骨性Ⅰ類青少年上氣道形態(tài)和舌骨位置的錐體束CT分析

【摘要】:目的:分析骨性Ⅰ類均角型(正常骨面型)不同年齡組青少年上氣道形態(tài)和舌骨位置的差異,以及各年齡組的性別差異;分析骨性Ⅰ類不同垂直骨面型女性青少年上氣道形態(tài)和舌骨位置的差異。方法:按照納入標(biāo)準(zhǔn)和排除標(biāo)準(zhǔn)選取2014年7月至2016年3月期間到烏魯木齊市口腔醫(yī)院因正畸治療前檢查需要而拍攝CBCT的青少年患者66例,其中骨性Ⅰ類均角型青少年患者40例(男性20例,女性20例),骨性Ⅰ類低角型青少年女性患者13例,骨性Ⅰ類高角型青少年女性患者13例。將患者CBCT圖像導(dǎo)入MIMICS16.0軟件,完成上氣道重建和上氣道形態(tài)和舌骨位置的測(cè)量。40例正常骨面型的青少年患者按年齡分組比較上氣道形態(tài)和舌骨位置可能存在的年齡和性別差異,并采用兩樣本t檢驗(yàn)進(jìn)行統(tǒng)計(jì)分析。39例不同垂直骨面型女性患者按照GoGn-SN角大小分為3組:低角組(GoGn-SN27.3°);均角組(27.3°GoGn-SN37.7°);高角組(GoGn-SN37.7°),比較不同垂直骨面型女性青少年患者上氣道形態(tài)和舌骨位置間可能存在的差異,并采用單因素方差分析進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果:隨年齡增長(zhǎng),上氣道的各測(cè)量項(xiàng)目均有增大,男性上氣道L3、LAT3、LAT4、VOL1、VOL4和男性舌骨H-Y、H-EB、H-PNS以及女性上氣道L1和L,差異具有統(tǒng)計(jì)學(xué)意義(P0.05),女性舌骨在各測(cè)量項(xiàng)目中差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);在14~16歲組,上氣道L4、L、LAT3、VOL4和舌骨H-Y、H-EB、H-PNS,存在性別間差異(P0.05);不同垂直骨面型組女性上氣道形態(tài)在鼻咽部AP1及腭咽部AP2、R2差異具有顯著性(P0.05),上氣道長(zhǎng)度、橫截面、體積等差異無(wú)顯著性(P0.05);均角組AP2較低角組減小,高角組AP1和AP2均較低角組減小,高角組R2較低角組增大,差異具有顯著性(AP1、R2:P0.01;AP2:P0.05)。不同垂直骨面型組女性舌骨位置在C3-Me、H-X、及H-S-Ba差異具有顯著性(P0.01),高角組均較低角組減小,差異具有顯著性(P0.01)。結(jié)論:隨年齡增長(zhǎng),男性鼻咽、喉咽體積增長(zhǎng)明顯,男性舌骨向下移位,14~16歲青少年女性喉咽氣道較男性大,但男性舌骨相對(duì)于顱底位置較女性更靠下。不同垂直骨面型組女性青少年上氣道形態(tài)和舌骨位置存在明顯差異。
