【摘要】：目的系統(tǒng)評價聯(lián)合乙型肝炎(以下簡稱乙肝)免疫球蛋白和乙型肝炎疫苗阻斷乙肝表面抗原(hepatitis B surface antigen,HBs Ag)陽性母親對其新生兒造成母嬰傳播的阻斷效果,為新生兒乙肝預(yù)防提供理論證據(jù)。方法檢索文獻數(shù)據(jù)庫,收集以聯(lián)合免疫為主要干預(yù)措施,對HBs Ag陽性和(或)HBs Ag、乙肝e抗原(hepatitis B e antigen,HBe Ag)雙陽性的慢性乙肝孕婦及其所生的新生兒進行免疫治療的隨機對照試驗(randomized controlled trials,RCT),運用Revman 5.0進行Meta分析。結(jié)果共納入6項研究,試驗組426人,對照組373人。結(jié)果顯示,聯(lián)合乙肝疫苗和免疫球蛋白使用相比單獨使用疫苗獲得更佳的母嬰傳播阻斷效果,其嬰兒HBs Ag感染風(fēng)險低,且差異有統(tǒng)計學(xué)意義(RR=0.23,95%CI:0.15~0.34,P0.001);亞組分析顯示,無論感染狀態(tài),聯(lián)合疫苗干預(yù)均能在單陽性或雙陽性HBs Ag感染狀態(tài)的母親中取得較好的母嬰傳播阻斷效果(RR合并單陽=0.23,95%CI:0.07~0.71,P=0.010,RR合并雙陽=0.22,95%CI:0.13~0.38,P0.001)。結(jié)論聯(lián)合使用乙肝免疫球蛋白和乙肝疫苗更能降低新生兒HBs Ag感染率。但受研究樣本限制,仍需大樣本和高質(zhì)量的RCT試驗來證實。
[Abstract]:Objective to evaluate the effect of combined hepatitis B immunoglobulin and hepatitis B vaccine on the blocking of mother-to-child transmission in their newborns by blocking hepatitis B surface antigen-HBs Ags positive mothers. Methods to search the literature database and collect the combined immunization as the main intervention measures for the prevention of neonatal hepatitis B. A randomized controlled trial of immunotherapy for chronic hepatitis B pregnant women and their newborns with HBs Ag positive and (or) double positive HBs Ag and hepatitis be antigen-HBe Agwas carried out by using Revman 5.0 for Meta analysis. Results six studies were carried out. There were 426 patients in the trial group and 373 in the control group. The results showed that the combination of hepatitis B vaccine and immunoglobulin had better blocking effect of mother-to-child transmission than that of the vaccine alone, and the risk of infantile HBs Ag infection was lower. And the difference was statistically significant (RRX 0.2395 CI: 0.150.34 P0.001). Subgroup analysis showed that, regardless of the infection status, The combined vaccine intervention could obtain better blocking effect of mother-to-child transmission in mothers with single or double positive HBs Ag infection. Conclusion combined use of hepatitis B immunoglobulin and hepatitis B vaccine can be more effective in combination with 0.2395 CI: 0.07 + 0.71% P0. 010 + RR combined with double positive HBs Ag infection. Conclusion the combined use of hepatitis B immunoglobulin and hepatitis B vaccine is more effective. To reduce the infection rate of HBs Ag in neonates. Large samples and high quality RCT tests are still required.
