【摘要】：目的:對青島地區(qū)1536例口腔頜面部創(chuàng)傷患者進(jìn)行回顧性研究,分析總結(jié)其臨床流行病學(xué)特點(diǎn),為提高本地區(qū)口腔頜面部創(chuàng)傷防治效果和救治水平提供參考依據(jù)。材料與方法:選取青島地區(qū)比較有代表性的兩家軍地三級甲等醫(yī)院的病案資料進(jìn)行回顧分析研究,包括:2002年1月1日~2015年12月31日期間,青島大學(xué)附屬醫(yī)院和中國人民解放軍解放軍第401醫(yī)院口腔頜面外科收治的口腔頜面部創(chuàng)傷病例共計(jì)1536例(810+726),將數(shù)據(jù)匯總進(jìn)行統(tǒng)計(jì)學(xué)分析,了解其臨床特點(diǎn),如發(fā)病原因、受傷部位、損傷程度等,進(jìn)行系統(tǒng)、全面的分析研究。結(jié)果:口腔頜面部創(chuàng)傷患者人數(shù)占住院總?cè)藬?shù)17633例的8.71%,隨著住院總?cè)藬?shù)逐年增加,創(chuàng)傷患者數(shù)量相對保持穩(wěn)定;男女比為3.95:1,平均年齡30.79歲,發(fā)病率最高的是21~30歲年齡段,共468例(30.73%)。發(fā)病原因主要為交通事故傷860例(55.99%);有1188例發(fā)生頜面部骨折,占77.34%,部位以下頜骨骨折最為常見(720例,46.88%),下頜骨骨折最容易出現(xiàn)的是頦部骨折333例(46.25%),其次為髁狀突骨折252例(35.00%),全身合并傷中最多的為顱腦損傷282例(19.01%)。結(jié)論:口腔頜面部創(chuàng)傷發(fā)病率與年齡和性別有關(guān),主要好發(fā)于青壯年,男性發(fā)病率高于女性。創(chuàng)傷原因主要為交通事故傷。骨折為常見創(chuàng)傷類型,部位以下頜骨最為多見。顱腦損傷是口腔頜面部創(chuàng)傷最常見的全身合并傷。治療應(yīng)首先重視全身狀況,優(yōu)先處理顱腦及其他重要臟器的合并傷,頜骨骨折的治療以開放復(fù)位堅(jiān)強(qiáng)內(nèi)固定為主。
[Abstract]:Objective: to study retrospectively 1536 patients with oral and maxillofacial trauma in Qingdao, and to analyze and summarize their clinical epidemiologic characteristics so as to provide a reference for improving the prevention and treatment of oral and maxillofacial trauma in Qingdao. Materials and methods: the medical records of two representative military and civilian Grade 3A hospitals in Qingdao were retrospectively analyzed, including: from January 1, 2002 to December 31, 2015, from January 1, 2002 to December 31, 2015. A total of 1536 cases of oral and maxillofacial trauma were admitted to the affiliated Hospital of Qingdao University and the 401st Hospital of the Chinese people's Liberation Army in oral and maxillofacial surgery. The data were collected and analyzed statistically to find out the clinical characteristics, such as the causes of the disease. Injury location, injury degree, etc., systematic, comprehensive analysis and research. Results: the number of trauma patients in oral and maxillofacial region accounted for 8.71% of the total number of 17633 hospitalized patients. With the increase of the total number of patients in hospital, the number of trauma patients remained relatively stable, the ratio of male to female was 3.95: 1, the average age was 30.79 years old, and the highest incidence rate was in the age group of 21 to 30 years old. There were 468 cases with 30. 73C. The main causes were 860 cases of traffic accident injury and 1188 cases of maxillofacial fracture. The most common site of mandibular fracture was fracture of mandible in 720 cases (46.88). The most common fracture of mandible was mental fracture (333 cases), followed by condylar fracture (252 cases), craniocerebral injury (282 cases) and craniocerebral injury (19.01). Conclusion: the incidence of oral and maxillofacial trauma is related to age and sex. The main cause of injury is traffic accident. Fracture is a common type of trauma, the most common site is the mandible. Craniocerebral injury is the most common combined injury in oral and maxillofacial region. The treatment should pay attention to the condition of the whole body first and give priority to the treatment of the combined injury of craniocerebral and other important organs. The treatment of jaw fracture is mainly open reduction and firm internal fixation.
