【摘要】：目的:分析比較頜骨囊腫鄰近牙齒在手術(shù)前后的牙髓活力變化,探討頜骨囊腫鄰近牙齒根尖孔至囊腫的最近距離與牙髓活力變化的關(guān)系,從而為頜骨囊腫病變鄰近牙齒的術(shù)前處理提供臨床借鑒。方法:1、收集2016年4月至2016年12月期間,青島大學附屬醫(yī)院收治的33例頜骨囊腫(牙源性)患者的鄰近囊腫的57顆牙齒,應用牙髓活力電測試儀分別檢測這些牙齒在手術(shù)前1周內(nèi)與手術(shù)后3個月的牙髓活力值,并計算兩者之間的差值(記為d,d=術(shù)后3個月的牙髓活力值-術(shù)前1周內(nèi)的牙髓活力值)。2、手術(shù)前在CBCT(植入物屏幕)掃描圖像上,利用系統(tǒng)自帶的線性測量軟件測量頜骨鄰近牙齒的根尖孔至囊腫的最近距離(以D表示),以頜骨囊腫鄰近牙齒的根尖孔至囊腫的最近距離為依據(jù)設(shè)置為4個組,分別為:(1)R1組:D≤1mm,(2)R2組:1mmD≤2mm,(3)R3組:2mmD≤3mm,(4)R4組:D3mm。3、對4個組牙齒的術(shù)前術(shù)后牙髓活力值的差值及差值與根尖孔至囊腫的最近距離分別應用SPSS 22.0統(tǒng)計學軟件中的方差分析和spearman關(guān)聯(lián)性分析的統(tǒng)計學方法進行分析。結(jié)果:1、57顆牙齒在術(shù)后3個月有16顆死髓牙,占總數(shù)的28.07%。4個組在術(shù)前1周內(nèi)與術(shù)后3個月測試的牙髓活力值的差值的統(tǒng)計結(jié)果顯示:F=5.916,P=0.0010.05(檢驗水準α=0.05),差異具有統(tǒng)計學意義;R1組(D≤1mm)與其他3個組之間的比較結(jié)果中,標準誤sx分別為:8.353、7.953、7.519,P值均小于0.05(檢驗水準α=0.05),差異具有統(tǒng)計學意義,R2組(1mmD≤2mm)、R3組(2mmD≤3mm)、R4組(D3mm)間兩兩比較時,P值均大于0.05,差異無統(tǒng)計學意義。2、頜骨囊腫鄰近牙齒在手術(shù)前1周內(nèi)與手術(shù)后3個月的牙髓活力變化的差值(d)與這些牙齒的根尖孔至囊腫的最近距離(D)的關(guān)聯(lián)性分析結(jié)果顯示:r=-0.345,P=0.0090.05(檢驗水準α=0.05),兩者之間呈負相關(guān),且差異具有統(tǒng)計學意義。結(jié)論:1、囊腫鄰近牙齒術(shù)后死髓的概率為28.07%。2、囊腫鄰近牙齒的根尖孔至囊腫的距離≤1mm時,術(shù)后牙髓活力受損的機率高達64.71%,建議術(shù)前行根管治療,囊腫鄰近牙齒的根尖孔至囊腫的距離為1mm~3mm時,可予以觀察,囊腫鄰近牙齒的根尖孔至囊腫的距離3mm時,術(shù)前無需行根管治療。
[Abstract]:Objective: to compare the changes of pulp activity in teeth adjacent to maxillary cyst before and after operation, and to explore the relationship between the proximal distance from apical foramen to cyst and the change of pulp activity. Therefore, it can provide clinical reference for the preoperative treatment of adjacent teeth with jaw cyst disease. Methods from April 2016 to December 2016, 57 teeth were collected from 33 patients with maxillary cyst (odontogenic) treated in Qingdao University affiliated Hospital. The pulp activity of these teeth was measured 1 week before operation and 3 months after operation by using dental pulp activity test. The difference between the two was calculated (denoted d = the pulp activity value of 3 months postoperatively) and the pulp activity value of 1 week before operation was calculated. The CBCT (implant screen) scan image was performed before the operation. The proximal distance from apical foramen to cyst of adjacent teeth of jaw was measured by linear measurement software (expressed as D), and four groups were set according to the closest distance from apical foramen of teeth adjacent to maxillary cyst to cyst.
