上頜尖牙腭側(cè)阻生鄰近側(cè)切牙冠根形態(tài)特征的錐形束CT研究

【摘要】:目的上頜尖牙腭側(cè)阻生是口腔常見的阻生畸形,但其病因仍存在爭議。文中旨在通過錐形束CT(Cone-beam computed tomography,CBCT)三維成像研究上頜尖牙腭側(cè)埋伏阻生患者鄰近側(cè)切牙冠根形態(tài)大小特征以探究其病因。方法選擇2012年9月至2015年3月四川醫(yī)科大學附屬口腔醫(yī)院正畸科就診的錯牙合患者84例,對所有患者共計94顆上頜尖牙(實驗組:上頜尖牙腭側(cè)埋伏阻生患者24例,27顆阻生尖牙;對照組A:30例上頜尖牙唇側(cè)埋伏阻生患者,37顆阻生尖牙;對照組B:30例上頜尖牙正常萌出患者,30顆尖牙)使用口腔專用CBCT影像掃描。利用CS 3D Imagine軟件選擇需要的參考平面測量所選尖牙方位及鄰近側(cè)切牙形態(tài),對17個測量值[牙體長度、牙冠長度、牙根長度、唇側(cè)釉牙骨質(zhì)界(cementoenamel junction,CEJ)處近遠中徑等]進行統(tǒng)計分析。結果實驗組牙體長度、牙冠長度、離CEJ4 mm處牙冠近遠中徑[(21.240±1.651)、(8.150±0.889)、(5.900±0.581)mm]均小于對照組A[(22.930±1.849)、(9.270±0.559)、(7.260±0.579)mm],差異有統(tǒng)計學意義(P0.05);CEJ處近遠中徑、離CEJ4 mm處牙根頰舌徑、CEJ處頰舌徑、尖牙牙尖頂?shù)焦跔钇矫妗⒀篮掀矫娴木嚯x亦小于對照組A(P0.05)。實驗組牙體長度、牙冠長度、離CEJ4 mm處牙冠近遠中徑均小于對照組B的[(23.200±0.474)、(9.070±0.534)、(6.680±0.684)mm],差異有統(tǒng)計學意義(P0.05);牙根長度、CEJ處近遠中徑、CEJ處頰舌徑、離CEJ 4 mm處牙根頰舌徑、離8 mm處牙根頰舌徑、尖牙牙尖頂?shù)绞笭钇矫婕把篮掀矫娴木嚯x均較對照組B差異有統(tǒng)計學意義(P0.05)。腭側(cè)阻尖牙牙尖頂?shù)绞笭钇矫、冠狀平面、牙合平面的距離、尖牙長軸與矢狀平面、冠狀平面所成角度與鄰近側(cè)切牙的牙根長度、CEJ處頰舌徑、離CEJ 4 mm處牙根頰舌徑、離8 mm處牙根頰舌徑之間存在相關性(P0.05)。腭側(cè)阻生尖牙尖牙頂?shù)绞笭钇矫、牙合平面的距離和年齡存在相關性(r=-0.472、-0.489,P0.05)。結論上頜尖牙腭側(cè)阻生的主要原因之一是鄰近側(cè)切牙過小而對尖牙引導不足。年齡越大,上頜尖牙腭側(cè)阻生程度越嚴重。
[Abstract]:Objective maxillary canine lateral impaction is a common impacted malformation in oral cavity, but its etiology is still controversial. The purpose of this study was to study the morphological and size characteristics of the crowns of adjacent incisors in patients with palatal impacted maxillary canine by conical beam CT (Cone-beam computed tomography,CBCT) in order to investigate its etiology. Methods from September 2012 to March 2015, 84 patients with malocclusion were selected from Department of Orthodontics, affiliated Stomatology Hospital of Sichuan Medical University. A total of 94 maxillary canine teeth (experimental group: 24 maxillary canine side impacted patients, 27 impacted canine teeth, control group A: 30 maxillary canine labial side impacted patients, 37 impacted canine teeth; Control group B: 30 patients with normal maxillary canine eruption and 30 canine teeth) were scanned by oral CBCT. Using CS 3D Imagine software to select the needed reference plane to measure the azimuth of canine teeth and the shape of adjacent lateral incisors, 17 measurements were made [tooth length, crown length, root length].
