骨性Ⅱ類不同垂直骨面型的顱面骨三維形態(tài)特征研究

【摘要】:目的本研究采用CBCT為研究手段,對不同垂直骨面型的成年骨性II類錯鄈畸形患者的顱面骨組織進(jìn)行三維測量分析,將高角組、均角組及低角組各項(xiàng)測量指標(biāo)進(jìn)行對比,總結(jié)出不同垂直骨面型錯鄈畸形患者的顱面骨三維結(jié)構(gòu)特點(diǎn),為正畸患者的診斷、治療及預(yù)后提供科學(xué)的解剖學(xué)基礎(chǔ)和理論支持。材料與方法按一定納入標(biāo)準(zhǔn)選取2012年6月至2014年9月到天津市口腔醫(yī)院正畸科就診的骨性II類高角、均角、低角患者治療前錐形束CT圖像各30組。選取下頜骨測量項(xiàng)目10項(xiàng),顱顴上頜復(fù)合體測量項(xiàng)目15項(xiàng)對比三組間各測量指標(biāo),分析出不同垂直骨面型患者的顱面骨結(jié)構(gòu)特點(diǎn)。結(jié)果研究一顯示:高角組患者Co-Go,Co-Co',Go-Go',∠Go-Me-Go',∠Go-N-Go',∠Go-S-Go',∠Co-Me-Co'較均角組患者小,∠Co-Go-Me較均角組患者大;低角組患者Co-Go,Go-Go',Go-Me,下頜聯(lián)合深度,∠Go-Me-Go',∠Go-N-Go',∠Go-S-Go'較均角組患者大,∠Co-Go-Me較均角組患者小。高角組患者各項(xiàng)測量指標(biāo)均較低角組小。研究二顯示:高角組患者N-Ba,∠N-S-Ba,ANS-N,腭高較均角組大,Sm-Sm’,Tzl-Tzl’,Tz2-Tz2’,Ore-Ore’,Maz-Maz′,腭寬較均角組小;低角組Sm-Sm’,Tzl-Tzl’,Tz2-Tz2’,Ore-Ore’,Ori-Ori’,ANS-Ptm,Maz-Maz′,ANS-PNS,腭寬較均角組大,N-Ba,∠N-S-Ba,ANS-N,腭高較均角組小;高角組N-Ba,∠N-S-Ba,ANS-N,腭高較低角組大,Sm-Sm’,Tzl-Tzl’,Tz2-Tz2’,Ore-Ore’,Ori-Ori’,ANS-Ptm,Maz-Maz′,ANS-PNS,腭寬較低角組小。結(jié)論1.骨性II類高角患者具有較短的下頜升支高度,下頜骨后部較窄,開張較小;骨性II類低角患者具有較長的下頜升支高度和下頜體長度,下頜骨后部較寬,開張較大,下頜聯(lián)合深度亦較大。隨著下頜平面角的增大,下頜骨高度、長度和寬度基本呈增大的趨勢,低角患者的下頜骨發(fā)育較高角患者相對不足。2.高角型患者的顱底角較低角型者大,不同垂直骨面型患者的前顱底長度和后顱底長度差異性不大,由于顱底角的增大導(dǎo)致高角型患者的顱底總體長度上略大。3.低角型患者的眶距及面中部寬度、上頜的寬度和長度、腭部的寬度和長度較高角型明顯增大,在上頜的高度和腭高上,則高角型患者較低角型數(shù)值上更大些。4.總體來說,高角型患者的垂直向發(fā)育較低角型者明顯,而低角型患者的水平向及矢狀向的發(fā)育較高角型者更為明顯。5.不同垂直骨面型患者的顱面結(jié)構(gòu)具有不同的結(jié)構(gòu)特征,在進(jìn)行正畸治療設(shè)計(jì)時,應(yīng)充分加以考慮。
[Abstract]:Objective to measure and analyze the craniofacial bone tissue of adult patients with different vertical bone types of II malocclusion by means of CBCT, and to compare the measurement indexes among the high angle group, the average angle group and the low angle group. The three-dimensional structure of craniofacial bone in patients with malocclusion of different vertical bone types was summarized, which provided scientific anatomical basis and theoretical support for the diagnosis, treatment and prognosis of orthodontic patients. Materials and methods from June 2012 to September 2014, 30 CT images of conical bundle were selected from orthodontic department of Tianjin Stomatology Hospital. Ten items of mandibular measurement items and 15 items of craniozygomatic-maxillary complex measurement items were selected to compare each index of three groups, and the characteristics of craniofacial bone structure of patients with different vertical bone types were analyzed. Results the results showed that the Co-Go,Co-Co',Go-Go', Go-Me-Go', Go-N-Go', Go-S-Go', Co-Me-Co' in the high angle group was smaller than that in the average angle group. The Co-Go-Me was larger than that in the average angle group. In the low angle group, the depth of the mandibular joint of the patients with Co-Go,Go-Go',Go-Me, was larger than that of the patients of the average angle group, and that of the patients with the average angle group was smaller than that of the patients with the average angle group. All the indexes in high angle group were smaller than those in low angle group. The second study showed that the palate height of the patients with high angle group was larger than that of the average angle group, and the palatal width of Sm-Sm',Tzl-Tzl',Tz2-Tz2',Ore-Ore',Maz-Maz', group was smaller than that of the average angle group. The palatal width of Sm-Sm',Tzl-Tzl',Tz2-Tz2',Ore-Ore',Ori-Ori',ANS-Ptm,Maz-Maz',ANS-PNS, in the low angle group was larger than that in the average angle group, and the palate height was smaller than that in the average angle group. In the high angle group, N-Baand N-S-BaanS-Ns, the palatal height was larger than that in the low angle group, and the Sm-Sm',Tzl-Tzl',Tz2-Tz2',Ore-Ore',Ori-Ori',ANS-Ptm,Maz-Maz',ANS-PNS, palatal width was smaller than that in the low angle group. Conclusion 1. The height of mandibular ramus was shorter, the posterior part of mandible was narrower and the opening of mandible was smaller in patients with high angle of bone II. Patients with low angle of bone II have longer height of mandibular ramus and length of mandibular body, wider posterior part of mandible, larger opening of mandibular joint and greater depth of mandibular joint. With the increase of the mandibular plane angle, the height, length and width of the mandible showed an increasing trend, and the mandibular development of the patients with low angle was less than that of the patients with the higher angle. 2. The cranial base angle of the patients with high angle is larger than that of the patients with low angle. The length of the anterior skull base and the posterior skull base of the patients with different vertical bone face types are not different. The total length of the skull base of the patients with high angle type is slightly larger than that of the patients with high angle type because of the increase of the angle of the skull base. The orbital distance and midfacial width, the width and length of maxilla, the width and length of palate were significantly increased in patients with low angle type, and the values of height of maxillary and palatine were larger in patients with high angle type than those in low angle type. In general, the vertical development of high angle type is more obvious than that of low angle type, while the horizontal and sagittal development of low angle type is more obvious than that of high angle type. The craniofacial structures of patients with different vertical bone face types have different structural characteristics, which should be fully considered in the design of orthodontic treatment.
