【摘要】：目的:比較上頜雙側(cè)中切牙外傷行冠延長術(shù)后,不同修復(fù)設(shè)計(jì)對牙槽骨應(yīng)力及牙齒動度的影響。方法:通過逆向工程技術(shù)建立齒冠延長術(shù)后不同骨下降高度的三維有限元模型,并將其設(shè)計(jì)為單冠、二聯(lián)冠、三聯(lián)冠、四聯(lián)冠修復(fù);然后用三維有限元法分析降低不同牙槽骨高度對樁核冠修復(fù)后牙槽骨應(yīng)力分布及牙齒動度的影響。結(jié)果:隨著牙槽骨高度的降低,所有修復(fù)形式組的牙槽骨應(yīng)力及牙齒位移都有所上升,其中單冠修復(fù)組的增加幅度最大;當(dāng)骨高度下降1 mm和2 mm時(shí),單冠和3種聯(lián)冠修復(fù)均能滿足牙周潛力的要求;而骨下降高度為3 mm時(shí),單冠修復(fù)組的應(yīng)力值超過了牙周潛力的范圍,二聯(lián)冠修復(fù)接近牙周潛力,三聯(lián)冠及四聯(lián)冠修復(fù)均小于牙周潛力。結(jié)論:去骨高度在2 mm以內(nèi)時(shí),單冠和二聯(lián)冠修復(fù)均可,但二聯(lián)冠可以降低手術(shù)后造成的不利影響;當(dāng)去骨高度為3 mm時(shí),必須采用聯(lián)冠修復(fù),最好選用三聯(lián)冠修復(fù)。
[Abstract]:Objective: to compare the effects of different restoration designs on alveolar bone stress and tooth movement after crown lengthening for bilateral maxillary central incisor injury. Methods: three dimensional finite element models of different osteodescending heights after tooth crown lengthening were established by reverse engineering, and were designed as single crown, double crown, triple crown and quadruple crown. Then the effect of reducing the height of alveolar bone on the stress distribution and tooth movement of posterior alveolar bone repaired by post-core crown was analyzed by three-dimensional finite element method. Results: with the reduction of alveolar bone height, the alveolar bone stress and tooth displacement increased in all kinds of restoration groups, especially in the single crown group. When the height of bone decreased by 1 mm and 2 mm, both single crown and three kinds of combined crown could meet the requirement of periodontal potential. The stress value of single crown group exceeded the range of periodontal potential when the height of bone decreased 3 mm. The restoration of double crown was close to periodontal potential, and that of triplex crown and quadruple crown was lower than that of periodontal potential. Conclusion: when the height of bone removal is less than 2 mm, both single and double crowns can be repaired, but the adverse effects after operation can be reduced by double crown, and when the height of bone removal is 3 mm, it is necessary to use combined crown repair, and it is better to use triplex crown to repair.
