【摘要】：背景:目前有多種修復(fù)方法可以應(yīng)用于臨床下頜骨修復(fù)重建,但在修復(fù)過程依然存在一些問題有待解決。目的:總結(jié)下頜骨缺損重建的方法及進(jìn)展,面臨的難題,展望下頜骨缺損重建的發(fā)展方向。方法:由第一作者用計(jì)算機(jī)檢索PubMed數(shù)據(jù)庫(kù)、萬(wàn)方數(shù)據(jù)庫(kù)和CNKI數(shù)據(jù)庫(kù)1985年至2017年有關(guān)下頜骨缺損重建進(jìn)展的文獻(xiàn),并進(jìn)行篩選、歸納和總結(jié)。英文檢索詞為"Mandibular reconstruction;bone tissue engineering;prosthesis device;operative complications",中文檢索詞為"下頜骨重建;骨組織工程;修復(fù)體裝置;術(shù)區(qū)并發(fā)癥"。從下頜骨缺損的分類、重建方法等方面進(jìn)行總結(jié),探討下頜骨重建存在的難題及未來發(fā)展方向。結(jié)果與結(jié)論:納入文獻(xiàn)48篇。(1)下頜骨缺損較為復(fù)雜,合理的缺損分類是下頜骨修復(fù)重建和臨床治療的關(guān)鍵;(2)對(duì)于因腫瘤切除導(dǎo)致的下頜骨缺損,快速修復(fù)更有利于面部外形和功能恢復(fù),已逐步獲得認(rèn)可;(3)目前下頜骨缺損重建的常用方法主要包括非血管化骨移植和血管化骨移植等,但手術(shù)創(chuàng)傷大,并發(fā)癥的發(fā)生,難以恢復(fù)咬合重建等是困擾臨床醫(yī)生的難題�？焖俪尚图夹g(shù)、計(jì)算機(jī)輔助設(shè)計(jì)與制造技術(shù)的出現(xiàn),不僅實(shí)現(xiàn)了手術(shù)的精確度,還顯著縮短了手術(shù)時(shí)間,使患者獲得良好的重建效果;(4)組織工程技術(shù)在下頜骨缺損重建修復(fù)方面具有廣闊的應(yīng)用前景,其與計(jì)算機(jī)輔助設(shè)計(jì)與制造技術(shù)的結(jié)合,能夠?qū)崿F(xiàn)下頜骨功能和形態(tài)的修復(fù),但是對(duì)于大塊下頜骨缺損修復(fù)的長(zhǎng)期效果評(píng)估仍需進(jìn)一步研究。
[Abstract]:Background: at present, there are a variety of repair methods can be used in clinical mandibular reconstruction, but there are still some problems to be solved in the process of restoration. Objective: to summarize the methods and progress of mandibular defect reconstruction, and to look forward to the development direction of mandibular defect reconstruction. Methods: the first author searched the literature about mandibular defect reconstruction from 1985 to 2017 in PubMed database, Wanfang database and CNKI database. The Chinese key words were "mandibular reconstruction; bone tissue engineering; prosthetic device; complications of the operation area"; and the Chinese key words were "mandibular reconstruction; bone tissue engineering; prosthetic device; complications of the operation area", and the Chinese key words were "mandibular reconstruction; bone tissue engineering; prosthetic device; operative area complication". This paper summarizes the classification and reconstruction methods of mandibular defects, and discusses the problems and future development of mandibular reconstruction. Results and conclusion: the mandibular defects included in 48 references were complicated, and the reasonable classification of mandibular defects was the key to the reconstruction and clinical treatment of mandible. Rapid repair is more conducive to facial shape and functional recovery, and has gradually gained recognition. At present, the commonly used methods for reconstruction of mandibular defects include non-vascularized bone transplantation and vascularized bone transplantation, but the surgical trauma is great and complications occur. Difficult to restore occlusal reconstruction is a difficult problem for clinicians. The emergence of rapid prototyping, computer-aided design and manufacturing technology has not only achieved the accuracy of the operation, but also significantly shortened the operation time. The technique of tissue engineering has a broad application prospect in the reconstruction of mandibular defect. The combination of the technique of computer aided design and manufacture can realize the functional and morphological restoration of mandible. However, the evaluation of long-term effect of large mandibular defects still needs further study.
