替牙期單側(cè)完全性唇腭裂患者下頜骨對(duì)稱性的錐形束CT研究

【摘要】:目的:對(duì)替牙期單側(cè)完全性唇腭裂(Unilateral cleft lip and palate,UCLP)患者與非CLP(Cleft lip and palate,CLP)患者的下頜骨的錐形束CT(Cone-beam computed tomography,CBCT)影像資料進(jìn)行對(duì)比分析,研究該類患者下頜骨的對(duì)稱性、探討替牙期UCLP患者下頜骨不對(duì)稱的部位和原因,為了解該類患者頜骨的形態(tài)特點(diǎn)、正畸臨床準(zhǔn)確診斷、治療提供理論依據(jù)和支持。方法:選取在青島大學(xué)附屬醫(yī)院口腔正畸科就診的替牙期UCLP患者20例(男12例,女8例,年齡9.24±1.56歲,頸椎骨齡9.13±1.53歲)作為UCLP組,另選替牙期非CLP患者20例(男12例,女8例,年齡9.22±1.39歲,頸椎骨齡9.20±1.46歲)作為對(duì)照組。對(duì)以上所有研究對(duì)象進(jìn)行CBCT掃描,獲得CBCT影像,利用Mimics17.0軟件對(duì)下頜骨進(jìn)行三維重建,在重建的三維圖像上確定髁突頂點(diǎn)Con、髁突外點(diǎn)Conl、髁突內(nèi)點(diǎn)Conm、喙突頂點(diǎn)Cor、下頜孔下點(diǎn)F等26個(gè)標(biāo)志點(diǎn),并對(duì)雙側(cè)下頜骨喙突長度、髁突長度、下頜角長度、下頜體中段長度、頦部長度、下頜升支長度、下頜體長度、髁突橫徑、尖牙區(qū)高度、尖牙區(qū)寬度、磨牙區(qū)高度、磨牙區(qū)寬度、下頜升支體積以及半側(cè)下頜骨體積進(jìn)行測(cè)量。所得數(shù)據(jù)使用SPSS22.0統(tǒng)計(jì)軟件進(jìn)行統(tǒng)計(jì)分析,對(duì)UCLP組裂隙側(cè)和非裂隙側(cè)、對(duì)照組的左右兩側(cè)分別進(jìn)行配對(duì)t檢驗(yàn),比較UCLP組患者和對(duì)照組雙側(cè)下頜骨各段長度、寬度、高度和體積的差異,分析UCLP組與對(duì)照組下頜骨對(duì)稱性的差異。結(jié)果:1.替牙期UCLP組患者裂隙側(cè)與非裂隙側(cè)下頜骨對(duì)稱性比較:替牙期UCLP患者裂隙側(cè)喙突長度、磨牙區(qū)頜骨高度、下頜升支體積均大于非裂隙側(cè),且差異具有統(tǒng)計(jì)學(xué)意義(P0.05);其余各項(xiàng)測(cè)量項(xiàng)目比較無統(tǒng)計(jì)學(xué)差異(P0.05)。2.對(duì)照組左右兩側(cè)下頜骨對(duì)稱性比較:替牙期非CLP患者左右兩側(cè)各測(cè)量項(xiàng)目比較無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:1.替牙期UCLP患者下頜骨存在不對(duì)稱性:其中裂隙側(cè)喙突長度、磨牙區(qū)頜骨高度、下頜升支體積大于非裂隙側(cè);其余下頜骨結(jié)構(gòu)基本對(duì)稱。2.替牙期非CLP患者左右兩側(cè)下頜骨具有較好的對(duì)稱性。
[Abstract]:Objective: to study the symmetry of mandible in patients with unilateral cleft lip and palate (unilateral cleft lip and palate or UCLP) and those without CLP(Cleft lip and palateur (UCLP) with conical bundle CT(Cone-beam computed tomographyCT-CTs (CBCTs), and to study the symmetry of mandible in patients with unilateral cleft lip and palate. To investigate the position and cause of mandibular asymmetry in patients with UCLP in dental replacement stage, and to provide theoretical basis and support for understanding the morphological characteristics of mandible, accurate diagnosis of orthodontic clinic and treatment of this kind of patients. Methods: twenty patients (male 12, female 8, age 9.24 鹵1.56 years, cervical vertebra bone age 9.13 鹵1.53 years) with UCLP in the orthodontic department of affiliated Hospital of Qingdao University were selected as UCLP group. Age 9.22 鹵1.39 years old, cervical spine bone age 9.20 鹵1.46 years old) as control group. All the subjects were scanned by CBCT, CBCT images were obtained, and the mandible was reconstructed by Mimics17.0 software. The condylar vertex Conl, condylar outer point, condylar internal point Conm, coracoid vertex Corm, mandibular foramen inferior point F were determined on the reconstructed 3D images. The bilateral mandibular coracoid process length, condylar process length, mandibular angle length, and middle mandibular length were determined. Chin length, mandibular ramus length, mandibular body length, transverse diameter of condyle, height of canine region, width of canine region, height of molar region, width of molar region, volume of mandibular ascending branch and volume of hemilateral mandible were measured. The data were statistically analyzed by SPSS22.0 software. The fracture side and non-fissure side of UCLP group and the left and right sides of the control group were matched with t test respectively. The length and width of bilateral mandibular segments in UCLP group and control group were compared. The height and volume of mandibular symmetry in UCLP group and control group were analyzed. The result is 1: 1. Comparison of mandibular symmetry between fissure side and unfractured side in UCLP group: the length of coracoid process, the height of mandible in molar region, and the volume of mandibular ascending branch in patients with UCLP at replacement stage were all larger than those in non-fractured side. The difference was statistically significant (P 0.05), and there was no statistical difference between the other items (P 0.05). 2. Symmetry comparison of left and right mandibles in the control group: there was no significant difference between the left and right sides of the patients with CLP in the stage of tooth replacement (P 0.05). Conclusion 1. There were asymmetry in mandible of UCLP patients in replacement stage: the length of coracoid process of fissure, the height of mandible in molar region, the volume of mandibular ramus was larger than that of non-fissure side, and the other mandibular structure was basically symmetrical. 2. The left and right mandibles of non-CLP patients at dental replacement stage have good symmetry.
