【摘要】：目的探討下頜阻生智齒拔除術(shù)后醫(yī)院感染患者的病原學(xué)特點(diǎn)及危險(xiǎn)因素,為術(shù)后感染采取積極的預(yù)防措施等方面提供思路。方法選擇醫(yī)院口腔科2014年6月-2016年4月期間收治的847例下頜阻生智齒的患者,均采取下頜阻生智齒拔除術(shù)治療,詳細(xì)記錄患者年齡、性別等一般臨床資料,記錄其術(shù)后感染率的發(fā)生情況、感染部位、病原學(xué)特點(diǎn),對(duì)引起其發(fā)生感染的相關(guān)因素進(jìn)行單因素和多因素Logistic分析。結(jié)果在847例下頜阻生智齒拔除術(shù)后的患者中,有115例患者感染,感染率為13.6%;發(fā)生感染的部位主要分布于創(chuàng)面、口腔黏膜和呼吸道;115例感染患者中共分離出病原菌127株,以革蘭陰性菌為主,共101株占79.5%;Logistic多元回歸分析結(jié)果顯示,年齡、手術(shù)時(shí)長(zhǎng)、手術(shù)方式、術(shù)后出血情況、術(shù)前未預(yù)防性使用抗菌藥物是下頜阻生智齒拔除術(shù)后感染的獨(dú)立相關(guān)因素(P0.05)。結(jié)論應(yīng)根據(jù)下頜阻生智齒拔除術(shù)后醫(yī)院感染的相關(guān)因素進(jìn)行綜合防控,降低感染率。
[Abstract]:Objective to explore the etiological characteristics and risk factors of nosocomial infection after mandibular impacted wisdom tooth extraction. Methods from June 2014 to April 2016, 847 patients with mandibular impacted wisdom teeth were treated with mandibular impacted wisdom tooth extraction. The patients' age, sex and other general clinical data were recorded in detail. The incidence of infection, site of infection and etiological characteristics were recorded. Univariate and multivariate Logistic analysis were carried out to analyze the related factors that caused the infection. Results among 847 patients after mandibular impacted wisdom tooth extraction, 115 patients were infected, the infection rate was 13.60.The site of infection was mainly distributed in wound, oral mucosa and respiratory tract. A total of 127 strains of pathogenic bacteria were isolated from 115 infected patients, of which 101 strains were Gram-negative bacteria (79.5%). Logistic multiple regression analysis showed that age, length of operation, operation mode, postoperative bleeding, preoperative non-prophylactic use of antimicrobial agents were the independent factors of infection after mandibular impacted wisdom tooth extraction (P0.05). Conclusion according to the related factors of nosocomial infection after mandibular impacted wisdom tooth extraction, comprehensive prevention and control should be carried out to reduce the infection rate.
