APN-1在糖尿病牙周炎小鼠牙齦組織的表達(dá)

【摘要】:研究發(fā)現(xiàn),低脂聯(lián)素血癥與T2DM發(fā)展過程中的高胰島素血癥和胰島素抵抗有關(guān)。故有學(xué)者提出,脂聯(lián)素也可作為探討患者將來發(fā)生糖尿病的一個(gè)獨(dú)立因子。在牙周炎和糖尿病的發(fā)生發(fā)展機(jī)制過程中,炎癥反應(yīng)失調(diào)和加劇是關(guān)鍵,有研究報(bào)道,對(duì)伴有糖尿病牙周炎患者進(jìn)行牙周干預(yù)治療后,患者血清脂聯(lián)素水平上升,牙周及全身狀況得到改善,有可能減少糖尿病及其并發(fā)癥的發(fā)生和發(fā)展。然而,糖尿病伴牙周炎患者的局部炎癥狀況及其參與機(jī)體胰島素抵抗的發(fā)生發(fā)展是否均與脂聯(lián)素水平變化有關(guān),這些問題都尚未得到解決。通過動(dòng)物實(shí)驗(yàn),建立伴有糖尿病的牙周炎動(dòng)物模型,研究脂聯(lián)素與血糖水平及炎癥狀態(tài)是否具有相關(guān)性,為研究糖尿病與牙周炎的相關(guān)性及日益增多的糖尿病伴有牙周炎患者的臨床治療及預(yù)防提供一些理論依據(jù)。 目的:通過檢測(cè)健康、糖尿病及糖尿病伴牙周炎昆明系小鼠牙齦組織中脂聯(lián)素基因及蛋白含量的變化,檢測(cè)三組鼠的血漿中脂聯(lián)素含量,探討脂聯(lián)素含量與血糖水平及牙周炎癥狀態(tài)是否具有相關(guān)性。 方法:45只3周齡的昆明系小鼠按不同造模方式分為三組: A組對(duì)照組(15只),標(biāo)準(zhǔn)顆粒飼料喂養(yǎng)。 B組糖尿病組(15只)。 15只健康鼠通過四氧嘧啶(Alloxan)注射法4周建立小鼠糖尿病模型(12mg/100g體重來誘導(dǎo))。 C組糖尿病+牙周炎模型(15只)。 15只健康鼠先與B組同時(shí)建糖尿病模型,糖尿病組造模成功后左、右上頜第一磨牙及第二磨牙牙頸部絲線結(jié)扎聯(lián)合高糖喂養(yǎng)6周建立小鼠糖尿病+牙周炎模型。 通過測(cè)檢AB兩組小鼠不同時(shí)間的血糖、空腹胰島素水平(Fins)及胰島素抵抗指數(shù),并與空白對(duì)照組對(duì)比來確認(rèn)小鼠糖尿病組造模成功。通過牙周組織病理組織學(xué)檢查及CBCT掃描觀察牙槽骨喪失程度判斷小鼠糖尿病+牙周炎模型造模成功。 分別取三組小鼠的牙齦組織,用RT-PCR、western blot檢測(cè)牙齦組織中脂聯(lián)素基因及蛋白含量的變化,并于鼠處死時(shí)取三組鼠的血漿,通過ELISA方法測(cè)量血漿中脂聯(lián)素含量進(jìn)行對(duì)比和統(tǒng)計(jì)學(xué)分析。 結(jié)果: 1.本實(shí)驗(yàn)發(fā)現(xiàn),B組造模后20天,小鼠血糖、空腹胰島素水平(Fins)及胰島素抵抗指數(shù)大于正常值,與空白對(duì)照組相比差異具有統(tǒng)計(jì)學(xué)意義(P0.01),小鼠糖尿病組造模成功。 2.牙齦組織病理組織學(xué)檢測(cè)顯示糖尿病伴牙周炎組小鼠牙齦炎癥表現(xiàn)最明顯、在單純糖尿病組部分小鼠可見牙齦組織輕度炎癥、健康組小鼠牙齦未見明顯炎癥;CBCT掃描結(jié)果顯示糖尿病伴牙周炎組小鼠牙槽骨喪失程度最明顯,糖尿病組小鼠次之,健康組小鼠未見明顯骨喪失。小鼠糖尿病+牙周炎組造模成功。 3.RT-PCR、western blot檢測(cè)小鼠牙齦組織中脂聯(lián)素基因及蛋白表達(dá)水平在三組間比較均有差異,存在統(tǒng)計(jì)學(xué)意義(P0.05)。在健康組表達(dá)水平最強(qiáng),在糖尿病組、糖尿病伴牙周炎組小鼠中表達(dá)水平依次減弱。 4. ELISA結(jié)果顯示APN蛋白在健康組小鼠血漿中表達(dá)水平最強(qiáng),在糖尿病組、糖尿病伴牙周炎組小鼠血漿中表達(dá)水平依次減弱;組間比較均有差異,存在統(tǒng)計(jì)學(xué)意義(P0.05)。 結(jié)論: 1.可通過四氧嘧啶(Alloxan)注射法有效建立小鼠糖尿病模型。可通過牙周局部結(jié)扎聯(lián)合高糖喂養(yǎng)有效建立小鼠糖尿病伴牙周炎模型。 2.糖尿病伴牙周炎組小鼠與糖尿病組小鼠、健康組小鼠相比,牙齦炎癥及骨喪失程度最明顯,脂聯(lián)素基因和蛋白含量在小鼠牙齦組織中表達(dá)顯著降低。提示糖尿病可能通過降低脂聯(lián)素水平促進(jìn)牙齦局部炎癥的發(fā)生、發(fā)展;與此同時(shí),牙周炎可能協(xié)同降低脂聯(lián)素水平,促進(jìn)糖尿病的發(fā)展。 3.糖尿病后牙齦組織及血漿中脂聯(lián)素表達(dá)降低可能是牙周炎發(fā)病及加重的重要因素之一,APN可能在糖尿病和牙周炎相互影響關(guān)系中起著一定作用,是糖尿病的保護(hù)性因子,且有一定的抗炎作用。為臨床治療和預(yù)防牙周炎提供理論依據(jù)。
