【摘要】： 流感大流行是指在全球范圍暴發(fā)的流感。有證據(jù)表明,從2003年至今,流感大流行暴發(fā)的潛在風(fēng)險(xiǎn)越來(lái)越高。我國(guó)作為流感的多發(fā)地之一,積極應(yīng)對(duì)流感大流行尤為必要。 在流感大流行應(yīng)對(duì)中,我們不僅要解決醫(yī)學(xué)方面的問(wèn)題,也需要解決倫理學(xué)方面的問(wèn)題。探討其間出現(xiàn)的倫理學(xué)問(wèn)題及其可以得到倫理辯護(hù)的解決辦法,是制定流感大流行應(yīng)對(duì)政策和法律法規(guī)的基礎(chǔ),同時(shí)也為評(píng)價(jià)應(yīng)對(duì)行動(dòng)是否可以得到辯護(hù)提供了倫理學(xué)依據(jù)。本文從分析我國(guó)應(yīng)對(duì)流感大流行的必要性入手,在闡釋公共衛(wèi)生倫理學(xué)基本原則的基礎(chǔ)上,分析論證了流感大流行應(yīng)對(duì)中三個(gè)主要公共衛(wèi)生倫理學(xué)問(wèn)題,試圖為我國(guó)流感大流行應(yīng)對(duì)行動(dòng)提供一些探討性的倫理學(xué)建議。 本文首先分析了流感大流行在世界范圍,尤其在我國(guó)發(fā)生的可能性以及應(yīng)對(duì)的必要性,簡(jiǎn)要介紹了世界衛(wèi)生組織和我國(guó)衛(wèi)生部制定的流感大流行應(yīng)急預(yù)案的主要內(nèi)容。其次,提出了評(píng)價(jià)流感大流行應(yīng)對(duì)行動(dòng)時(shí)需要援引的公共衛(wèi)生倫理學(xué)基本原則,即效用原則、公正原則、尊重原則、互助原則和相稱性原則。接著,對(duì)流感大流行應(yīng)對(duì)行動(dòng)中的三個(gè)主要公共衛(wèi)生倫理學(xué)問(wèn)題,即:檢疫/隔離措施中的倫理學(xué)問(wèn)題、匱乏醫(yī)療資源分配中的倫理學(xué)問(wèn)題、醫(yī)護(hù)人員提供救治義務(wù)的倫理學(xué)問(wèn)題,進(jìn)行了分析論證。最后,研究了有關(guān)流感大流行應(yīng)對(duì)倫理學(xué)指南的國(guó)際經(jīng)驗(yàn),對(duì)我國(guó)流感大流行應(yīng)對(duì)行動(dòng)提出了倫理學(xué)建議。
[Abstract]:An influenza pandemic is a global outbreak of influenza. There is evidence that the potential risk of influenza pandemic outbreaks has increased since 2003. As one of the most common places of influenza, it is necessary to deal with influenza pandemic actively in China. In influenza pandemic response, we need to solve not only medical problems, but also ethical problems. The discussion of the ethical problems and the solutions to the ethical defense are the basis of formulating the policy, laws and regulations of influenza pandemic response, and also provide the ethical basis for evaluating whether the response action can be defended or not. Based on the analysis of the necessity of responding to influenza pandemic in China, and on the basis of explaining the basic principles of public health ethics, this paper analyzes and demonstrates three major public health ethics issues in influenza pandemic response. This paper attempts to provide some exploratory ethical suggestions for the response to influenza pandemic in China. This paper first analyzes the possibility of influenza pandemic in the world, especially in our country, and the necessity to deal with it, and briefly introduces the main contents of the pandemic emergency plan formulated by WHO and the Ministry of Health of China. Secondly, it puts forward the basic principles of public health ethics that need to be invoked in evaluating influenza pandemic response, that is, the principle of utility, the principle of justice, the principle of respect, the principle of mutual aid and the principle of proportionality. Then, there are three major public health ethical issues in the influenza pandemic response, namely, the ethical issues in quarantine / quarantine measures, and the ethical issues in the allocation of scarce medical resources. The ethical problems of medical staff providing medical treatment are analyzed and demonstrated. Finally, the international experience of the ethical guidelines for influenza pandemic response is studied, and some ethical suggestions are put forward for the influenza pandemic response in China.
