腭中部微種植釘支抗系統(tǒng)的臨床效果評(píng)價(jià)

【摘要】:目的:初步探討腭中部微種植釘支抗系統(tǒng)在固定矯治中的支抗效果,為正畸臨床支抗的選擇提供參考。方法:選擇骨性Ⅰ、Ⅱ類需上頜強(qiáng)支抗的青少年正畸患者14例作為研究對(duì)象,均采用雙頜固定矯治器進(jìn)行正畸治療,在上頜減數(shù)兩顆第一雙尖牙并在腭中線旁、第一磨牙水平植入微種植釘一顆。以0.019 inch×0.022 inch(1inch=2.54 cm)的不銹鋼方絲橫腭桿固定連接微種植釘和雙側(cè)上頜第一磨牙,組成一個(gè)腭中部微種植釘支抗系統(tǒng)進(jìn)行正畸力的負(fù)載,下牙列不納入研究。植入微種植釘時(shí)(T0)、上頜間隙關(guān)閉或基本關(guān)閉時(shí)(T1)分別拍攝頭顱側(cè)位片進(jìn)行頭影測(cè)量描圖。分別采用SN-7平面及PP平面作為參考平面,對(duì)微種植釘以及上頜第一磨牙在T0、T1的位置差異進(jìn)行頭影測(cè)量分析。采用配對(duì)t檢驗(yàn)進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果:腭中部微種植釘植入后的臨床成功率為73.9%。14例進(jìn)行測(cè)量分析的病例中,微種植系統(tǒng)負(fù)載正畸力后仍保持穩(wěn)固,雙側(cè)第一磨牙沒有發(fā)生明顯的近中移動(dòng)。微種植釘以及上頜第一磨牙的位置變化在兩組間的差異沒有統(tǒng)計(jì)學(xué)意義。結(jié)論:本研究中設(shè)計(jì)的腭中部微種植支抗系統(tǒng)具有較好的穩(wěn)定性,在正畸治療中能夠起到強(qiáng)支抗的作用,可作為一種正畸強(qiáng)支抗的可選方案在臨床中應(yīng)用。
[Abstract]:Objective: to explore the effect of microimplant Anchorage system in the middle palatine for the selection of orthodontic Anchorage. Methods: fourteen cases of juvenile orthodontic patients who need strong maxillary Anchorage were selected as the study objects. All of them were treated with double jaw fixed appliance. The first bicuspid teeth were subtracted from the maxilla and next to the midline of the palate. The first molars were horizontally implanted with a microimplant nail. A 0.019 inch 脳 0.022 inch (1inch=2.54 cm) stainless steel square wire transverse palate rod was used to fix the micro-implant nail and bilateral maxillary first molar to form a mid-palatal implant Anchorage system for orthodontic loading. The lower dentition was not included in the study. When microimplant nail was implanted (T _ 0) and maxillary space was closed or basically closed (T _ 1), lateral cephalograms were taken for cephalometric tracing. The SN-7 plane and PP plane were used as reference plane to measure the position difference of the first maxillary molars and implants in T _ 0 T _ 1. The paired t test was used for statistical analysis. Results: the clinical success rate was 73.9% in the middle palatal implants. 14 cases were measured and analyzed. The orthodontic force of the microimplant system remained stable, and the bilateral first molars had no significant proximal movement. There was no significant difference between the two groups in the position changes of the microimplant nail and the maxillary first molar. Conclusion: the microimplant Anchorage system designed in this study has good stability and can play a role of strong Anchorage in orthodontic treatment. It can be used in clinic as an alternative scheme of orthodontic strong Anchorage.
