【摘要】：目的總結(jié)天然珊瑚充填治療小頦畸形的遠期療效,探討天然珊瑚與新生骨間的體積關(guān)系及新生骨的穩(wěn)定性。方法回顧分析1998年10月-2004年9月,采用頦部水平截骨結(jié)合天然珊瑚塊充填治療的12例小頦畸形患者臨床資料。男7例,女5例;年齡15~28歲,平均18.5歲。術(shù)前、術(shù)后即刻及末次隨訪時對患者面容及頭顱X線片進行測量,分析下唇點至頦下點垂直距離、頦前點至鼻根點和下牙槽點連線的距離、下牙槽點至頦下點的垂直距離、頦部截骨間隙垂直距離,并計算各指標復發(fā)率。結(jié)果患者口內(nèi)切口均Ⅰ期愈合,無手術(shù)并發(fā)癥發(fā)生�；颊呔@隨訪,隨訪時間8~12年,平均9.2年。頭顱X線片復查示,8年后珊瑚塊蜂窩狀結(jié)構(gòu)均完全消失,被骨組織替代。8年后取出鈦板時見頦部新生骨具有良好的強度和頦肌骨膜附著。末次隨訪時下唇點至頦下點垂直距離、下牙槽點至頦下點的垂直距離、頦部截骨間隙垂直距離均較術(shù)后即刻縮短,平均復發(fā)率分別為6.1%、22.9%、31.7%;而頦前點至鼻根點和下牙槽點連線的距離無明顯變化。9例患者對手術(shù)療效滿意;3例再次調(diào)整頦部形態(tài)邊緣。結(jié)論天然珊瑚充填治療小頦畸形后,盡管存在部分體積丟失,但能獲得穩(wěn)定的新骨和滿意的遠期療效。
[Abstract]:Objective to summarize the long-term effect of natural coral filling in the treatment of mental malformation and to explore the volume relationship between natural coral and new bone and the stability of new bone. Methods from October 1998 to September 2004, the clinical data of 12 patients with mental malformation were retrospectively analyzed. There were 7 males and 5 females, aged 15 to 28 years (mean 18.5 years). Before operation, immediately after operation and at the last follow-up, the facial features and head X-ray films were measured to analyze the vertical distance between the lower lip point and the submental point, the distance between the mental anterior point and the nasal root point and the inferior alveolar point, and the vertical distance between the inferior alveolar point and the submental point. The vertical distance of mental osteotomy space was calculated and the recurrence rate of each index was calculated. Results all the incisions were healed in the first stage, and no complications occurred. All patients were followed up for 8 ~ 12 years (mean 9.2 years). After 8 years, the honeycomb structure of corals disappeared completely and was replaced by bone tissue. After 8 years removal of titanium plate, it was found that the new mental bone had a good strength and the geniofilm periosteum attached. At the last follow-up, the vertical distance between the lower lip point and the submental point, the vertical distance between the inferior alveolar point and the submental point, and the vertical distance between the mental osteotomy space and the inferior alveolar point were shorter than those immediately after operation. The average recurrence rate was 6.1and 22.9g, respectively, while the distance from the anterior mental point to the nasal root point and the inferior alveolar point did not change significantly in 9 patients. Conclusion Natural coral filling can obtain stable new bone and satisfactory long-term effect despite partial volume loss in the treatment of mental malformation.
