【摘要】：目的:探討前牙種植患者術(shù)前的精神因素對(duì)術(shù)后疼痛及滿意度的影響。方法:隨機(jī)選取2013年3月至2015年6月于重慶醫(yī)科大學(xué)附屬口腔醫(yī)院種植科就診的前牙缺失患者,符合納入標(biāo)準(zhǔn)自愿選擇種植義齒修復(fù)前牙缺失的患者92例,術(shù)前對(duì)其進(jìn)行癥狀自評(píng)量表(symptom checklist-90,SCL90)和狀態(tài)-特質(zhì)焦慮量表(state-trait anxiety inventory,STAI)測(cè)評(píng),術(shù)后進(jìn)行痛視覺模擬評(píng)分(visual analogue scale,VAS)及自我滿意療效評(píng)分,測(cè)試每位受試者的精神心理狀況,并按患者焦慮程度分組與術(shù)后疼痛評(píng)分和自我滿意評(píng)分進(jìn)行Pearson相關(guān)性分析。結(jié)果:納入的前牙缺失患者SCL90總分高于全國(guó)常模;單項(xiàng)目分析中,前牙缺失患者與全國(guó)常模SCL90單項(xiàng)平均分比較,除焦慮項(xiàng)目得分高出12.9%外,其余項(xiàng)目得分與全國(guó)常模差異不明顯。按量表得分相關(guān)標(biāo)準(zhǔn)分組,不同狀態(tài)焦慮量表(state anxiety inventory,SAI)分組之間的術(shù)后疼痛評(píng)分和自我滿意評(píng)分差異都具有統(tǒng)計(jì)學(xué)意義(P0.05);不同特質(zhì)焦慮量表(trait anxiety inventory,TAI)分組之間的術(shù)后疼痛評(píng)分和自我滿意評(píng)分差異都具有統(tǒng)計(jì)學(xué)意義(P0.05),且趨勢(shì)與SAI一致。種植術(shù)前SAI、TAI和焦慮(SCL90)評(píng)分之間均有顯著的正相關(guān)關(guān)系,SAI評(píng)分與術(shù)后疼痛評(píng)分(R=0.471,P=0.024)、自我滿意(R=0.492,P=0.031)之間存在相關(guān)性;TAI評(píng)分同樣與術(shù)后疼痛評(píng)分(R=0.528,P=0.041)、自我滿意(R=0.573,P=0.047)之間存在相關(guān)性。結(jié)論:前牙缺失患者種植術(shù)前的精神因素與術(shù)后疼痛及滿意度之間具有一定的相關(guān)性,以后研究應(yīng)該綜合必要的影響因素來分析種植修復(fù)患者的精神因素,尋找有無體內(nèi)物質(zhì)變化,尋找易于推行的干預(yù)措施,將有助于加強(qiáng)醫(yī)患溝通,提高前牙種植修復(fù)的臨床診療質(zhì)量。
[Abstract]:Objective: to investigate the effect of psychological factors on postoperative pain and satisfaction in anterior dental implant patients. Methods: from March 2013 to June 2015, 92 patients with anterior tooth defect were randomly selected from the Department of Orthodontics, affiliated Stomatology Hospital of Chongqing Medical University. Symptom check list-90 (SCL90) and state-trait anxiety inventory (STAI) were evaluated before operation. Visual analogue scale (VASS) and self-satisfaction score were performed after operation. Pearson correlation analysis was carried out according to the degree of anxiety and postoperative pain score and self-satisfaction score. Results: the total score of SCL90 in the patients with anterior tooth loss was higher than that in the national norm, and in the single item analysis, the average score of the patients with anterior tooth loss was 12.9% higher than that of the national norm SCL90. The other items score and national norm difference is not obvious. According to the relevant criteria of the scale score, There were statistically significant differences in postoperative pain scores and self-satisfaction scores among groups with different state anxiety inventory (SAI), and the difference in postoperative pain scores and self-satisfaction scores between groups with different trait anxiety inventory (TAI) was significant (P < 0.05). All of them had statistical significance (P 0.05), and the trend was consistent with that of SAI. There was a significant positive correlation between SAITAI and anxiety SCL90 (P < 0.05). There was a correlation between SAI score and postoperative pain score (P = 0.471a, P = 0.024, P = 0.492P = 0.031). The score of Tai was also correlated with the postoperative pain score (R0.528P0.041) and self-satisfaction score (0.573P0. 047). Conclusion: there is a certain correlation between psychological factors before implantation and postoperative pain and satisfaction in patients with missing anterior teeth. It is helpful to strengthen the communication between doctors and patients and to improve the quality of clinical diagnosis and treatment.
