【摘要】：目的系統(tǒng)評(píng)價(jià)華法林對(duì)拔牙術(shù)后出血的影響。方法計(jì)算機(jī)檢索Pub Med、EMbase、The Cochrane Library(2016年9期)、CNKI、VIP和Wan Fang Data數(shù)據(jù)庫(kù)以及中國(guó)國(guó)家藥品不良反應(yīng)監(jiān)測(cè)系統(tǒng)和美國(guó)FDA藥品不良反應(yīng)監(jiān)測(cè)系統(tǒng),搜集關(guān)于華法林對(duì)拔牙術(shù)后出血影響的隨機(jī)對(duì)照試驗(yàn)和隊(duì)列研究,檢索時(shí)限均為建庫(kù)至2016年9月30日。由兩位評(píng)價(jià)者獨(dú)立篩選文獻(xiàn)、提取資料和評(píng)價(jià)納入研究的偏倚風(fēng)險(xiǎn)后,采用Rev Man 5.3軟件進(jìn)行Meta分析。結(jié)果共納入8個(gè)研究,其中3個(gè)為RCTs,5個(gè)為隊(duì)列研究。Meta分析結(jié)果顯示:停用或減量華法林組與不停用華法林組在拔牙術(shù)后出血并發(fā)癥發(fā)生率方面,其差異無(wú)統(tǒng)計(jì)學(xué)意義[RCTs:RR=0.86,95%CI(0.49,1.51),P=0.60;隊(duì)列研究:RR=0.67,95%CI(0.45,1.01),P=0.06]。結(jié)論當(dāng)前證據(jù)表明,停服華法林與否和拔牙術(shù)后出血并發(fā)癥并無(wú)具有統(tǒng)計(jì)學(xué)意義的相關(guān)性。受納入研究數(shù)量和質(zhì)量的限制,上述結(jié)論尚待更多高質(zhì)量研究予以驗(yàn)證。
[Abstract]:Objective To evaluate the effect of warfarin on post-extraction bleeding. Methods A randomized controlled trial and cohort study on the effects of warfarin on post-extraction bleeding was collected by computer-based search for Pub Med, EMbase, The Cochrane Library (September 2016), the CNKI, the VIP and the Wan Fang Data database, as well as the China National Drug Reaction Monitoring System and the US FDA ADR Monitoring System. The search time limit is from the construction bank to September 30,2016. A Meta-analysis was conducted using the Rev Man 5.3 software, following the independent screening of the literature, extraction data and evaluation of the bias risk of the study by the two evaluators. The results were included in 8 studies, of which 3 were RCTs and 5 were cohort studies. The results of the meta-analysis showed that there was no significant difference in the incidence of bleeding complications after tooth extraction (RCTs: RR = 0.86,95% CI (0.49, 1.51), P = 0.60; cohort study: RR = 0.67,95% CI (0.45, 1.01), P = 0.06]. Conclusion The present evidence indicates that there is no statistically significant correlation between the presence of warfarin and the complications of bleeding after tooth extraction. The above conclusions have yet to be validated by a greater number of high-quality studies, subject to the limitations of the number and quality of studies included.
