錐形束CT重建評價髁突骨性增齡性變化

【摘要】:背景:錐形束CT能夠發(fā)現(xiàn)髁突結(jié)構(gòu)及形態(tài)的異常改變,包括骨皮質(zhì)增厚、骨密度異常等,這些改變與顳下頜關(guān)節(jié)骨關(guān)節(jié)病密切相關(guān)。如何甄別骨關(guān)節(jié)病的影像學(xué)改變是正確診斷和治療的前提。對于一般人群顳下頜關(guān)節(jié)各年齡段的髁突生理性改變以及性別差異,目前尚缺乏臨床資料研究。目的:采用錐形束CT觀察不同性別及年齡人群髁突骨性增齡性變化。方法:收集883例無顳下頜關(guān)節(jié)癥狀患者的錐形束CT資料,按年齡(7-8,9-10,11,12,13,14,15,16,17,18,19,20,21,22,23,24,25,26-27,28-29,30-35,36-40,41-45歲)分為22組,采用examvision軟件測量髁突表面各點骨密度,并進(jìn)行統(tǒng)計學(xué)分析。結(jié)果與結(jié)論:隨年齡增長,對象髁突最前點、頂點骨密度增高達(dá)高峰期,后進(jìn)入穩(wěn)定期,40歲后呈下降趨勢;髁突最后點骨密度增高,達(dá)到高峰后下降,30歲后保持穩(wěn)定。男女各年齡段髁突最前點、頂點骨密度均大于最后點。各年齡段女性髁突各點骨密度均大于男性。結(jié)果證實,髁突骨密度具有一定的增齡性變化規(guī)律。髁突功能面骨密度大于非功能面。男性髁突發(fā)育遲于女性。實驗為顳下頜關(guān)節(jié)骨性疾病的診斷提供了參考。
[Abstract]:Background: conical beam CT can detect abnormal condylar structure and morphology, including thickening of bone cortex, abnormal bone density, etc. These changes are closely related to temporomandibular osteoarthropathy. How to identify the imaging changes of osteoarthropathy is a prerequisite for correct diagnosis and treatment. Objective: to observe the condylar bone aging changes in different sex and age population by conical beam CT. Methods: 883 cases without temporomandibular joint symptom were collected. The patients were divided into 22 groups according to their ages. The bone mineral density on the surface of condylar process was measured by examvision software, and the data were analyzed statistically. Results and conclusion: with age, the anterior point of condylar process and the peak value of bone mineral density of the condylar process increased to the peak. At the end of the condylar process, the bone mineral density increased at the end of the condylar process, reached the peak and remained stable after the age of 30. The anterior point of the condylar process in every age group of men and women, The bone mineral density of the apex was higher than that of the last point. The bone mineral density of condyle of all ages was higher than that of the male. Condylar bone mineral density (BMD) of condyle is higher than that of nonfunctional surface. The condyle development of male is later than that of female. The experiment provides a reference for the diagnosis of temporomandibular joint osteopathy.
