下頜第二恒磨牙C型根根管壁厚度的顯微CT分析

【摘要】:目的:測量下頜第二恒磨牙C型根的根管壁厚度。方法:采集到中國漢族患者拔除的C形根下頜第二恒磨牙40顆。顯微CT掃描后采用Mimics 10.01軟件進行三維重建;沿著牙體長軸從釉牙骨質(zhì)界(CEJ)到根尖頂每隔1 mm連續(xù)虛擬橫切牙根模型,觀測牙根橫截面上C形根管的類型(范兵分型),測量唇、舌側(cè)以及近、遠(yuǎn)中參考位點的根管壁厚度。雙因素方差分析及LSD-t檢驗比較不同方位的根管壁厚度。結(jié)果:40個下頜第二恒磨牙C形根被虛擬切割為370個根管橫截面,其中C1根管126個(34.1%),C2根管46個(12.4%),C3根管160個(43.2%)。除了CEJ下10 mm水平,舌側(cè)根管壁最小厚度均值總是小于頰側(cè),而兩者又均小于近、遠(yuǎn)中參考位點處的根管壁厚度。除了CEJ下12 mm水平,4個測量位點根管壁厚度均值間的差異均有統(tǒng)計學(xué)意義(P0.05)。C2和C3根管的頰側(cè)壁最小厚度在近中區(qū)最多見,C1根管頰、舌側(cè)以及C2、C3根管的舌側(cè)壁最小厚度多位于中央?yún)^(qū)。結(jié)論:下頜第二恒磨牙C型根管舌側(cè)根管壁較薄,是根管及樁道預(yù)備的危險區(qū)。
[Abstract]:Objective: to measure the thickness of root canal wall of C type root of mandibular second permanent molar. Methods: forty C-shaped mandibular second permanent molars were extracted from Chinese Han patients. Three-dimensional reconstruction was performed by Mimics 10.01 software after microCT scanning, and a continuous virtual incised root model was established from the enamel bone boundary along the long axis of the tooth to the root tip at intervals of 1 mm. The types of C-shaped root canals on the cross section of the root were observed. The thickness of the root canal wall at the lingual side and near and distal reference sites. The thickness of root canal wall in different azimuth was compared by two factor ANOVA and LSD-t test. Results: C shaped root of 40 mandibular second permanent molars was cut into 370 root canal cross sections, among them, C 1 root canal 126 and C 2 root canal 46 and C 3 root canal 160. Except for the 10 mm level under CEJ, the mean minimum thickness of the lingual lateral root wall was always smaller than that of the buccal side, and both of them were less than the proximal, distal reference site. In addition to the 12 mm level under CEJ, there were statistically significant differences in the mean thickness of root canal wall between the four measuring sites. The minimum thickness of the buccal lateral wall of the root canal P0.05, C2 and C3 was the most common in the proximal middle region. The minimum thickness of the lateral wall of tongue and C _ 2 C _ 3 root canal is located in the central region. Conclusion: the lateral root canal wall of C type root canal of mandibular second permanent molar is thin, which is a dangerous area for root canal and post canal preparation.
