山東地區(qū)成人下頜前牙與牙槽骨位置關(guān)系的CBCT研究

【摘要】:目的: 牙不是垂直地排列在牙槽骨中,而是具有一定的傾斜方向和傾斜角度。同時(shí)臨床工作中我們也發(fā)現(xiàn)下頜前牙與頜骨前端牙槽突的傾斜方向存在著一定的角度。本研究通過(guò)對(duì)山東地區(qū)成年人群下頜牙列完整者的CBCT影像前牙區(qū)進(jìn)行測(cè)量,初步探討下頜前牙與牙槽骨在正中矢狀面上的位置關(guān)系,為醫(yī)師的臨床治療,特別是下頜前牙區(qū)的美學(xué)治療提供參考。 方法: 從山東大學(xué)口腔醫(yī)院2012年12月份至2013年12月份所拍攝的口腔CT影像庫(kù)中選取150例(男女各75例,年齡段為19-48歲)合適的CBCT影像,并分為六個(gè)組:男性下頜中切牙、男性下頜側(cè)切牙、男性下頜尖牙、女性下頜中切牙、女性下頜側(cè)切牙、女性下頜尖牙。在CBCT影像上選擇合適的矢狀面,確定標(biāo)志點(diǎn),對(duì)下頜前牙牙槽嵴頂厚度、根尖距唇舌側(cè)骨板的最短距離及下頜前牙牙槽骨厚度進(jìn)行測(cè)量;確定牙體長(zhǎng)軸和牙槽骨長(zhǎng)軸,測(cè)量下頜前牙牙體長(zhǎng)軸相對(duì)于牙槽骨長(zhǎng)軸的傾斜角度(偏向舌側(cè)為正,偏向唇側(cè)為負(fù))。使用SPSS軟件對(duì)數(shù)據(jù)進(jìn)行統(tǒng)計(jì)分析,P0.05具有統(tǒng)計(jì)學(xué)差異。 結(jié)果: (1)線(xiàn)距測(cè)量結(jié)果:男性下頜中切牙牙槽嵴頂厚度、牙槽骨厚度、根尖到唇側(cè)骨板的最短距離、根尖到舌側(cè)骨板的最短距離平均為6.54mm、7.51mm、3.36mm、4.41mm;男性下頜側(cè)切牙平均為7.28mm、8.19mm、3.67mm、4.72mm;男性下頜尖牙平均為9.11mm、10.61mm、4.61mm、6.19mm。女性下頜中切牙牙槽嵴頂厚度、牙槽骨厚度、根尖到唇側(cè)骨板的最短距離、根尖到舌側(cè)骨板的最短距離平均為6.16mm、6.72mm、2.83mm、4.13mm;女性下頜側(cè)切牙平均為6.86mm、7.35mm、3.28mm、4.26mm;女性下頜尖牙平均為8.34mm、9.17mm、3.70mm、5.68mm。 (2)角度測(cè)量結(jié)果:男性下頜中切牙、側(cè)切牙、尖牙牙體長(zhǎng)軸相對(duì)于牙槽骨長(zhǎng)軸偏向唇/舌側(cè)的角度值平均為4.77°、6.12°、5.01°,女性下頜中切牙側(cè)切牙、尖牙牙體長(zhǎng)軸相對(duì)于牙槽骨長(zhǎng)軸偏向唇/舌側(cè)的角度值平均為5.38°5.81°、6.21°。男性與女性下頜前牙牙體長(zhǎng)軸相對(duì)于牙槽骨長(zhǎng)軸偏向舌側(cè)的角度值均不超過(guò)20°,偏向唇側(cè)的角度均不超過(guò)10° (3)性別差異:男性與女性在下頜中切牙、側(cè)切牙、尖牙的牙槽嵴頂厚度、牙槽骨厚度、根尖到唇側(cè)骨板的最短距離、根尖到舌側(cè)骨板的最短距離上有統(tǒng)計(jì)學(xué)差異(P0.05)。男性與女性在下頜中切牙、下頜側(cè)切牙牙體長(zhǎng)軸相對(duì)于牙槽骨長(zhǎng)軸偏向唇/舌側(cè)的角度上無(wú)統(tǒng)計(jì)學(xué)差異(P0.05);男性與女性在下頜尖牙牙體長(zhǎng)軸相對(duì)于牙槽骨長(zhǎng)軸偏向唇/舌側(cè)的角度上有統(tǒng)計(jì)學(xué)差異(P0.05) 結(jié)論: 下頜前牙與頜骨前端牙槽突的傾斜方向不一致,其牙體長(zhǎng)軸與牙槽骨長(zhǎng)軸存在著角度,其中80%以上牙體長(zhǎng)軸相對(duì)于牙槽骨長(zhǎng)軸偏向舌側(cè)。同時(shí)牙體長(zhǎng)軸相對(duì)于牙槽骨長(zhǎng)軸偏向舌側(cè)的角度值不超過(guò)20°,偏向唇側(cè)的角度不超過(guò)10°。 下頜前牙牙槽嵴頂厚度自下頜中切牙、下頜側(cè)切牙、下頜尖牙依次增厚,與下頜前牙頸部頰舌徑由前向后的遞增趨勢(shì)類(lèi)似,下頜前牙牙槽骨厚度亦如是。 下頜前牙根尖到舌側(cè)骨板的最短距離大于下頜前牙根尖到唇側(cè)骨板的最短距離。 牙體長(zhǎng)軸相對(duì)于牙槽骨長(zhǎng)軸偏向唇舌側(cè)的角度關(guān)系在下頜中切牙、下頜側(cè)切牙不存在性別差異,但在下頜尖牙存在性別差異。牙槽嵴頂厚度、牙槽骨厚度、根尖到唇側(cè)骨板的最短距離、根尖到舌側(cè)骨板的最短距離均具有性別差異。 同時(shí)依據(jù)CBCT可以準(zhǔn)確地提供下頜前牙在正中矢狀面上與牙槽骨的位置和角度關(guān)系,為下頜前牙的美學(xué)治療特別是種植和正畸治療提供參考,避免并發(fā)癥的發(fā)生。
