【摘要】：目的了解河南省中醫(yī)系統(tǒng)醫(yī)院感染現(xiàn)狀,為控制醫(yī)院感染、提高醫(yī)療服務(wù)質(zhì)量提供決策依據(jù)。方法按照原衛(wèi)生部《醫(yī)院感染監(jiān)測(cè)規(guī)范》的要求,采用查閱住院病歷和床旁調(diào)查的方法,于2013年5月20日在河南省60所中醫(yī)院開展醫(yī)院感染現(xiàn)患率調(diào)查,借助全國(guó)醫(yī)院感染監(jiān)控管理培訓(xùn)基地的網(wǎng)絡(luò)直報(bào)系統(tǒng)收集數(shù)據(jù)。結(jié)果 60所中醫(yī)院應(yīng)查18 112例,實(shí)查18 031例,實(shí)查率99.55%;醫(yī)院感染平均現(xiàn)患率為2.21%,綜合ICU醫(yī)院感染現(xiàn)患率最高,為23.68%;Ⅰ類手術(shù)切口患者手術(shù)部位醫(yī)院感染平均現(xiàn)患率為0.74%;醫(yī)院感染現(xiàn)患率按科室排列由高到低為:綜合ICU、兒科新生兒組、神經(jīng)外科、胸外科、血液病科、腫瘤科、神經(jīng)內(nèi)科、內(nèi)分泌科、普外科、消化科;不同規(guī)模醫(yī)院醫(yī)院感染現(xiàn)患率差異無統(tǒng)計(jì)學(xué)意義;所有調(diào)查醫(yī)院抗菌藥物使用率平均為35.24%,不同規(guī)模醫(yī)院抗菌藥物使用率差異有統(tǒng)計(jì)學(xué)意義(P0.05);除兒科及綜合ICU外,科室抗菌藥物使用率前十位均為外科。結(jié)論中醫(yī)系統(tǒng)內(nèi)綜合ICU為醫(yī)院感染高�？剖�;醫(yī)院規(guī)模越大,抗菌藥物使用率越低;中醫(yī)系統(tǒng)醫(yī)療機(jī)構(gòu)依然需要加強(qiáng)醫(yī)院感染管理工作。
[Abstract]:Objective to understand the present situation of hospital infection in traditional Chinese medicine (TCM) system in Henan Province, and to provide decision basis for controlling nosocomial infection and improving the quality of medical service. In May 20th 2013, a survey on the prevalence of nosocomial infection was carried out in 60 Chinese medicine hospitals in Henan Province by means of consulting hospital records and bedside surveys. Results 18 112 cases should be checked in 60 Chinese medicine hospitals, 18 031 cases should be checked, and the actual investigation rate was 99.550.The average prevalence rate of hospital infection was 2.21, and the nosocomial infection rate of comprehensive ICU was the highest. The average incidence rate of nosocomial infection in patients with type I surgical incision was 0.74. The incidence rate of nosocomial infection ranged from high to low by department of department: integrated ICU, pediatric neonatal group, neurosurgery, thoracic surgery, hematological department, oncology, neurology, neurology. There was no significant difference in nosocomial infection prevalence rate among different size hospitals in endocrine department, general surgery department and digestive department. The average utilization rate of antimicrobial agents in all hospitals investigated was 35.24. There were significant differences in the utilization rates of antimicrobial agents between hospitals of different sizes (P 0.05), except paediatrics and comprehensive ICU. Conclusion Comprehensive ICU in TCM system is high risk department for nosocomial infection; the larger the hospital scale, the lower the utilization rate of antimicrobial agents; and the more hospital infection management work should be strengthened in the medical institutions of TCM system.
