【摘要】：目的:研究在顯微根尖手術(shù)治療難治性慢性根尖周炎時(shí)應(yīng)用MTA的可行性、特點(diǎn)、成功率及相關(guān)影響因素。方法:分析2013年1月~2015年12月間本院牙體牙髓科完成的顯微根尖外科手術(shù)106例計(jì)146顆難治性慢性根尖周炎患牙的臨床資料,其中上頜前牙75例、下頜前牙13例、上頜前磨牙6例、下頜前磨牙4例、上頜磨牙4例、下頜磨牙4例。手術(shù)包括根尖切除、根尖倒預(yù)備、根尖倒充填主要步驟,術(shù)后1年回訪,通過(guò)臨床癥狀和放射學(xué)檢查評(píng)估療效,并對(duì)預(yù)后影響因素進(jìn)行卡方檢驗(yàn)分析。結(jié)果:顯微根尖外科手術(shù)聯(lián)合MTA應(yīng)用的1年成功率達(dá)95.9%(140/146),根據(jù)患者性別、年齡、牙位、根尖病灶范圍分別統(tǒng)計(jì)手術(shù)療效,組間差異均無(wú)統(tǒng)計(jì)學(xué)意義。失敗5例6顆患牙,主要原因?yàn)楦奂昂喜⒅囟妊乐苎住＝Y(jié)論:在治療難治性慢性根尖周炎時(shí)采用顯微根尖手術(shù)聯(lián)合MTA應(yīng)用可取得滿意效果;顯微根尖手術(shù)是治療慢性根尖周炎的有效手段。
[Abstract]:Objective: to study the feasibility, characteristics, success rate and related influencing factors of MTA in the treatment of refractory chronic periapical periodontitis. Methods: the clinical data of 146 refractory chronic periapical periodontitis teeth were analyzed from January 2013 to December 2015 in 106 patients with refractory chronic periapical periodontitis, including 75 cases of maxillary anterior teeth and 13 cases of mandibular anterior teeth. There were 6 cases of maxillary premolars, 4 cases of mandibular premolars, 4 cases of maxillary molars and 4 cases of mandibular molars. The operation included apical resection, apical inversion preparation and apical filling. 1 year after operation, the curative effect was evaluated by clinical symptoms and radiology, and the prognostic factors were analyzed by chi-square test. Results: the 1-year success rate of microapical surgery combined with MTA was 95.9% (140 脳 146). According to the sex, age, tooth position and the range of apical lesions, there was no significant difference between the two groups. 5 cases (6 teeth) failed, the main causes were root fracture and severe periodontitis. Conclusion: microapical surgery combined with MTA can achieve satisfactory results in the treatment of refractory chronic periapical periodontitis, and microapical surgery is an effective method for the treatment of chronic periapical periodontitis.
