【摘要】：目的以甘肅省榆中縣2009年-2016年報(bào)告的法定傳染病病例為資料,對(duì)法定傳染病進(jìn)行描述性研究及趨勢(shì)分析,確定榆中縣重點(diǎn)防控的傳染病種類以及高危人群,為提出針對(duì)性的傳染病防控措施提供理論依據(jù)。方法從《中國(guó)疾病預(yù)防控制信息系統(tǒng)》中提取法定傳染病資料,核實(shí)原始資料的真實(shí)性。對(duì)法定傳染病資料從總體發(fā)病情況(發(fā)病率、發(fā)病順位、構(gòu)成)、死亡情況以及主要法定傳染病三間分布(人群分布,時(shí)間分布與地區(qū)分布)等方面進(jìn)行描述性研究及趨勢(shì)分析,對(duì)其中的計(jì)數(shù)資料進(jìn)行c2檢驗(yàn),a=0.05。結(jié)果1.總體發(fā)病情況甘肅省榆中縣2009年-2016年沒有甲類法定傳染病病例與乙類法定傳染病按照甲類管理的病例報(bào)告。8年間報(bào)告乙類與丙類法定傳染病共計(jì)12765例,年均報(bào)告發(fā)病率為362.37/10萬(wàn),總體呈波動(dòng)性降低態(tài)勢(shì)。報(bào)告法定傳染病年均發(fā)病率順位依次為:病毒性肝炎,肺結(jié)核,細(xì)菌性痢疾,其它感染性腹瀉與流行性腮腺炎。報(bào)告病例按照不同傳播途徑分類進(jìn)行統(tǒng)計(jì)分析,血源性及性傳播傳染病年均發(fā)病率位居首位,呼吸道傳染病年均發(fā)病率位居第2位,腸道傳染病年均發(fā)病率位居第3位,血源性及性傳播傳染病以病毒性肝炎病例報(bào)告病例為主,其報(bào)告病例占血源性及性傳播傳染病報(bào)告總病例的93.94%。2.法定傳染病死亡情況榆中縣2009年-2016年8年間報(bào)告死亡病例共12例,年均報(bào)告死亡率為0.34/10萬(wàn),病死率為0.09%。3.報(bào)告病例“三間分布”情況報(bào)告病例人群分布情況是,男性患者發(fā)病率高于女性(c2=655.713 p0.01),男女性別比為1.69:1;發(fā)病人群主要是轄區(qū)內(nèi)的農(nóng)民與18-歲年齡組人群;報(bào)告病例時(shí)間分布情況以5月、6月、7月法定傳染病多發(fā);報(bào)告病例地區(qū)分布以西南部山區(qū)居民法定傳染病的發(fā)病率高于東北部山區(qū)與中部川區(qū)居民發(fā)病率(c2=36.350 p0.01)。4.整體發(fā)病情況甘肅省榆中縣2009年-2016年8年間報(bào)告法定傳染病年均發(fā)病率低于同期國(guó)家(486.58/10萬(wàn))與甘肅省(390.57/10萬(wàn))發(fā)病率水平(c2=189.534 p0.01),報(bào)告年均死亡率亦低于同期國(guó)家(1.22/10萬(wàn))與甘肅省(0.49/10萬(wàn))死亡率水平(c2=107.957 p0.01)。結(jié)論1.甘肅省榆中縣法定傳染病報(bào)告病例的年均發(fā)病率總體呈波動(dòng)性降低態(tài)勢(shì);報(bào)告年均死亡率與病死率低于同期國(guó)家與甘肅省年均死亡率與病死率水平。2.甘肅省榆中縣法定傳染病防治的重點(diǎn)人群是18-歲年齡組、農(nóng)民與男性居民;5、6、7月份法定傳染病多發(fā);西南部山區(qū)居民法定傳染病的發(fā)病率高于東北部山區(qū)與中部川區(qū)居民發(fā)病率;病毒性肝炎、肺結(jié)核、細(xì)菌性痢疾、其它感染性腹瀉與流行性腮腺炎是重點(diǎn)防控的法定傳染病。
[Abstract]:Objective based on the reported cases of legal infectious diseases in Yuzhong County, Gansu Province from 2009 to 2016, the descriptive study and trend analysis of legal infectious diseases were carried out to determine the types of infectious diseases and the high risk population in Yuzhong County, Gansu Province. To provide theoretical basis for the prevention and control of infectious diseases. Methods the data of legal infectious diseases were extracted from China Disease Prevention and Control Information system to verify the authenticity of the original data. The general incidence (incidence, sequence of incidence, constitution, death and distribution of the main legal infectious diseases) (population distribution, time distribution and regional distribution) of the legal infectious diseases were studied and their trends were analyzed. The counting data were examined by c2 test. Result 1. The overall incidence of infectious diseases in Yuzhong County, Gansu Province, from 2009 to 2016, there were no cases of Class A statutory infectious diseases and Class B statutory infectious diseases reported in accordance with Category A cases. During the period of 8 years, a total of 12765 cases of Class B and Class C legal infectious diseases were reported, with an average annual reported incidence rate of 3,62.37% / 100, 000. Overall volatility decreased. The annual incidence of infectious diseases was ranked as viral hepatitis, tuberculosis, bacillary dysentery, other infectious diarrhea and mumps. The reported cases were statistically analyzed according to different transmission routes. The annual incidence of blood-borne and sexually transmitted infectious diseases ranked first, the annual incidence of respiratory infections ranked second, and the annual incidence of enteric diseases ranked third. The main reported cases of hematogenous and sexually transmitted infectious diseases were viral hepatitis cases, and the reported cases accounted for 93.94% of the total reported cases of blood-borne and sexually transmitted infectious diseases. Death status of legal Infectious Diseases in Yuzhong County from 2009 to 2016, 12 deaths were reported, with an average annual reported mortality rate of 0.34 / 100, 000 and a mortality rate of 0.09 percent and a mortality rate of 0.09 percent. The incidence of male patients was higher than that of females (655.713 p0.01g), and the ratio of male to female was 1.69: 1. The time distribution of reported cases was more frequent in May, June and July, and the incidence of statutory infectious diseases in the southern mountainous areas west of the reported cases was higher than that in the northeast and central Sichuan areas. The overall incidence of infectious diseases in Yuzhong County, Gansu Province, from 2009 to 2016 was lower than that reported in the same period (486.58% / 100 000) and in Gansu Province (3,90.57% / 100 000). The reported annual mortality rate was also lower than that of the same period. ) and Gansu Province (0.49 / 100 000) mortality level of 107.957 p0.01g. Conclusion 1. The average annual incidence of reported cases of statutory infectious diseases in Yuzhong County of Gansu Province was fluctuating, and the reported annual mortality and mortality were lower than those of other countries and Gansu Province in the same period. In Yuzhong County, Gansu Province, the key population for the prevention and control of statutory infectious diseases was the 18-year-old age group, and the incidence of statutory infectious diseases among farmers and male residents was more frequent in July than that in the northeast and central Sichuan areas, and the incidence of statutory infectious diseases in the southwest mountainous areas was higher than that in the northeast and central Sichuan regions. Viral hepatitis, tuberculosis, bacillary dysentery, other infectious diarrhoea and mumps are key legal infectious diseases.
