開放式與閉合式牽引上頜阻生尖牙的錐形束CT研究

【摘要】:目的:采用開放式與閉合式牽引矯治上頜阻生尖牙(impactedmaxillary canine, IMC),應(yīng)用錐形束CT(cone beam computedtomography,CBCT)測(cè)量?jī)煞N方法矯治前后牙根、邊緣牙槽骨以及根尖部牙槽骨的變化量,為臨床上選擇治療上頜阻生尖牙的開窗方法提供參考。 方法:選取在重慶醫(yī)科大學(xué)附屬口腔醫(yī)院正畸科接受CBCT掃描并完成外科正畸聯(lián)合治療的29例上頜埋伏阻生尖牙患者,分成2組,外科開窗,一組18例患者共18顆上頜阻生尖牙采用開放式開窗為開放組,另一組11例患者共14顆上頜阻生尖牙采用閉合式開窗作為閉合組,所有患者均采用直絲弓矯治技術(shù)進(jìn)行正畸牽引治療,對(duì)牽引前后上頜阻生尖牙以及對(duì)照側(cè)尖牙的牙根,上頜阻生尖牙邊緣牙槽骨以及根尖部牙槽骨的變化進(jìn)行CBCT分析,采用SPSS16.0軟件對(duì)測(cè)量數(shù)據(jù)進(jìn)行配對(duì)T檢驗(yàn)。 結(jié)果: 1、開放式牽引組和閉合式牽引組的上頜阻生尖牙以及對(duì)照側(cè)尖牙矯治前后均有輕度的牙根吸收,但是差異沒有統(tǒng)計(jì)學(xué)意義(P0.05) 2、開放式牽引組唇、腭側(cè)牙槽骨高度與矯治前比較,差異有顯著性(p0.05),閉合式牽引組唇、腭側(cè)牙槽骨高度與矯治前比較,差異無顯著性(p0.05),開放式牽引組根尖部唇側(cè)牙槽骨厚度與矯治前比較,差異有顯著性(p0.05),開放式牽引組根尖部腭側(cè)牙槽骨厚度與矯治前比較,差異無顯著性(p0.05),,閉合式牽引組根尖部唇、腭側(cè)牙槽骨厚度與矯治前比較,差異無顯著性(p0.05)。 結(jié)論: 1、上頜阻生尖牙采用開放式牽引和閉合式牽引后牙根均有不明顯的吸收。 2、上頜阻生尖牙采用閉合式牽引后邊緣牙槽骨以及根尖部牙槽骨的吸收量小于開放式牽引組。
[Abstract]:Objective: to measure the changes of anterior and posterior root, marginal alveolar bone and apical alveolar bone of maxillary canine (IMC) in maxillary canine of maxillary canine by open and closed traction, and to measure the changes of root, marginal alveolar bone and apical alveolar bone of maxillary canine by CT(cone beam computed tomographic technique.To provide a reference for clinical choice of maxillary impacted canine fenestration.Methods: a total of 29 patients with impacted maxillary impacted teeth underwent CBCT scanning and surgical orthodontic treatment in Department of Orthodontics, affiliated Stomatology Hospital of Chongqing Medical University, and were divided into 2 groups.In one group, 18 maxillary impacted canines were treated with open fenestration, the other 11 patients with 14 maxillary impacted canines were treated with closed fenestration.All patients were treated by orthodontic traction with straight wire appliance. CBCT analysis was performed on the root of maxillary impacted canine and control canine, the alveolar bone at the edge of maxillary impacted canine and the alveolar bone in the apical region of maxillary impacted canine before and after traction.SPSS16.0 software was used to test the measured data by paired T test.Results:1. The maxillary impacted canines and control canines in open traction group and closed traction group had slight root resorption before and after orthodontic treatment, but the difference was not statistically significant (P 0.05).2. The height of labial and palatine alveolar bone in open traction group was significantly higher than that before orthodontic treatment (P 0.05), and the height of palatine alveolar bone in closed traction group was higher than that before orthodontic treatment.The thickness of labial alveolar bone in open traction group was significantly higher than that before orthodontic treatment, and the thickness of alveolar bone in apical palatine of open traction group was higher than that before orthodontic treatment.There was no significant difference between the two groups (P 0.05). There was no significant difference in the thickness of alveolar bone between the apical lip and palatal alveolar bone in the closed traction group (P 0.05).Conclusion:1. The root absorption of maxillary impacted canine was not obvious after open traction and closed traction.2. The absorption of the alveolar bone and the alveolar bone in the apical region of the maxillary impacted canine was less than that in the open traction group.
