青年女性月經(jīng)期與排卵期加力對(duì)上頜尖牙移動(dòng)和疼痛程度的對(duì)比研究

【摘要】:目的:探討青年女性患者在月經(jīng)期與排卵期加力對(duì)正畸牙移動(dòng)影響,觀測(cè)兩組加力后疼痛程度是否具有統(tǒng)計(jì)學(xué)差異,以了解女性患者在月經(jīng)期與排卵期正畸加力后牙齒的移動(dòng)特征、疼痛程度的改變,豐富正畸學(xué)臨床理論和方法。方法: 1.首先選取60例于2012年1月至2013年6月間來(lái)重慶醫(yī)科大學(xué)附屬口腔醫(yī)院正畸科就診的青年女性患者,18-23歲、未婚、無(wú)生育史,需減數(shù)第一雙尖牙,采用種植釘支抗的青年女性患者進(jìn)行月經(jīng)周期問(wèn)卷調(diào)查、基礎(chǔ)體溫測(cè)定及排卵試紙測(cè)定排卵期,篩選出月經(jīng)周期長(zhǎng)度在28-32天,月經(jīng)量穩(wěn)定而規(guī)律的受試者。按照試驗(yàn)的納入標(biāo)準(zhǔn)和排除標(biāo)準(zhǔn),篩選出24例符合試驗(yàn)條件的受試者。2.符合條件的受試者臨床中采用統(tǒng)一的矯治技術(shù)和方法排齊上頜牙列后,隨機(jī)分為月經(jīng)期加力組和排卵期加力組,應(yīng)用種植體支抗遠(yuǎn)中移動(dòng)尖牙。兩組受試者均分別于加力前,加力后4周、8周及12周測(cè)量?jī)山M尖牙向遠(yuǎn)中移動(dòng)的距離,同時(shí)取模型測(cè)量尖牙旋轉(zhuǎn)角度,進(jìn)行兩組間尖牙移動(dòng)距離和旋轉(zhuǎn)角度差異的統(tǒng)計(jì)分析。3.應(yīng)用目前較為公認(rèn)的描述疼痛程度級(jí)別的視覺(jué)模擬評(píng)分法(VAS)讓受試者分別于加力后的6小時(shí)、1天、2天、3天、4天、5天、6天及7天對(duì)加力后疼痛的程度進(jìn)行問(wèn)卷調(diào)查記錄。以上研究均采用SPSS18.0軟件包對(duì)兩組尖牙向遠(yuǎn)中移動(dòng)的距離、尖牙移動(dòng)中旋轉(zhuǎn)角度的改變和加力后疼痛程度及延續(xù)時(shí)長(zhǎng)進(jìn)行進(jìn)行獨(dú)立樣本t檢驗(yàn)統(tǒng)計(jì)學(xué)分析比較。結(jié)果:1、月經(jīng)期加力組尖牙向遠(yuǎn)中移動(dòng)距離明顯大于排卵期加力組,差異有明顯統(tǒng)計(jì)學(xué)意義(p0.05)。2、月經(jīng)期加力組尖牙旋轉(zhuǎn)度大于排卵期加力組,有明顯統(tǒng)計(jì)學(xué)差異(p0.05)。3、兩試驗(yàn)組間加力后產(chǎn)生疼痛的程度無(wú)明顯統(tǒng)計(jì)學(xué)差別(p0.05)。結(jié)論:1、青年女性月經(jīng)期加力尖牙遠(yuǎn)中移動(dòng)距離及旋轉(zhuǎn)角度的改變明顯大于排卵期加力,提示在青年女性患者月經(jīng)期加力牙齒移動(dòng)較快,可以提高矯治效率,同時(shí)也需密切關(guān)注該時(shí)期支抗控制。2、兩試驗(yàn)組間加力后產(chǎn)生的疼痛程度無(wú)明顯差異,提示在本試驗(yàn)條件下,疼痛不能作為女性月經(jīng)期正畸加力的禁忌癥。
[Abstract]:Objective: to investigate the effect of postmenopausal stress on orthodontic tooth movement in young female patients during menstrual period and ovulatory period. To understand the characteristics of tooth movement and the change of pain degree of female patients after orthodontic treatment during menstrual period and ovulation stage, and to enrich the clinical theory and methods of orthodontics. Methods: 1. From January 2012 to June 2013, 60 young female patients, aged 18-23 years, who were admitted to the Orthodontics Department of the affiliated Stomatology Hospital of Chongqing Medical University, were selected. They were unmarried, had no history of childbearing, and needed to reduce their first bicuspid teeth. The menstrual cycle questionnaire was conducted in young female patients with implant nail Anchorage, the ovulation period was measured by basic body temperature test and ovulation test paper, and the subjects whose menstrual cycle length was 28-32 days and menstrual volume stable and regular were selected. According to the inclusion criteria and exclusion criteria, 24 eligible subjects were selected. The subjects were randomly divided into two groups: postmenopausal stress group and ovulatory stress group. Implant Anchorage was used to resist distal canine movement. The distance between the two groups was measured before, 4 weeks, 8 weeks and 12 weeks after the application. The model was used to measure the rotation angle of the canine teeth, and the statistical analysis was made on the distance and rotation angle of the canine teeth between the two groups. The visual analogue scale (VAS), which is widely used to describe the degree of pain, was used to record the degree of pain at 6 hours, 1 day, 2 days, 3 days, 4 days, 5 days, 6 days and 7 days. SPSS18.0 software package was used to analyze and compare the distance between the canine teeth to the distal distance, the rotation angle during the canine movement and the pain degree and duration of the canine movement. Results: 1. The distance of distal movement of canine in menstruating group was significantly longer than that in ovulatory group (p0.05). 2. The rotation degree of canine in menstruating period was greater than that in ovulatory poststimulation group. There was significant statistical difference (p0.05). 3. There was no significant difference in the degree of pain after adding force between the two groups (p0.05). Conclusion: 1. The change of the distance and rotation angle of extra force canine teeth in young women during menstrual period is obviously larger than that in ovulatory period, which suggests that the tooth movement of young female patients during menstrual period is faster and can improve the efficiency of correction. At the same time, we also need to pay close attention to the control of Anchorage in this period. 2. There is no significant difference in pain degree between the two groups after adding force, which suggests that pain can not be regarded as contraindication of orthodontic stress in female menstrual period under this experimental condition.
