome II criteria. The prevalence of gastrointestinal diseases was calculated and the risk factors of gastrointestinal diseases were analyzed by multivariate logistic regression analysis. The data were analyzed by using statistical software SAS8.2 and Epidata 3.02. Results: 3567 qualified questionnaires were obtained. The prevalence rates of gastrointestinal diseases were: gastroesophageal reflux disease 2.16, dyspepsia 6.57, dysphagia 0.73, gastritis 11.21, peptic ulcer 4.74g, gallbladder disease 2.97cm, irritable bowel syndrome 0.20, inflammatory bowel disease 2.16% and esophageal carcinoma and stomach cancer 0.066.2 ROME II. The prevalence rate of GERD was 2.94, ruminant syndrome 1.26, functional dyspepsia 5.07, GERD 2.47 IBS 4.12, functional abdominal distension 4.29 and functional constipation 7.04, GERD 2.94, ruminant syndrome 1.26, functional dyspepsia 5.07, IBS 4.12, functional abdominal distension 4.29, functional constipation 7.04. The prevalence of active diarrhea was 0.73. 3, different sex, There were differences in the prevalence of gastrointestinal diseases between regions and ages. The prevalence of gastrointestinal diseases in women was higher than that in men (18.90) and in rural areas (P _ (0.0001). The prevalence rate of gastrointestinal diseases in rural areas was higher than that in urban areas (P _ (0.0001) and P _ (0.0001). People with more than two kinds of gastrointestinal diseases account for 37.38.4% of the population with gastrointestinal diseases. In cities, education, general health status, ability to engage in daily life, mental state and family genetic history are risk factors for gastrointestinal diseases. The adjusted OR values were 0.861 / 1.610 / 1.254 / 1.292 and 1.427, respectively. In rural areas, sex, age, general health status, labor intensity at work, mental status and family history of gastrointestinal disease are risk factors for gastrointestinal disease.
