美學(xué)區(qū)單牙種植修復(fù)齦乳頭高度相關(guān)影響因素的回顧性研究

【摘要】:[目的]分析鄰接處最低點(diǎn)到牙槽嵴頂?shù)木嚯x(CPB)和種植體與鄰牙之間的水平距離(ITD)兩個(gè)因素對(duì)上頜前牙美學(xué)區(qū)單個(gè)種植修復(fù)體齦乳頭高度的影響。為臨床醫(yī)生準(zhǔn)確地判斷治療效果,科學(xué)的設(shè)計(jì)手術(shù)方案、規(guī)避風(fēng)險(xiǎn)以及評(píng)估齦乳頭的再生可能提供參考。[方法]對(duì)2009年6月——2012年6月于煙臺(tái)市口腔醫(yī)院進(jìn)行上前牙美學(xué)區(qū)單牙種植修復(fù)并行使功能一年以上的患者進(jìn)行回顧性研究。共46例患者46枚Ankylos種植體納入本研究;颊吣挲g19~66歲,平均41.5歲,其中男性25例,女性21例;颊邚(fù)診時(shí)記錄臨床資料、拍攝口內(nèi)照片以及標(biāo)準(zhǔn)平行投照的X線片,將數(shù)字化的X線片導(dǎo)入電子測(cè)量軟件測(cè)量鄰接處最低點(diǎn)到牙槽嵴頂?shù)木嚯x(CPB),種植體與鄰牙之間的水平距離(ITD)。以統(tǒng)計(jì)軟件SPSS17.0進(jìn)行統(tǒng)計(jì)分析兩因素對(duì)齦乳頭存在的影響。[結(jié)果]對(duì)46名患者92個(gè)牙齦乳頭的研究顯示,ITD近遠(yuǎn)中的平均值分別為3.20±0.95mm,3.13±0.87mm, CPB近遠(yuǎn)中的平均值分別為5.6±1.7mm,5.6±1.6mm。當(dāng)鄰接處最低點(diǎn)到牙槽嵴頂?shù)木嚯x(CPB) 3-5mm,種植體與鄰牙之間的水平距離(ITD) 2.5-4mm時(shí),對(duì)于上頜前牙區(qū)單個(gè)種植體的牙齦乳頭的高度有顯著的影響(p0.05)。[結(jié)論]當(dāng)鄰接處最低點(diǎn)到牙槽嵴頂?shù)木嚯x(CPB) 3-5mm,種植體與鄰牙之間的水平距離(ITD) 2.5-4mm時(shí),容易獲得較理想的牙齦乳頭?梢宰鳛榕R床醫(yī)師評(píng)估前牙區(qū)單個(gè)種植修復(fù)體的美學(xué)效果,制定完善的治療計(jì)劃,準(zhǔn)確地預(yù)測(cè)治療效果的依
[Abstract]:[objective] to analyze the influence of the distance from the lowest point adjacent to the alveolar crest (CPB) and the horizontal distance between implant and adjacent teeth (ITD) on the height of gingival papilla in the aesthetic area of maxillary anterior teeth. It may provide a reference for clinicians to accurately judge the therapeutic effect, to design the surgical plan scientifically, to avoid the risk and to evaluate the regeneration of the gingival papilla. [methods] A retrospective study was carried out on the patients with single-tooth implant restoration in the aesthetic area of anterior teeth from June 2009 to June 2012 in Yantai Stomatological Hospital. A total of 46 Ankylos implants were included in this study. The age of the patients was 19- 66 years (mean 41.5 years), including 25 males and 21 females. Clinical data were recorded, intraoral photographs were taken and standard parallel radiographs were taken. The digital X-ray film was introduced into electronic measurement software to measure the distance between the lowest point of adjacent site and the top of alveolar ridge, and the horizontal distance between implant and adjacent teeth was measured. The influence of two factors on gingival papilla was analyzed by SPSS 17.0 software. [results] A study of 92 gingival papillaries in 46 patients showed that the average values of ITD in the proximal distance were 3.20 鹵0.95mm / 3.13 鹵0.87mm, and those of CPB were 5.6 鹵1.7mm / 5.6 鹵1.6mm. When the distance from the lowest point adjacent to the alveolar crest is 3-5 mm, and the horizontal distance between implant and adjacent teeth is 2.5-4mm, the height of gingival nipples of a single implant in the maxillary anterior tooth region is significantly affected. [conclusion] when the distance from the lowest point adjacent to the alveolar crest is 3-5 mm and the horizontal distance between implant and adjacent teeth is 2.5-4mm, it is easy to obtain ideal gingival papilla. It can be used as a clinician to evaluate the aesthetic effect of a single implant prosthesis in the anterior tooth area, formulate a perfect treatment plan, and accurately predict the efficacy of the treatment.
