【摘要】：目的：觀察環(huán)狀植骨術同期牙種植短期臨床效果并總結其相關手術操作要點,為臨床操作提供指導。 方法：7例牙槽骨嚴重吸收的患者接受環(huán)狀植骨術同期植入種植體,術后五個月行上部修復。分別于術前和術后3-5個月時拍口腔CT,測量牙槽骨增加高度和唇舌側骨吸收量。于上部冠修復后3個月時,對其牙齦附著水平、牙齦乳頭分級和牙周探診深度進行測量分析。比較唇舌側骨吸收量和種植牙位與鄰牙牙周探診深度的差異。統計骨環(huán)的成活率及種植體的存留率,同時觀察術后并發(fā)癥并了解患者的滿意度。 結果：術后3-5個月復查,骨環(huán)成活率為89%,種植體的存留率為100%。骨高度平均增量為6.94mm,骨吸收量平均為2.64mm,種植體頸部平均骨厚度為7.52mm,平均附著齦水平近中為1.94mm,遠中為1.81mm,唇舌側骨吸收量比較及種植體與鄰牙齦溝探診深度的比較均無統計學差異。術后無明顯的并發(fā)癥,修復效果好,患者滿意度高。 結論：環(huán)狀植骨術同期植入種植體,手術操作相對容易,縮短了治療療程,近期效果可靠,遠期效果有待進一步觀察。
[Abstract]:Objective: to observe the short-term clinical effect of dental implants in annular bone grafting and summarize the relevant operative points to provide guidance for clinical operation. Methods Seven patients with severe alveolar bone resorption underwent annular bone grafting and implants were implanted at the same time. The upper part was repaired 5 months after operation. Oral CTS were taken 3-5 months before and 3-5 months after operation to measure the height of alveolar bone and the bone resorption of lip and tongue. Gingival attachment level, gingival papilla grading and periodontal probing depth were measured and analyzed 3 months after upper crown restoration. To compare the amount of bone resorption and the depth of periodontal diagnosis between the implant and adjacent teeth. The survival rate of bone ring and the survival rate of implant were counted, and the postoperative complications and patients' satisfaction were observed. Results: the survival rate of bone ring was 89 and the survival rate of implant was 100. The average increment of bone height was 6.94 mm, the average bone resorption was 2.64 mm, the average bone thickness of implant neck was 7.52 mm, the average attachment gingival level was 1.94 mm in the middle and 1.81 mm in the distal. There was no significant difference in the amount of bone resorption in the lip and tongue and the depth of probing between the implant and the adjacent gingival sulcus. There were no obvious postoperative complications, good repair effect and high patient satisfaction. Conclusion: the implantation of implants with annular bone graft is relatively easy to operate and shortens the course of treatment. The short term effect is reliable and the long-term effect needs further observation.
