【摘要】：目的:隨著人們生活條件的改善,人們對美的追求越來越高,因此,關注錯頜畸形的人也日益增多,越來越多的患者選擇接受固定正畸治療。牙齦炎是固定矯治過程中常見的并發(fā)癥之一,對患者進行常規(guī)口腔衛(wèi)生宣教及牙周治療效果往往不夠盡善盡美。有研究表明,低強度激光(1ow level laser,LLL)具有生物調(diào)節(jié)作用,能夠減輕組織炎癥反應,促進傷口愈合。本研究通過臨床實驗,探討低強度激光治療(1ow level laser therapy,LLLT)對固定正畸牙齦炎的療效,為口腔臨床工作者對固定正畸牙齦炎的治療提供參考,為LLL在正畸臨床上的應用提供理論依據(jù)。方法:選取固定正畸牙齦炎患者20例,年齡12-18歲(男12例,女8例),平均年齡15.2歲。實驗前對每位患者進行常規(guī)的口腔衛(wèi)生宣教及牙周潔治,徹底清除牙石、軟垢。牙周基礎治療一個周后,每位患者選擇31、32、41、42四顆牙為指數(shù)牙,左、右側隨機分為實驗組和對照組,測量兩組的菌斑指數(shù)(Plaque index,PLI)、牙齦指數(shù)(Gingival index,GI)、出血指數(shù)(Blooding index,BI)、探診深度(probing depth,PD)四個牙周臨床指標,并檢測兩組齦溝液中IL-1β的濃度,其數(shù)據(jù)定為“治療前”。實驗組牙齦給予低強度激光治療(650nm,25mw),1次/日,連照3天;對照組不作處理。一周后,再次測量實驗組和對照組四種牙周臨床指標及齦溝液中IL-1β的濃度,其數(shù)據(jù)定為“治療后”。所有檢查由同一醫(yī)師完成。比較治療前、后各項指標并進行統(tǒng)計學處理。結果:1.牙周臨床指標的比較:實驗組與對照組PLI、GI、BI、PD值治療前比較差異均無統(tǒng)計學意義,治療后四項指標,實驗組均明顯低于對照組,差異有統(tǒng)計學意義(p0.05)。2.齦溝液IL-1β濃度的比較:治療前實驗組與對照組齦溝液中IL-1β濃度比較無統(tǒng)計學差異,治療后實驗組齦溝液中IL-1β濃度明顯低于對照組,差異有統(tǒng)計學意義(p0.05)。結論:LLLT對固定正畸患者牙齦炎有一定的防治作用。
[Abstract]:Objective: with the improvement of people's living conditions, people's pursuit of beauty is becoming higher and higher. Therefore, more and more people pay attention to malocclusion, and more patients choose to receive fixed orthodontic treatment. Gingivitis is one of the common complications in the process of fixation and correction. Some studies have shown that low intensity laser 1ow level laser LLL) has the biological regulation effect, can reduce the inflammation reaction of the tissue and promote wound healing. The purpose of this study was to investigate the efficacy of low intensity laser in the treatment of orthodontic gingivitis, to provide reference for the treatment of orthodontic gingivitis and to provide theoretical basis for the clinical application of LLL in orthodontic treatment. Methods: twenty patients with fixed orthodontic gingivitis aged 12 to 18 years (12 males and 8 females) with an average age of 15.2 years were selected. Every patient was given routine oral hygiene education and periodontal cleaning before the experiment. After a week of basic periodontal treatment, each patient was randomly divided into experimental group and control group. The plaque index (pli), gingival index (Gingival index), bleeding index (Blooding index) and probing depth probing (PDD) were measured. The concentration of IL-1 尾 in gingival crevicular fluid was determined as "before treatment". The gingiva of the experimental group was treated with low intensity laser therapy once a day, and the control group was not treated. One week later, four periodontal clinical indexes and the concentration of IL-1 尾 in gingival crevicular fluid were measured again in the experimental group and the control group. All examinations are performed by the same physician. The indexes before and after treatment were compared and statistically analyzed. The result is 1: 1. Comparison of periodontal clinical indexes: there was no significant difference between the experimental group and the control group before and after treatment. After treatment, the four indexes in the experimental group were significantly lower than those in the control group, and the difference was statistically significant (P 0.05 路2). Comparison of IL-1 尾 concentration in gingival crevicular fluid: there was no significant difference in IL-1 尾 concentration between experimental group and control group before treatment. After treatment, IL-1 尾 concentration in gingival crevicular fluid in experimental group was significantly lower than that in control group (P 0.05). Conclusion: WL LLLT can prevent and cure gingivitis in fixed orthodontic patients.
