趨化因子CCL7在口腔白斑及口腔鱗癌組織中的表達及意義

【摘要】:WHO將口腔白斑歸入癌前病變的范疇。而口腔鱗狀細(xì)胞癌是我國口腔頜面部最為常見的惡性腫瘤。鱗癌局部侵襲力強,常向區(qū)域淋巴結(jié)轉(zhuǎn)移,晚期容易發(fā)生遠(yuǎn)處轉(zhuǎn)移。目前它的主要有效的治療方法仍以手術(shù)切除為主,,即使輔助放射治療、化學(xué)藥物治療等綜合序列治療,它的預(yù)后效果往往不佳,生存率低。因此,為了口腔鱗癌的早期預(yù)防、發(fā)現(xiàn)及診斷,提高口腔鱗癌患者的生存質(zhì)量及存活率,深入探討口腔鱗癌的發(fā)生發(fā)展機制并針對性地采取個體化綜合序列治療顯得尤為重要。 大量的研究證明,各種免疫因子、趨化因子及其受體在腫瘤的發(fā)生發(fā)展過程中起著及其重要的作用。CCL7是趨化因子CC家族中的一員,其在募集單核細(xì)胞、腫瘤轉(zhuǎn)移等生物學(xué)效應(yīng)中起著重要作用,有研究表明該因子在宮頸癌、胰腺癌、胃癌、結(jié)直腸癌等惡性腫瘤中與腫瘤的發(fā)展轉(zhuǎn)移有關(guān)。CCL7在口腔鱗狀細(xì)胞癌中表達情況的研究較少,本研究采用免疫組化技術(shù),對口腔白斑、口腔鱗癌中CCL7的表達進行初步研究,結(jié)合臨床資料進行分析,來探討CCL7在它們當(dāng)中的表達情況。 目的 檢測CCL7在口腔白斑和口腔鱗癌患者腫瘤組織中的表達情況,分析與臨床病理參數(shù)的關(guān)系,初步探討CCL7在口腔癌前病變惡變及口腔鱗癌發(fā)生發(fā)展中的作用。 材料和方法 1.樣本資料:18例口腔白斑組織,其中12例單純增生型白斑,6例中或中重度增生型白斑;35例口腔鱗癌腫瘤組織,12例高分化型鱗癌,13例中或中低分化型鱗癌,10例中或中低分化且伴有頸部淋巴結(jié)轉(zhuǎn)移型鱗癌;11例口腔正常黏膜組織; 2.實驗方法:采用免疫組織化學(xué)染色(SABC)的方法進行檢測CCL7的表達; 3.統(tǒng)計方法:所得實驗數(shù)據(jù)用SPSS19.0軟件進行分析,各組間免疫組化的陽性率及臨床病理分級等計數(shù)資料的統(tǒng)計分析比較采用X2檢驗,平均光密度值采用t檢驗,p0.05為差異具有統(tǒng)計學(xué)意義。 結(jié)果 口腔鱗癌患者組織中CCL7的表達高于口腔白斑組織及正?谇火つそM織,差異具有統(tǒng)計學(xué)意義(p0.05)?谇击[癌組織中臨床分期晚期伴有頸部淋巴結(jié)轉(zhuǎn)移的組織中CCL7的表達高于臨床早期無淋巴結(jié)轉(zhuǎn)移組織,p0.05,差異具有統(tǒng)計學(xué)意義。而中或中重度增生的白斑組織中CCL7的表達高于單純增生的白斑組織及正?谇火つそM織,p0.05,差異具有統(tǒng)計學(xué)意義。 結(jié)論 CCL7在中重度異常增生的白斑組織及口腔鱗癌患者腫瘤組織中的高表達,且與臨床分期及頸部淋巴結(jié)轉(zhuǎn)移相關(guān),提示CCL7可能在口腔癌前病變惡變及口腔鱗癌的發(fā)生發(fā)展過程中起著一定的作用。
[Abstract]:WHO classified oral leukoplakia as precancerous lesions.Oral squamous cell carcinoma (OSCC) is the most common malignant tumor in oral and maxillofacial region in China.Squamous cell carcinoma has strong local invasiveness and often metastases to regional lymph nodes, and is prone to distant metastasis in late stage.At present, its main effective treatment method is still surgical resection, even though the adjuvant radiotherapy, chemotherapeutic therapy and other comprehensive sequence therapy, its prognosis is often poor, the survival rate is low.Therefore, in order to prevent, detect and diagnose oral squamous cell carcinoma early, improve the quality of life and survival rate of oral squamous cell carcinoma patients, deeply explore the development mechanism of oral squamous cell carcinoma and take individualized comprehensive sequence therapy.A large number of studies have proved that all kinds of immune factors, chemokines and their receptors play an extremely important role in tumorigenesis and progression. CCL7 is a member of the CC family of chemokines, which is recruiting monocytes.Tumor metastasis and other biological effects play an important role, some studies have shown that the factor in cervical cancer, pancreatic cancer, gastric cancer,There are few studies on the expression of CCL7 in oral squamous cell carcinoma (OSCC) in colorectal cancer and other malignant tumors. The expression of CCL7 in oral leukoplakia and oral squamous cell carcinoma (OSCC) was studied by immunohistochemical technique.To explore the expression of CCL7 in them by analyzing the clinical data.PurposeTo detect the expression of CCL7 in oral leukoplakia and oral squamous cell carcinoma (OSCC), to analyze the relationship between CCL7 and clinicopathological parameters, and to explore the role of CCL7 in the development of oral squamous cell carcinoma (OSCC) and precancerous lesion.Materials and methods1.Sample data: 18 cases of oral leukoplakia,12 cases of simple proliferative leukoplakia 6 cases of moderate or severe proliferative leukoplakia 35 cases of oral squamous cell carcinoma tumor 12 cases of highly differentiated squamous cell carcinoma 13 cases of middle or low differentiated squamous cell carcinoma 10 cases of middle or low differentiation with cervical lymph node metastasis11 cases of normal oral mucosa;2.Methods: immunohistochemical staining was used to detect the expression of CCL7.3.Methods: the experimental data were analyzed by SPSS19.0 software. The positive rate of immunohistochemistry and the clinical pathological grade were compared by X2 test.The average optical density was significantly different by t test (p 0.05).ResultThe expression of CCL7 in oral squamous cell carcinoma was significantly higher than that in oral leukoplakia and normal oral mucosa.The expression of CCL7 in oral squamous cell carcinoma (OSCC) with advanced stage and cervical lymph node metastasis was significantly higher than that in early stage without lymph node metastasis (p0.05).The expression of CCL7 in moderate or severe hyperplastic leukoplakia tissues was higher than that in simple proliferative leukoplakia tissues and normal oral mucosa tissues (P 0.05).ConclusionThe high expression of CCL7 in leukoplakia and oral squamous cell carcinoma patients with moderate or severe dysplasia was associated with clinical stage and cervical lymph node metastasis.The results suggest that CCL7 may play an important role in the development of oral squamous cell carcinoma and precancerous lesion.
