【摘要】：目的:探討1例雙側(cè)下頜第二磨牙阻生伴牙旁囊腫的病因及治療,為其診治提供參考。方法:患者囊腫累及左下頜第一磨牙,術(shù)前行根管治療,局麻下拔除左下頜第二磨牙,刮匙刮除病變區(qū)殘余牙囊、肉芽組織及囊腫壁,球鉆打磨創(chuàng)腔及尖銳骨創(chuàng)緣,并磨除左下頜第一磨牙遠中部分根尖,術(shù)中取組織行組織病理學檢查。結(jié)果:X線片可見雙側(cè)下頜第二磨牙近中阻生,左下頜第二磨牙冠周有一邊界清楚類圓形密度減低區(qū),鄰牙牙根吸收。術(shù)后病理回報示牙旁囊腫。結(jié)論:牙齒阻生伴牙旁囊腫可發(fā)生于除第三磨牙以外的其余牙位,其診斷需結(jié)合臨床表現(xiàn)、病理和影像學檢查,多學科會診有利于其診斷和治療。
[Abstract]:Objective: to investigate the etiology and treatment of bilateral mandibular second molar impaction associated with parodontal cyst, and to provide reference for its diagnosis and treatment. Methods: the first molar of left mandible was involved in the cyst, root canal treatment was performed before operation, and the second molar of left mandible was extracted under local anesthesia. The residual bursa, granulation tissue and cyst wall in the lesion area were removed by curettage, and the wound cavity and sharp bone were ground with a ball drill, and the distal tip of the left first molar was removed. Results the bilateral mandibular second molars had proximal and middle impingement and there was a clear circular reduced density region around the crown of the left mandibular second molar. The pathological results of the adjacent tooth root resorption show the parodontoid cyst. Conclusion: tooth impaction with parodontoid cyst may occur in the other teeth except the third molar, and its diagnosis should be combined with clinical manifestation, pathology and imaging examination. Multi-disciplinary consultation is beneficial to its diagnosis and treatment.
