菌群定植與種植體周齦溝深度的關(guān)系

【摘要】:目的通過分析不同齦溝深度的種植體周圍菌群分布的特點(diǎn),了解齦溝深度與菌群定植的關(guān)系。方法選擇我院口腔科進(jìn)行義齒種植的患者38名作為研究對象,測量患者種植體周圍袋深度(Peri-implant Probing pokect Depth,PPD),并根據(jù)PPD將研究對象劃分為一組(PPD3 mm)及二組(PPD≥3 mm)兩組,比較兩組患者改良菌斑指數(shù)、牙齦出血指數(shù)種植體周圍菌群的檢出率及CFU構(gòu)成情況。結(jié)果本次共檢查了78顆種植體,一組55顆種植牙、二組共23顆種植牙,一組患者的mPLI平均為0.67±0.71、GBI平均為0.43±0.54;二組患者的mPLI平均為1.74±0.89、GBI平均為1.67±0.68。一組及二組調(diào)查對象種植體的mPLI及GBI差異具有統(tǒng)計(jì)學(xué)意義(P0.001)。有益菌血鏈球菌及小韋榮菌一組檢出率高于二組(P0.05);而致病菌具核梭桿菌、中間型普氏菌檢出率二組高于一組(P0.05),變黑普雷沃菌、牙齦卟啉菌在兩組間的檢出率無差異。一組與二組各類菌群菌落形成單位的構(gòu)成比總體分布差異具有統(tǒng)計(jì)學(xué)意義(P0.001),一組患者有益菌群革蘭陽性兼性厭氧菌如血鏈球菌及其與鏈球菌的比例較高,而有害菌群革蘭氏陰性厭氧桿菌如具核梭桿菌、變黑普雷沃氏菌所占比例較低,而二組則呈相反的分布狀況。結(jié)論種植體周齦溝深度與菌群定植有一定的關(guān)系,齦溝較深的種植體周齦下菌群有益菌較少,而有害菌所占比例較高。
