【摘要】：目的:手術(shù)先行方案治療牙頜面畸形是目前的研究熱點(diǎn)。在采用手術(shù)先行方案的治療中,發(fā)現(xiàn)正頜手術(shù)后3-4個(gè)月牙齒加速移動(dòng),提出了正頜手術(shù)致頜面部損傷加快牙齒移動(dòng)的觀點(diǎn)。本文通過(guò)實(shí)驗(yàn)分析手術(shù)先行方案中術(shù)后牙齒快速移動(dòng)的原因。材料與方法:12只成年新西蘭大白兔,分為AB兩組,每組6只,A組為實(shí)驗(yàn)組,B組為對(duì)照組,A以兔左下頜骨建立手術(shù)先行方案模型,術(shù)后以75 gf為初始力移動(dòng)左下頜第一前臼齒向前。B組僅用75 gf初始力移動(dòng)左下頜第一前臼齒向前。結(jié)果:A組中左下頜第一前臼齒移動(dòng)距離平均為0.96 mm,B組為0.91 mm,兩組間無(wú)明顯差異,p=0.36。結(jié)論:實(shí)驗(yàn)中對(duì)下頜骨的損傷并不明顯引起術(shù)后下頜牙齒快速移動(dòng),局部因素包括咬合功能減弱、牙頜面關(guān)系改善致生物力改變等的影響在采用手術(shù)先行方案時(shí)的不可忽視。
[Abstract]:Objective: surgical treatment of maxillofacial deformities is a hot topic at present. In the treatment of maxillofacial injury caused by orthognathic surgery, the accelerated tooth movement was found at 3 months and 4 months after orthognathic surgery, and the viewpoint was put forward that the maxillofacial injury caused by orthognathic surgery could accelerate tooth movement. In this paper, the causes of rapid tooth movement during and after operation were analyzed experimentally. Materials and methods: twelve adult New Zealand white rabbits were divided into two groups: AB group (n = 6), A group (n = 6), B group (n = 6) and control group (n = 6). The first premolar of the left mandible was moved forward with 75 gf as the initial force, and only the first premolar of the left mandible was moved forward with the initial force of 75 gf in group B. Results: in group A, the mean distance of movement of left mandibular first premolars was 0.96 mm,B and 0.91 mm, but there was no significant difference between the two groups (p = 0.36). Conclusion: the mandibular injury in the experiment did not cause the rapid movement of the mandibular teeth after operation. The local factors including the weakening of the occlusal function and the change of the biological power caused by the improvement of the relationship between the teeth and the maxillofacial surface should not be ignored when the operation was carried out.
