Pyk2和p-AKT在舌癌組織中的表達(dá)及臨床意義

【摘要】:目的:探討富含脯氨酸的酪氨酸激酶2(Pyk2)和磷酸化的絲氨酸/蘇氨酸蛋白激酶(p-AKT)在舌鱗狀細(xì)胞癌組織(TSCC)及其癌旁非腫瘤組織中的表達(dá)差異及臨床意義。方法:收集青島大學(xué)附屬煙臺(tái)毓璜頂醫(yī)院2012年1月至2016年5月手術(shù)切除的45例舌癌組織標(biāo)本及其配對(duì)癌旁非腫瘤組織30例作為研究對(duì)象。采用免疫組織化學(xué)法檢測(cè)Pyk2、p-AKT蛋白的表達(dá)情況。應(yīng)用SPSS19.0統(tǒng)計(jì)學(xué)軟件分析二者蛋白表達(dá)水平與舌癌患者性別、年齡、腫瘤直徑、病理分化程度、臨床分期、淋巴結(jié)轉(zhuǎn)移等臨床病理學(xué)參數(shù)之間的關(guān)系以及Pyk2、p-AKT蛋白的相關(guān)性。結(jié)果:(1)Pyk2和p-AKT蛋白在舌癌組織中陽性表達(dá)率高于癌旁非腫瘤組織。Pyk2蛋白在舌癌組織、癌旁非腫瘤組織中的陽性表達(dá)率分別為57.78%、6.67%(χ2=17.937,P=0.000),并且p-AKT陽性表達(dá)率分別為55.56%、16.67%(χ2=9.780,P=0.002),差異均較明顯,具有統(tǒng)計(jì)學(xué)意義。(2)Pyk2蛋白的表達(dá)在病理分化程度中,高分化者陽性表達(dá)率為46.87%,中、低分化者陽性表達(dá)率約為84.62%(P0.05)。TNM分期中,Ⅰ-Ⅱ期陽性表達(dá)率約為41.94%,Ⅲ-Ⅳ期陽性表達(dá)率為92.85%(P0.05)。有淋巴結(jié)轉(zhuǎn)移組陽性表達(dá)率約為91.67%,無淋巴結(jié)轉(zhuǎn)移組陽性表達(dá)率約為45.45%(P0.05)。Pyk2蛋白的表達(dá)與病理分化程度、臨床分期及區(qū)域淋巴結(jié)轉(zhuǎn)移有關(guān),而與性別、年齡、腫瘤直徑之間無統(tǒng)計(jì)學(xué)意義。(3)p-AKT蛋白的表達(dá)在病理分化程度中,高分化者陽性表達(dá)率約為43.75%,中、低分化者陽性表達(dá)率為84.62%,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。p-AKT蛋白的表達(dá)只與病理分化程度有關(guān),與性別、年齡、腫瘤直徑、臨床分期及區(qū)域淋巴結(jié)轉(zhuǎn)移無關(guān)。(4)45例舌癌組織中Pyk2陽性表達(dá)26例,其中p-AKT共同表達(dá)陽性16例。(5)30例癌旁非腫瘤組織中,Pyk2、p-AKT蛋白同時(shí)表達(dá)陰性13例。經(jīng)Spearman秩相關(guān)分析,二者呈正相關(guān)(P=0.005,γs=0.412)。結(jié)論:在舌癌組織中Pyk2、p-AKT蛋白呈高表達(dá),而在癌旁非腫瘤組織中呈低表達(dá)甚至無表達(dá)。Pyk2蛋白的表達(dá)與病理分化程度、臨床分期及區(qū)域淋巴結(jié)轉(zhuǎn)移有關(guān),而p-AKT蛋白的表達(dá)只與病理分化程度有關(guān)。p-AKT代表了P13K/AKT信號(hào)通路的的激活狀態(tài),Pyk2作為該通路的上游分子,二者之間表現(xiàn)出正相關(guān),提示Pyk2可能通過參與P13K/AKT信號(hào)轉(zhuǎn)導(dǎo)通路引起或促進(jìn)舌癌的發(fā)生、發(fā)展,聯(lián)合檢測(cè)二者有望成為TSCC惡性程度的指標(biāo)。并可能作為新的治療靶點(diǎn)應(yīng)用于腫瘤的生物靶向治療,為舌癌的早期篩查、臨床診斷、預(yù)后評(píng)價(jià)與監(jiān)測(cè)提供新標(biāo)識(shí)。
[Abstract]:Objective: To investigate the difference and clinical significance of the expression of proline rich tyrosine kinase 2 (Pyk2) and phosphorylated serine / threonine protein kinase (p-AKT) in tongue squamous cell carcinoma (TSCC) and adjacent nontumor tissues. Methods: 45 cases of surgical excision from January 2012 to May 2016 of the affiliated Yantai Yuhuangding Hospital of Qiingdao University were collected. 30 cases of tongue cancer tissue and 30 cases of non cancer tissue adjacent to cancer were used as research subjects. Immunohistochemical method was used to detect the expression of Pyk2 and p-AKT protein. The expression level of two proteins was analyzed by SPSS19.0 statistics software and the sex, age, tumor diameter, pathological differentiation, clinical staging, lymph node metastasis, and so on. The relationship between the pathological parameters and the correlation of Pyk2 and p-AKT protein. Results: (1) the positive expression rate of Pyk2 and p-AKT protein in tongue cancer tissues was higher than that of.Pyk2 protein in the nontumor tissues of the noncancerous tissues, and the positive expression rates in