【摘要】：目的:探討服用阿司匹林者在上頜牙周炎患牙拔除后正常凝血所需的血小板聚集率(PAgT)水平和術(shù)前合理的停藥天數(shù)。方法:選取90例接受單顆上頜重度牙周炎患牙拔除的服藥者(100 mg/d),分為前牙組(A組)、前磨牙組(B組)和磨牙組(C組),每組30例,包括術(shù)前不停藥、停藥3d和5d者各10例。對(duì)其行以花生四烯酸(AA)為誘導(dǎo)劑的光學(xué)法血小板聚集率檢測(LTA)后實(shí)施拔牙,根據(jù)拔牙創(chuàng)凝血分級(jí)表(Ⅰ級(jí)為正常,Ⅱ、Ⅲ級(jí)出血量依次增加)評(píng)估病例,分析術(shù)前不停藥、不同停藥時(shí)間對(duì)創(chuàng)面凝血的影響,采用受試者工作特征曲線(ROC curve,SPSS11.6)探討使用PAgT(AA誘導(dǎo)值)預(yù)判術(shù)后正常凝血的最佳診斷界值。結(jié)果:1Ⅱ級(jí)病例在A組中為3例不停藥和1例停藥3d者,在B組中為5例不停藥、2例停藥3d和1例停藥5d者,在C組中為6例不停藥、4例停藥3d和1例停藥5 d者;3組其余病例為Ⅰ級(jí)。2A、B、C組AA誘導(dǎo)值最佳診斷界值分別為10.15%、13.25%和16.5%。結(jié)論:服用阿司匹林的患者(100 mg/d)在拔除單顆上頜前牙、前磨牙、磨牙重度牙周炎患牙前經(jīng)LTA-AA檢測,如PAgT≥10.15%、13.25%和16.5%,可預(yù)計(jì)拔牙創(chuàng)凝血正常,術(shù)前可不停藥;反之出血量增加,前牙病例可停藥3 d,前磨牙、磨牙病例可停藥5 d,并完善創(chuàng)面止血措施。
[Abstract]:Objective: to investigate the (PAgT) level of platelet aggregation rate and the reasonable days of drug withdrawal before and after tooth extraction in patients with maxillary periodontitis.
