【摘要】：目的:評價牙槽突擴張聯(lián)合引導骨再生術(GBR)和植骨術同期植入ITI種植體的遠期效果。方法:回顧分析對55名患者(男35名,女20名)行牙槽突擴張聯(lián)合GBR植骨術,同期植入ITI Straumann種植體83顆的臨床效果。結果:患者平均年齡36.3歲(18~60歲),平均骨愈合時間6.9個月(4.0~20.7個月)。完成56例修復體,其中種植單冠28例,種植聯(lián)冠8例,種植單端橋5例,種植固定橋13例,種植支持式覆蓋義齒2例。修復完成后每年定期隨訪,包括臨床檢查和X線測量,累計觀察期8年。2名患者(3.6%)術中發(fā)生唇側骨板骨折。4名患者(7.5%)修復后失訪。49名患者平均隨訪時間3.1年(0.5~7.5年),種植體8年累計存留率為100%。種植體平均邊緣骨高度為頸緣下2.57 mm。10例修復體發(fā)生修復并發(fā)癥。結論:針對嚴重牙槽突寬度骨量不足的患者,牙槽突擴張聯(lián)合GBR植骨牙種植術是一種可行的種植方案。
[Abstract]:Objective: to evaluate the long-term effects of alveolar process dilation combined with guided bone regeneration (GBR) and bone graft implantation with ITI implants. Methods: 55 patients (35 males and 20 females) were treated with alveolar process dilation and GBR bone grafting. 83 ITI Straumann implants were implanted simultaneously. Results: the average age of the patients was 36.3 years (1860 years) and the average bone healing time was 6.9 months (4.0 ~ 20.7 months). 56 cases of prostheses were completed, including 28 cases of single implant crown, 8 cases of implant combined crown, 5 cases of implant single end bridge, 13 cases of implant fixed bridge, 2 cases of implant supported overdenture. The repair is followed up regularly every year, including clinical examination and X-ray measurement. The cumulative observation period was 8 years. 2 patients (3.6%) developed labial bone plate fracture during operation. 4 patients (7.5%) lost their visit after repair. 49 patients were followed up for an average of 3.1 years (0.5 ~ 7.5 years). The accumulative survival rate of implant was 100%. The mean marginal bone height of the implant was 2. 57 mm.10 below the cervical margin. Conclusion: alveolar process dilation combined with GBR implantation is a feasible implant for patients with severe alveolar process width deficiency.
