程序性死亡分子1及其配體在口腔鱗狀細胞癌患者外周血中的表達及臨床意義

【摘要】:目的檢測口腔鱗狀細胞癌患者外周血中程序性死亡分子1(PD-1)及其配體(PD-L1)的表達水平,探討其生物學(xué)及臨床意義。方法選取82例口腔鱗狀細胞癌患者(口腔鱗癌組)和25例健康對照者(對照組)為研究對象,收集研究對象的外周血,采用流式細胞術(shù)檢測外周血T淋巴細胞表面PD-1、PD-L1的表達及T淋巴細胞亞群計數(shù),采用酶聯(lián)免疫吸附方法檢測血清中可溶性PD-1(s PD-1)和可溶性PD-L1(s PD-L1)的表達水平。采用SPSS 17.0軟件對數(shù)據(jù)進行檢驗和相關(guān)性分析。結(jié)果口腔鱗癌組外周血CD8+T淋巴細胞百分數(shù)明顯高于對照組(P0.05),而CD3+、CD4+T淋巴細胞百分數(shù)及CD4+/CD8+T亞群百分數(shù)比值均低于對照組(P0.05)?谇击[癌組外周血CD4+、CD8+T淋巴細胞表面PD-1、PD-L1的陽性表達率均高于對照組(P0.01)?谇击[癌組血清s PD-L1水平高于對照組(P0.05),而s PD-1水平二者差異無統(tǒng)計學(xué)意義(P0.05)。s PD-L1的表達與臨床分期、腫瘤分化程度及淋巴結(jié)轉(zhuǎn)移狀態(tài)相關(guān)(P0.05),與性別、年齡、腫瘤部位及大小無關(guān)。結(jié)論口腔鱗狀細胞癌患者外周血T淋巴細胞亞群存在不同程度的免疫功能抑制,CD4+和CD8+T淋巴細胞表面PD-1及PD-L1表達顯著升高。異常升高的s PD-L1可能與口腔鱗狀細胞癌的發(fā)生發(fā)展有關(guān)。
[Abstract]:Objective to detect the expression of programmed death molecule (PD-1) and its ligand PD-L1 in peripheral blood of patients with oral squamous cell carcinoma (OSCC) and to explore its biological and clinical significance. Methods 82 patients with oral squamous cell carcinoma (OSCC group) and 25 healthy controls (control group) were selected to collect peripheral blood. The expression of PD-1 and PD-L1 on peripheral T lymphocytes and T lymphocyte subsets were detected by flow cytometry. The expression levels of soluble PD-1(s PD-1 and soluble PD-L1(s PD-L1) in serum were detected by enzyme-linked immunosorbent assay (Elisa). SPSS 17.0 software was used to test and analyze the data. Results the percentage of CD8 T lymphocytes in the patients with oral squamous cell carcinoma was significantly higher than that in the control group (P 0.05), while the percentage of CD3 T lymphocytes and the ratio of CD4 / CD8 T subsets were lower than those of the control group. The positive expression rate of PD-1 and PD-L1 on peripheral CD4 and CD8 T lymphocytes in oral squamous cell carcinoma group was higher than that in control group (P 0.01). The level of serum s PD-L1 in oral squamous cell carcinoma group was higher than that in control group (P 0.05), but there was no significant difference between the expression of s PD-1 and clinical stage, tumor differentiation and lymph node metastasis. There was no correlation between the expression of s PD-L1 and sex, age, location and size of tumor. Conclusion T lymphocyte subsets in peripheral blood of patients with oral squamous cell carcinoma have different immunosuppressive effects on the expression of PD-1 and PD-L1 on the surface of CD4 and CD8 T lymphocytes. The abnormal increase of s PD-L1 may be related to the occurrence and development of oral squamous cell carcinoma.