non tumor tissues were 57.78%, 6.67% (2=17.937, P=0.000), respectively, and the positive expression rate of p-AKT was respectively 55.56%, 16.67% (x 2=9.780, P=0.002), the difference was significant. (2) the expression of Pyk2 protein was in the degree of pathological differentiation, the positive expression rate of the high differentiation was 46.87%, and the positive expression rate of the low differentiation was about 84.62% (P0.05).TNM stage, the positive expression rate of I to II was about 41.94%, and the positive expression rate of stage III to IV was 92.85% (P0.0 5) the positive expression rate in the lymph node metastasis group was about 91.67%, the positive expression rate of the non lymph node metastasis group was about 45.45% (P0.05), the expression of.Pyk2 protein was related to the degree of pathological differentiation, the clinical stage and the regional lymph node metastasis, but there was no statistical significance between the sex, age and tumor diameter. (3) the expression of p-AKT protein was in the degree of pathological differentiation, The positive expression rate of the high differentiation was about 43.75%, and the positive expression rate of the low differentiation was 84.62%. The difference was statistically significant (P0.05) the expression of.P-AKT protein was only related to the degree of pathological differentiation, and was not related to sex, age, tumor diameter, clinical stage and regional lymph node metastasis. (4) 26 cases of positive expression of Pyk2 in 45 cases of tongue cancer, of which p-AKT was common. 16 cases of the same expression were positive. (5) 30 cases of non tumor tissues adjacent to cancer, Pyk2 and p-AKT protein were negative in 13 cases. The two was positive correlation (P=0.005, gamma s=0.412) by Spearman rank correlation analysis. Conclusion: Pyk2, p-AKT protein is highly expressed in the tongue cancer tissue, and the expression of low expression or no expression of.Pyk2 protein in the nontumor tissue adjacent to the carcinoma and even no expression of.Pyk2 protein The degree of differentiation, clinical staging and regional lymph node metastasis, and the expression of p-AKT protein only associated with the degree of pathological differentiation.P-AKT represents the activation state of the P13K/AKT signaling pathway. Pyk2 is the upstream molecule of the pathway, and there is a positive correlation between the two, suggesting that Pyk2 may be caused or promoted by the participation of the P13K/AKT signal transduction pathway. The occurrence, development and joint detection of tongue cancer are expected to be an indicator of the malignancy of TSCC, and may be used as a new target for the biological target therapy of tumor. It provides a new identification for the early screening, clinical diagnosis, prognosis evaluation and monitoring of tongue cancer.
