碳纖維增強“All-on-4”即刻修復體的臨床評價

【摘要】:目的:評價碳纖維增強"All-on-4"即刻修復體的臨床效果。方法:根據(jù)納入標準及排除標準,收集2008年4月至2014年3月在北京大學口腔醫(yī)學院種植中心應用"All-on-4"技術完成的上頜或下頜無牙頜種植即刻修復體,根據(jù)有無碳纖維增強分為對照組和增強組。2008年4月至2012年12月共納入對照組60例患者(男32例,女28例),完成即刻修復體71例(上頜28例,下頜43例);2013年1月至2014年3月共納入增強組23例患者(男13例,女10例),完成即刻修復體28例(上頜9例,下頜19例)。記錄修復體相應患者的詳細病例資料,以永久修復日期為截點,觀察即刻修復體在使用期間內有無折裂,增強組即刻修復體組織表面有無纖維暴露及口內黏膜情況,掃描電鏡觀察碳纖維與樹脂交界面的結合情況。結果:對照組和增強組即刻修復體在患者年齡[(57.3±10.1)歲vs.(55.1±11.4)歲]、性別(男32例和女28例vs.男13例和女10例)、上/下頜位(上頜28例和下頜43例vs.上頜9例和下頜19例)、術前即刻拔牙病例數(shù)(46例vs.23例)、修復體平均使用時間[(7.8±1.3)個月vs.(7.5±1.1)個月]及即刻修復體對頜牙列狀態(tài)分布方面差異無統(tǒng)計學意義(P0.05)。對照組71例即刻修復體共發(fā)生21例(29.6%)基托折裂,碳纖維增強組28例即刻修復體,在行使功能期間未發(fā)現(xiàn)有修復體折裂,增強組即刻修復體折裂率顯著性低于對照組(P=0.001),且修復體組織表面均未見碳纖維暴露,口內黏膜正常無刺激。掃描電鏡可見碳纖維與樹脂之間結合相對緊密,纖維表面可見黏附的樹脂顆粒。結論:對"All-on-4"即刻修復體基托樹脂進行碳纖維增強,能減少修復體折裂的發(fā)生,效果可靠。
[Abstract]:Objective: to evaluate the clinical effect of carbon fiber reinforced "All-on-4" immediate prosthesis. Methods: according to the inclusion criteria and exclusion criteria, the immediate prostheses of maxillary or mandibular edentulous implants were collected from April 2008 to March 2014 in the Center of Dental Medicine, Peking University, using "All-on-4" technique. From April 2008 to December 2012, 60 patients (32 males and 28 females) were included in the control group, and 71 cases (28 maxillary, 43 mandible) were performed immediate prosthesis. From January 2013 to March 2014, 23 patients (13 males and 10 females) were included in the enhanced group, and 28 cases (9 maxillary, 19 mandible) were performed immediate prosthesis. The detailed case data of the corresponding patients were recorded, and the date of permanent repair was taken as the cut-off point to observe whether the immediate prosthesis had fracture or not, and whether the tissue surface of the immediate prosthesis in the enhancement group was exposed to fibers and the mucous membrane in the mouth. The interface between carbon fiber and resin was observed by SEM. Results: the age of the patients with immediate prosthesis in the control group and the enhanced group [(57.3 鹵10.1) years old vs. (55.1 鹵11.4) years old] and sex (32 males and 28 females vs.) were found. 13 males and 10 females, upper / mandibular position (28 maxillary and 43 mandibular vs.) There were 9 cases of maxilla and 19 cases of mandible, and 46 cases of vs.23 were extracted immediately before operation. There was no significant difference in mean service time (7.8 鹵1.3) months vs. (7.5 鹵1.1) months and immediate prosthesis in the distribution of maxillary dentition status (P0.05). In the control group, 21 cases (29.6%) of the immediate restoration occurred in the control group, and 28 cases in the carbon fiber reinforced group. No fracture was found during the exercise of the function. The fracture rate was significantly lower in the enhancement group than that in the control group (Pn0.001), and no carbon fiber exposure was found on the surface of the prosthesis, and there was no irritation in the oral mucosa. Scanning electron microscope showed that carbon fiber and resin bonded relatively closely, the surface of the fiber adhesive resin particles can be seen. Conclusion: carbon fiber reinforcement of "All-on-4" immediate prosthesis base resin can reduce the fracture rate and the effect is reliable.
