替牙期單側(cè)完全性唇腭裂患者鼻上頜復合體對稱性研究

【摘要】:目的:對替牙期單側(cè)完全性唇腭裂(Unilateral complete cleft lip and palate,UCLP)患者的錐形束CT(Cone Beam Computed Tomography,CBCT)影像進行對比分析,研究替牙期UCLP患者鼻上頜復合體的對稱性,為其顱面形態(tài)的研究以及臨床治療提供更好的理論依據(jù)。方法:選擇20例替牙期UCLP患者作為UCLP組,20例替牙期非唇腭裂患者作為對照組。所有研究對象進行錐形束CT掃描,獲得CBCT影像,導入MIMICS 17.0軟件進行三維重建。在三維圖像上確定LPA點(梨狀孔最外側(cè)緣點)、INM點(鼻額縫最下點)、Maz點(顴牙槽嵴點)、SPM點(上頜第一磨牙牙槽緣點)、SPC點(上頜尖牙牙槽緣點)、SPr點(上頜中切牙牙槽緣點)等34個標志點,并建立三維坐標系,用軟件自帶的測量工具對兩組患者鼻上頜復合體的各項線距進行測量。用SPSS 22.0統(tǒng)計軟件包對測量結(jié)果進行統(tǒng)計和分析,比較UCLP患者裂隙側(cè)與非裂隙側(cè)鼻上頜復合體的對稱性,分析其裂隙側(cè)與非裂隙側(cè)的生長差異;比較對照組患者鼻上頜復合體的對稱性。結(jié)果:1.鼻上頜復合體水平向?qū)ΨQ性UCLP組患者裂隙側(cè)LPA點處上頜骨的寬度、SPr點處單側(cè)牙弓寬度大于非裂隙側(cè),INM點處鼻上頜復合體的寬度、SPC點處單側(cè)牙弓寬度小于非裂隙側(cè),存在統(tǒng)計學差異(P0.05)。對照組左右側(cè)各測量值之間無統(tǒng)計學差異(P0.05)。2.鼻上頜復合體矢狀向?qū)ΨQ性UCLP組患者裂隙側(cè)的LPA點、Maz點、SPr點處上頜骨的矢狀向發(fā)育差于非裂隙側(cè),SPM、SPC點位置相對于非裂隙側(cè)偏前,存在統(tǒng)計學差異(P0.05)。對照組左右側(cè)各測量值之間無統(tǒng)計學差異(P0.05)。2.鼻上頜復合體垂直向?qū)ΨQ性UCLP組患者裂隙側(cè)的LPA點處上頜骨高度、INM點處鼻上頜復合體的高度、Maz點處上頜骨的高度大于非裂隙側(cè),SPC點、SPr點處上頜骨高度小于非裂隙側(cè),存在明顯統(tǒng)計學差異(P0.05)。對照組左右側(cè)各測量值之間無統(tǒng)計學差異(P0.05)。結(jié)論:1、替牙期UCLP患者鼻上頜復合體存在不對稱性,且不對稱主要集中在鼻腔和牙槽骨處。2、替牙期非唇腭裂患者鼻上頜復合體無明顯不對稱。
[Abstract]:Objective: to study the symmetry of nasal and maxillary complex in patients with unilateral cleft lip and palate (Unilateral complete cleft lip and palate,UCLP) by comparing and analyzing the CT (Cone Beam Computed Tomography,CBCT images of conical bundle in patients with unilateral cleft lip and palate. To provide a better theoretical basis for the study of craniofacial morphology and clinical treatment. Methods: 20 patients with UCLP were selected as UCLP group and 20 patients with non-cleft lip and palate as control group. All the subjects were scanned by conical beam CT, and CBCT images were obtained and imported into MIMICS 17.0 software for 3D reconstruction. The LPA point (), INM point at the most lateral edge of piriform foramen) (), Maz point at the lowest point of the nasal suture (), SPM point at the zygomatic alveolar ridge point) (), SPC point at the alveolar edge point of the maxillary first molar) was determined on 3D images. 34 mark points such as SPr point (incisor point) and three dimensional coordinate system were set up to measure the distance of each line of nasal maxillary complex between two groups of patients by using the software's own measuring tool. The measurement results were analyzed by SPSS 22.0 software package. The symmetry of the nasal maxillary complex between the fissured side and the non-fissured side of UCLP was compared, and the growth difference between the fissured side and the non-fissured side was analyzed. The symmetry of nasal maxillary complex in control group was compared. Results: 1. In UCLP group, the width of maxillary bone at the LPA point of the fissure side, the width of the unilateral arch at the SPr point was larger than that of the non-fissured side, the width of the nasal maxillary complex at the INM point and the width of the unilateral arch at the SPC point was smaller than that at the non-fissured side. There was statistical difference (P0.05). There was no statistical difference between the left and right side of the control group (P0.05). In the sagittal symmetry UCLP group, the sagittal development of the maxilla at the LPA, Maz and SPr points of the maxillary complex was worse than that of the non-fissured side, and the position of the SPM,SPC point was significantly different from that of the non-fissured side (P0.05). There was no statistical difference between the left and right side of the control group (P0.05). In the UCLP group, the maxillary height at the LPA point at the fissure side, the maxillary complex height at the INM point, the maxillary height at the Maz point were higher than those at the non-fissured side, the maxillary height at the SPC point and the SPr point were lower than those at the non-fissured side. There was significant statistical difference (P0.05). There was no statistical difference between the left and right side of the control group (P0.05). Conclusion: 1. There is asymmetry in nasal maxillary complex in patients with UCLP, and the asymmetry is mainly in nasal cavity and alveolar bone. 2. There is no obvious asymmetry in nasal maxillary complex in patients with non-cleft lip and palate.
