【摘要】：目的探究頜間牽引釘聯(lián)合微型鈦板內(nèi)固定修復(fù)頜骨骨折患者療效。方法選取石河子大學(xué)醫(yī)學(xué)院第一附院2013年6月~2015年5月收治的82例頜骨骨折患者,依據(jù)治療方法不同分為對照組與研究組,各41例。對照組實施微型鈦板內(nèi)固定治療,研究組實施頜間牽引釘與微型鈦板內(nèi)固定聯(lián)合治療。術(shù)后隨訪半年,統(tǒng)計兩組手術(shù)用時、臨床療效、骨折愈合時間、不良反應(yīng)情況,并對比兩組治療前后牙周情況相關(guān)指標(biāo)[菌斑指數(shù)(PI)、軟垢指數(shù)(DI)、牙齦指數(shù)(GI)]評分情況。結(jié)果兩組手術(shù)用時比較,差異無統(tǒng)計學(xué)意義(P0.05);與對照組相比,研究組骨折愈合時間較短,差異有統(tǒng)計學(xué)意義(P0.05);兩組治療優(yōu)良率相比,研究組97.56%(40/41)遠高于對照組80.49%(33/41),差異有統(tǒng)計學(xué)意義(P0.05);治療前,兩組PI、DI、GI差異無統(tǒng)計學(xué)意義(P0.05);治療后,與對照組相比,研究組PI、DI、GI均較低,差異有統(tǒng)計學(xué)意義(P0.05);兩組不良反應(yīng)發(fā)生率差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論頜間牽引釘與微型鈦板內(nèi)固定聯(lián)合應(yīng)用于頜骨骨折治療中,可有效提高治療性優(yōu)良率,顯著縮短骨折愈合時間,且對牙周組織影響較小,安全性較高。
[Abstract]:Objective to investigate the effect of intermaxillary traction nail combined with mini-titanium plate internal fixation in the repair of jaw fracture. Methods from June 2013 to May 2015, 82 patients with jaw fracture were selected from the first affiliated Hospital of Shihezi University Medical College. According to different treatment methods, 82 patients were divided into control group and study group, 41 cases in each group. The control group was treated with micro titanium plate fixation and the study group was treated with intermaxillary traction nail and micro titanium plate fixation. The clinical efficacy, fracture healing time and adverse reaction were analyzed. The periodontal indexes (plaque index (Pi), scaling index (DI), gingival index (GI) were compared before and after treatment. Results there was no significant difference in operative time between the two groups (P0.05); compared with the control group, the fracture healing time in the study group was shorter and the difference was statistically significant (P0.05); the excellent and good rate of treatment in the two groups was higher than that in the control group (P0.05). The study group (97.56%) (40 / 41) was significantly higher than the control group (80.49% (33 / 41), the difference was statistically significant (P0.05), before treatment, there was no significant difference between the two groups (P0.05). The difference was statistically significant (P0.05), and there was no significant difference in the incidence of adverse reactions between the two groups (P0.05). Conclusion the combined use of intermaxillary traction nail and mini-titanium plate fixation in the treatment of maxillary fracture can effectively improve the excellent and good rate of treatment, shorten the healing time of fracture, and have little effect on periodontal tissue and high safety.
