【摘要】：目的分析舌癌口底區(qū)淋巴結(jié)轉(zhuǎn)移分布的特點(diǎn),探討舌癌貫通口底切除的合理術(shù)式。方法回顧性分析26例行舌-口底-頸聯(lián)合根治的舌癌病例,所有患者均行下頜骨舌側(cè)骨膜與舌骨之間患側(cè)口底組織貫通切除的手術(shù)方式,將口底區(qū)標(biāo)本分成舌淋巴結(jié)和舌動(dòng)脈起始部淋巴結(jié)兩組進(jìn)行病理分析,觀察術(shù)后口底瘺發(fā)生幾率和吞咽功能恢復(fù)情況,并對(duì)術(shù)后腫瘤復(fù)發(fā)轉(zhuǎn)移情況進(jìn)行隨訪分析。結(jié)果舌淋巴結(jié)出現(xiàn)率23.1%(7/26);舌動(dòng)脈起始部淋巴結(jié)出現(xiàn)率15.3%(4/26)。在出現(xiàn)舌淋巴結(jié)患者中轉(zhuǎn)移率為71.4%(5/7);在出現(xiàn)舌動(dòng)脈起始部淋巴結(jié)患者中轉(zhuǎn)移率為50.0%(2/4)。術(shù)后口底瘺的發(fā)生率為7.7%(2/26)。所有患者術(shù)后均恢復(fù)經(jīng)口進(jìn)食。隨訪期內(nèi)4例患者發(fā)生對(duì)側(cè)頸部或咽旁、咽后淋巴結(jié)轉(zhuǎn)移,全部26例患者未見原發(fā)灶、患側(cè)頜下、患側(cè)舌骨區(qū)、患側(cè)頸部復(fù)發(fā)。結(jié)論舌癌貫通口底切除術(shù)式符合淋巴引流規(guī)律,徹底清掃了口底淋巴引流通路及可能出現(xiàn)的舌淋巴結(jié)及舌動(dòng)脈起始端淋巴結(jié),經(jīng)臨床應(yīng)用,取得了較好的腫瘤局部及區(qū)域轉(zhuǎn)移控制率。
[Abstract]:Objective to analyze the characteristics of lymph node metastasis in the oral floor area of tongue cancer, and to explore the rational operation method for the resection of tongue cancer through the floor of mouth. Methods 26 cases of tongue cancer were retrospectively analyzed in 26 cases of tongue cancer treated by combined tongue-oral fundus neck radical resection. All the patients were treated with transfixion excision between the lingual periosteum and the hyoid bone. The specimens were divided into two groups: the tongue lymph nodes and the initial lymph nodes of the lingual artery for pathological analysis. The occurrence rate of postoperative fundus fistula and the recovery of swallowing function were observed. The recurrence and metastasis of tumor after operation were followed up. Results the incidence of tongue lymph node was 23.1- / 26g, the rate of tongue artery initial lymph node was 15.33- / 26g, the metastasis rate was 71.4% / 7% in the patients with tongue lymph node, and the rate was 71.4% in the initial part of the lingual artery, and in the initial part of the lingual artery, the rate of metastasis was 71.4% / 7%. The incidence of postoperative fundus fistula was 7. 7 / 26. All patients recovered from oral feeding. During the follow-up period, 4 patients developed contralateral neck or parapharyngeal fistula. Retropharyngeal lymph node metastasis was found in all of the 26 patients without primary foci, with recurrence in the affected side of the jaw, the affected hyoid region, and the neck of the affected side. Conclusion radical resection of the oral floor of tongue cancer is consistent with the law of lymphatic drainage. The lymphatic drainage pathway of the floor of the mouth and the possible tongue lymph nodes and the initial lymph nodes of the lingual artery were thoroughly dissected. The local and regional metastasis control rate of the tumor was obtained by clinical application.
