【摘要】：背景慢性牙周炎（chronic periodontitis，CP）是我國成年人喪失牙齒的首位原因。類風(fēng)濕性關(guān)節(jié)炎（Rheumatoid arthritis，RA）是一類發(fā)病機(jī)制尚未完全闡明的自身免疫性疾病。研究證明，牙周炎和類風(fēng)濕性關(guān)節(jié)炎二者在免疫病理學(xué)上有許多相似之處，由于牙周炎和類風(fēng)濕性關(guān)節(jié)炎是如此類似，有學(xué)者甚至認(rèn)為他們是同一類疾病。近年來研究的熱點(diǎn)在于牙周炎與類風(fēng)濕性關(guān)節(jié)炎在臨床上的相關(guān)性。 目的本研究通過比較慢性牙周炎患者和健康人群中血清中C-反應(yīng)蛋白（C-Reactive Protein，，CRP）和類風(fēng)濕因子（Rheumatoid Factor，RF）的濃度水平，研究慢性牙周炎患者與健康人群的類風(fēng)濕因子及C-反應(yīng)蛋白水平的狀況，進(jìn)而進(jìn)一步分析了慢性牙周炎與類風(fēng)濕性關(guān)節(jié)炎的關(guān)聯(lián)。本文將針對慢性牙周炎與類風(fēng)濕性關(guān)節(jié)炎疾病在臨床上的關(guān)系進(jìn)行研究。 方法本研究在獲得患者知情同意并簽署同意書的基礎(chǔ)上，進(jìn)行了血清相關(guān)因子的醫(yī)學(xué)檢測。研究對象選擇慢性牙周炎患者和健康人群，采集研究對象的靜脈血，進(jìn)行血清CRP及RF的檢測。針對采集到的血清檢測得出的CRP及RF水平的數(shù)據(jù)：慢性牙周炎組數(shù)據(jù)（41組）健康對照組（54組）慢性牙周炎牙周系統(tǒng)治療后3個月（11組），對其進(jìn)行研究，運(yùn)用統(tǒng)計(jì)學(xué)軟件SPSS13.0，針對兩組數(shù)據(jù)進(jìn)行概況分析，探討了靜脈血血清中CRP和RF水平與慢性牙周炎的關(guān)聯(lián)性。在此基礎(chǔ)上，通過多種回歸分析模型進(jìn)行研究，探討血清中C-反應(yīng)蛋白與類風(fēng)濕性因子的關(guān)系。結(jié)合慢性牙周炎患者的特點(diǎn)，分析了慢性牙周炎與類風(fēng)濕性關(guān)節(jié)炎的關(guān)系。 結(jié)果1健康組及慢性牙周炎組血清中C-反應(yīng)蛋白及類風(fēng)濕因子均呈現(xiàn)較強(qiáng)的相關(guān)性，表明二者之間存在某種關(guān)聯(lián)；2通過多種回歸分析，牙周炎組中C-反應(yīng)蛋白及類風(fēng)濕因子的回歸結(jié)果不明確，而健康組血清中二者呈現(xiàn)線性關(guān)聯(lián)性，即C-反應(yīng)蛋白水平隨著類風(fēng)濕因子水平的增高而增高；3慢性牙周炎組的C-反應(yīng)蛋白水平及類風(fēng)濕因子平均水平均高于健康組，推斷結(jié)果表明，慢性牙周炎疾病增高了血清中C-反應(yīng)蛋白的水平，也影響了患者的類風(fēng)濕因子水平。慢性牙周炎與類風(fēng)濕性關(guān)節(jié)炎可能存在關(guān)聯(lián)；4由于牙周炎組血清中C-反應(yīng)蛋白受身體多種因素的影響，且各因素對其的影響程度尚不明確，導(dǎo)致C-反應(yīng)蛋白及類風(fēng)濕因子的關(guān)聯(lián)性不明朗，其內(nèi)在關(guān)系，還有待于進(jìn)一步的深入研究；5慢性牙周炎患者經(jīng)過系統(tǒng)牙周治療3個月后，其C-反應(yīng)蛋白平均水平由4.2下降到2.55，類風(fēng)濕因子水平由9.23下降到8.79。 結(jié)論本研究根據(jù)臨床血清檢測的結(jié)果進(jìn)行分析，通過C-反應(yīng)蛋白與類風(fēng)濕因子的相關(guān)性研究，揭示了牙周炎與類風(fēng)濕性關(guān)節(jié)炎具有臨床上的關(guān)聯(lián)性。
[Abstract]:Background chronic periodontitis chronic periodontis (CPD) is the leading cause of tooth loss in Chinese adults. Rheumatoid arthritis (RA) is an autoimmune disease whose pathogenesis has not been fully elucidated. Studies have shown that periodontitis and rheumatoid arthritis have many similarities in immunopathology, because periodontitis and rheumatoid arthritis are so similar, some scholars even think they are the same disease. The focus of research in recent years is the clinical correlation between periodontitis and rheumatoid arthritis. Objective to compare the levels of serum C-reactive protein (CRP) and rheumatoid factoroid (RF) in patients with chronic periodontitis and healthy subjects. To study the level of rheumatoid factor and C-reactive protein in patients with chronic periodontitis and healthy people, and further analyze the association between chronic periodontitis and rheumatoid arthritis. The relationship between chronic periodontitis and rheumatoid arthritis is studied. Methods on the basis of obtaining the informed consent and signing the consent form, the serum related factors were detected. Patients with chronic periodontitis and healthy people were selected and their venous blood was collected to detect serum CRP and RF. According to the data of serum CRP and RF levels: chronic periodontitis group (41 groups) healthy control group (54 groups) chronic periodontitis periodontal system treatment 3 months after 11 groups, the study was carried out. The relationship between serum CRP and RF levels in venous blood and chronic periodontitis was studied by SPSS 13.0 software. On this basis, the relationship between serum C-reactive protein and rheumatoid factor was studied by multiple regression models. The relationship between chronic periodontitis and rheumatoid arthritis was analyzed. Results (1) there was a strong correlation between serum C-reactive protein and rheumatoid factor in healthy group and chronic periodontitis group, indicating that there was a certain correlation between them by multiple regression analysis. The regression results of C- reactive protein and rheumatoid factor in periodontitis group were not clear, but there was a linear correlation between them in serum of healthy group. That is to say, the level of C-reactive protein and the average level of rheumatoid factor in patients with chronic periodontitis increased with the increase of rheumatoid factor level. Chronic periodontitis increased the level of serum C-reactive protein and affected the level of rheumatoid factor. Chronic periodontitis may be associated with rheumatoid arthritis because the serum C-reactive protein in periodontitis group is affected by many factors of the body, and the influence of each factor on it is not clear. The relationship between C- reactive protein and rheumatoid factor is uncertain, and the relationship between C- reactive protein and rheumatoid factor remains to be further studied after 3 months of systematic periodontal treatment in patients with chronic periodontitis. The average level of C-reactive protein decreased from 4.2 to 2.55, and the level of rheumatoid factor decreased from 9.23 to 8.79. Conclusion this study is based on the analysis of clinical serum test results, through the study of the correlation between C-reactive protein and rheumatoid factor, it is revealed that periodontitis and rheumatoid arthritis have a clinical relevance.
