【摘要】： 目的了解寧夏傷害住院患者的傷害分布特征和疾病負(fù)擔(dān),為傷害的預(yù)防控制提供理論依據(jù)。方法先采用分層隨機(jī)抽樣方法。按地理位置將寧夏分為4個(gè)地區(qū)(銀北、銀川、銀南和固原),在每個(gè)地區(qū)所有縣級(jí)以上綜合性醫(yī)院中隨機(jī)抽取1家市級(jí)醫(yī)院、1家縣級(jí)醫(yī)院,銀川地區(qū)在此基礎(chǔ)上,再在其獨(dú)有的區(qū)級(jí)醫(yī)院中隨機(jī)抽取一所,共計(jì)抽取9家醫(yī)院。再采用系統(tǒng)抽樣方法,分別對(duì)每所醫(yī)院所摘錄的病案按病案號(hào)排序后各抽取出50%的出院日期為2006年1月1日至2006年12月31日,并據(jù)《ICD-10損傷與中毒外部原因分類》診斷明確的傷害住院病例共5906例作為研究對(duì)象。結(jié)果1.傷害患者中以文化程度較低者所占比例較大;被調(diào)查者中男性多于女性(2.59:1),但自殺/自殘者女性多于男性(1.26:1);15～歲組和35～歲組人數(shù)合計(jì)占總傷害患者的76.90%;傷害原因位于前三位的是交通事故(40.30%)、他人加害(18.46%)和跌倒/墜落(17.52%);60～歲組第一位傷害原因是跌倒/墜落(38.74%);燒燙傷患者占0～歲組人數(shù)的23.66%;職業(yè)位于前三位的分別是農(nóng)民(45.95%)、工人(21.39%)和學(xué)生(11.56%)。城鄉(xiāng)比為1:1.31;死亡47例,其中因交通事故死亡占53.19%,頭部受傷占51.06%。2.經(jīng)濟(jì)損失次均7436.18元,總經(jīng)濟(jì)損失4391.81萬(wàn)元。次均經(jīng)濟(jì)損失位于前三位的分別是燒燙傷、自殺/自殘和交通事故。銀北地區(qū)、下肢傷、肌肉和肌腱損傷、住院天數(shù)和醫(yī)療費(fèi)用5個(gè)因素對(duì)住院期間主要經(jīng)濟(jì)損失有影響,OR值分別為1.763、2.001、16.760、1.092和1.003。平均每位死亡患者潛在壽命損失為38.30年,潛在工作損失30.00年、潛在經(jīng)濟(jì)損失353520.00元,潛在價(jià)值損失39.66年。結(jié)論寧夏傷害住院患者的分布特征明確,疾病負(fù)擔(dān)嚴(yán)重,應(yīng)盡早采取措施進(jìn)行預(yù)防和控制。
[Abstract]:Objective to understand the characteristics of injury distribution and disease burden of hospitalized patients in Ningxia, and to provide theoretical basis for prevention and control of injury. Methods the method of stratified random sampling was used to divide Ningxia into 4 regions (Yinbei, Yinchuan) according to their geographical location. In Yinnan and Guyuan Yuan, one city hospital and one county level hospital were randomly selected from all general hospitals at and above the county level in each region. On this basis, one hospital was randomly selected from its unique district hospital in Yinchuan. A total of 9 hospitals were selected. Then, by systematic sampling, 50% medical records were extracted from each hospital from January 1st 2006 to December 31st 2006. According to the classification of external causes of ICD-10 injury and poisoning, a total of 5906 injuries were diagnosed as inpatients. Results 1.The proportion of the injured patients with lower education level was higher than that of the patients with lower education level. Men were more than women 2.59: 1, but women who committed suicide / self-disability accounted for 76.90% of the total number of injured patients. The total number of persons aged 15 to 35 accounted for 76.90% of the total injuries caused by traffic accidents (40.30%) and falls / falls / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall / fall. The first cause of injury in the 60-year-old group was fall / fall 38.74; burn patients accounted for 23.66% of the 0-year-old group; the first three occupations were peasants 45.9595 and workers 21.399.The ratio between urban and rural areas was 1: 1.31; the death rate was 47 cases. Of these, 53.19 were killed in traffic accidents, 51.06.2. the economic losses averaged 7436.18 yuan, and the total economic losses were forty-three million nine hundred and eighteen thousand and one hundred yuan. The average economic losses in the first three places were burning and scalding, suicide / self-mutilation, and traffic accidents, respectively. In Yinbei area, the lower limbs were injured. The OR values of muscle and tendon injury, length of stay in hospital and medical cost were 1.763t 2.001g 16.7601,1.092 and 1.003.The average potential life loss per dead patient was 38.30 years, and the potential job loss was 30.0 years. The potential economic loss is 353520.00 yuan and the potential value loss is 39.66 years. Conclusion the distribution characteristics of the inpatients with injury in Ningxia are clear and the burden of disease is serious. Measures should be taken to prevent and control it as soon as possible.
