不同磨耗程度的下頜第一磨牙咬合接觸區(qū)域的口內(nèi)掃描精度對比

【摘要】:目的:對比研究Cerec系統(tǒng)對輕、中度磨耗下頜第一磨牙口內(nèi)咬合接觸的掃描精度差異,并探討輕、中度磨耗下頜第一磨牙咬合接觸區(qū)域的分布差異,為Cerec椅旁系統(tǒng)制作的修復(fù)體咬合接觸精度的提高及臨床調(diào)牙合提供理論依據(jù)。方法:選取2015年5月至2015年12月來我院修復(fù)科就診或進(jìn)行口腔咨詢的患者,知情同意后按照Smith and Knight磨耗指數(shù)標(biāo)準(zhǔn),對受檢者的下頜第一磨牙咬合面進(jìn)行磨耗程度分類;依據(jù)納入標(biāo)準(zhǔn)選擇其中42名受檢者的84顆牙齒作為研究對象,按照磨耗程度分組,I組為輕度磨耗組,II組為中度磨耗組,使用Cerec Omnicam系統(tǒng)對受檢者進(jìn)行口內(nèi)光學(xué)印模,通過Inlab軟件獲取下頜第一磨牙的虛擬咬合接觸分布圖像;使用40μm厚度寶詩咬合紙?jiān)谑軝z者口內(nèi)進(jìn)行牙尖交錯(cuò)位緊咬合,Canon EOS 600D數(shù)碼單反相機(jī)拍攝下頜第一磨牙口內(nèi)咬合接觸印跡圖像,利用Adobe Photoshop CS6圖像軟件分析輕、中度磨耗下頜第一磨牙咬合接觸區(qū)域分布差異,并計(jì)算虛擬咬合與口內(nèi)咬合接觸區(qū)域的重疊率,評估Cerec系統(tǒng)對兩種磨耗程度的下頜第一磨牙口內(nèi)咬合接觸的掃描精度。結(jié)果:1、輕度磨耗組的牙尖交錯(cuò)位咬合接觸分布區(qū)域主要分布于遠(yuǎn)中尖頂及近遠(yuǎn)中舌斜面、遠(yuǎn)中頰尖頂及近遠(yuǎn)中舌斜面、近中頰尖頂及近遠(yuǎn)中舌斜面、遠(yuǎn)中舌尖遠(yuǎn)中頰斜面和牙合面遠(yuǎn)中邊緣嵴,中度磨耗組咬合接觸區(qū)域則分布于下頜第一磨牙的遠(yuǎn)中尖頂及近遠(yuǎn)中舌斜面、遠(yuǎn)中頰尖頂及近遠(yuǎn)中舌斜面、近中頰尖頂及近遠(yuǎn)中舌斜面、遠(yuǎn)中舌尖遠(yuǎn)中頰斜面、中央窩和牙合面近遠(yuǎn)中邊緣嵴,兩組間的咬合接觸分布區(qū)域差異具有統(tǒng)計(jì)學(xué)意義(p0.05);2、輕度磨耗組咬合接觸區(qū)域重疊率為86.68±1.41,中度磨耗組的咬合接觸區(qū)域重疊率為87.60±2.06,中度磨耗組咬合接觸區(qū)域的重疊率要高于輕度磨耗組,差異具有統(tǒng)計(jì)學(xué)意義(p0.05)。結(jié)論:1、輕度磨耗組下頜第一磨牙牙尖交錯(cuò)位咬合接觸區(qū)域分布較為集中,咬合接觸區(qū)域多分布于功能尖頂及其牙尖斜面和遠(yuǎn)中邊緣嵴,隨著磨耗程度的加深,咬合接觸分布范圍擴(kuò)大,相比輕度磨耗組,中度磨耗組中的下頜第一磨牙咬合接觸增加了中央窩和近中邊緣嵴兩個(gè)咬合接觸區(qū)域;2、Cerec椅旁系統(tǒng)對中度磨耗組的下頜第一磨牙牙尖交錯(cuò)位咬合接觸的掃描精度高于輕度磨耗組。
[Abstract]:Objective: to compare the scanning accuracy of Cerec system between mild and moderate wear mandibular first molar intraoral occlusal contact, and to explore the distribution of the first mandibular first molar occlusion contact area between mild and moderate wear. It provides a theoretical basis for the improvement of occlusal contact accuracy and clinical occlusal adjustment of prosthesis made by Cerec chair system. Methods: from May 2015 to December 2015, the patients who were admitted to our hospital from May 2015 to December 2015 for oral consultation were selected. According to the Smith and Knight wear index standard, the first mandibular molar occlusal surface was classified according to the degree of wear. According to the inclusion criteria, 84 teeth of 42 subjects were selected as study objects. According to the degree of wear, group I was divided into mild wear group and moderate wear group. Cerec Omnicam system was used to carry out intraoral optical impression on the subjects. The virtual occlusal contact distribution images of mandibular first molar were obtained by Inlab software. A 40 渭 m thick Baoshi bite paper was used to capture the first mandibular first molar intraoral occlusal contact imprinting image with interlocking occlusal canon EOS 600D digital SLR camera, and Adobe Photoshop CS6 image software was used to analyze the light. The distribution of the first mandibular first molar occlusal contact area was different, and the overlap rate between the virtual occlusion and the intraoral occlusion was calculated to evaluate the scanning accuracy of the Cerec system for the occlusion of the first mandibular first molar. Results in the mild attrition group, the occlusal contact area in the interlocking position of the cusp was mainly distributed on the distal tip and the proximal middle tongue, the distal buccal tip and the proximal middle tongue, the proximal middle buccal tip and the proximal distal lingual oblique. The distal tip of the tongue and the distal marginal ridge of the occlusal surface were distributed in the distal tip of the first mandibular molars and the proximal distal oblique surface of the tongue, the distal buccal tip and the proximal distal oblique surface of the tongue in the moderate attrition group, while in the moderate attrition group, the occlusal contact area was located on the distal tip of the first mandibular molars and the proximal distal lingual oblique plane. The apex of the mesiobuccal canine and the oblique surface of the proximal middle tongue, the distal tip of the tongue, the central fossa and the occlusal surface near the distal marginal ridge, The difference of occlusal contact area between the two groups was statistically significant (p 0.05). The overlap rate of occlusal contact area was 86.68 鹵1.41 in mild wear group, 87.60 鹵2.06 in moderate wear group, and 87.60 鹵2.06 in moderate wear group. Higher than the mild wear group, The difference was statistically significant (P 0.05). Conclusion the occlusal contact area of the mandibular first molar cusp in the mild wear group was more concentrated, and the occlusal contact area was mostly distributed in the functional cusp, its cusp inclined plane and distal marginal ridge, and with the increase of wear degree, the occlusal contact area of the mandibular first molar was more concentrated than that of the control group. The distribution of occlusal contact was wider than that of mild wear group. The occlusal contact of the mandibular first molar in the moderate wear group increased the scanning accuracy of the occlusal contact between the central fossa and the proximal marginal ridge of the mandibular first molar in the interlocking position of the mandibular first molar in the moderate wear group was higher than that in the mild wear group.
