【摘要】：背景:上頜后牙缺損修復(fù)可以采用種植牙技術(shù),但以往的種植模板大多利用傳統(tǒng)石膏模型完成,無法有效保證種植體植入的準(zhǔn)確性,容易導(dǎo)致各種并發(fā)癥的出現(xiàn),影響修復(fù)效果。目的:探討計(jì)算機(jī)輔助設(shè)計(jì)和計(jì)算機(jī)輔助制作外科導(dǎo)板在上頜后牙種植修復(fù)過程中的應(yīng)用效果。方法:納入30例上頜后牙牙列缺損患者,均實(shí)施種植修復(fù)治療。經(jīng)CT上頜骨掃描,利用計(jì)算機(jī)輔助設(shè)計(jì)/計(jì)算機(jī)輔助制造技術(shù)(computer aided design/computer aided manufacturing technology,CAD/CAM)制作外科導(dǎo)板,在導(dǎo)板輔助下植入種植體進(jìn)行修復(fù)。植入后測量虛擬植入位置和實(shí)際植入的偏差情況,隨訪12個(gè)月,觀察臨床修復(fù)效果與患者口腔牙周組織情況。結(jié)果與結(jié)論:(1)30例患者一共涉及36個(gè)CAD/CAM外科導(dǎo)板,經(jīng)患者戴入口腔之后均獲得穩(wěn)定的固位效果;(2)36枚種植體,均在外科導(dǎo)板輔助下順利完成準(zhǔn)確的,植入過程未出現(xiàn)任何問題;(3)植入后種植體的虛擬植入位置和實(shí)際植入位置之間存在的頭部偏離情況極小,不會對臨床種植體植入精度產(chǎn)生影響;(4)植入前與植入后6及12個(gè)月患者的口腔牙周組織齦溝液、腫瘤壞死因子α、齦溝出血指數(shù)無明顯變化;(5)結(jié)果說明在上頜后牙臨床治療過程中,利用CAD/CAM外科導(dǎo)板進(jìn)行種植體修復(fù)治療會出現(xiàn)一定的種植體頭部偏離和角度偏離現(xiàn)象,但并不影響種植體的植入精度,修復(fù)效果理想,且治療后患者處于良好的牙周組織狀態(tài)。
[Abstract]:Background: dental implants can be used to repair maxillary posterior tooth defects. However, most of the implant templates used in the past were completed by traditional plaster models, which could not effectively guarantee the accuracy of implant implants and could easily lead to various complications. Affect the effect of restoration. Objective: to investigate the effect of computer aided design (CAD) and computer aided fabrication (CAD) of surgical guide plate in the process of maxillary posterior tooth implant restoration. Methods: 30 patients with maxillary posterior dentition defect were treated with implant repair. Through CT scanning of maxilla, the surgical guide plate was made by computer aided design/computer aided manufacturing technology (CAD / CAD / CAM), and the implant was implanted with the aid of the guide board for repair. After implantation, the deviation between the virtual implant location and the actual implantation was measured and followed up for 12 months to observe the effect of clinical restoration and the oral periodontal tissue of the patients. Results and conclusion A total of 36 CAD / CAM surgical guides were involved in 30 patients with 1 / 30 cases, and 236 implants were successfully completed with the aid of surgical guide plate after wearing them into the oral cavity. There is no problem with the implant.) there is a minimal head deviation between the virtual implant location and the actual implant location. Gingival crevicular fluid (GCF) in oral periodontal tissue of patients 6 and 12 months before and 6 and 12 months after implantation did not affect the accuracy of clinical implants. Tumor necrosis factor 偽, no significant change in gingival sulcus bleeding index (TNF- 偽). The results showed that during the clinical treatment of maxillary posterior teeth, implant prosthesis repair with CAD / CAM surgical guide board would lead to a certain deviation of implant head and angle. But it does not affect the implant implantation precision, the effect of restoration is ideal, and the patients are in good periodontal tissue state after treatment.
