【摘要】：目的:檢測(cè)評(píng)價(jià)患者術(shù)后3個(gè)月、術(shù)后1年的吞咽功能變化,探討頦下島狀肌皮瓣在舌癌術(shù)后組織缺損重建中的臨床應(yīng)用。方法:收集2014年12月至2015年12月間在河北醫(yī)科大學(xué)第四醫(yī)院口腔科住院治療的14例舌癌根治術(shù)后采用頦下島狀肌皮瓣行舌缺損修復(fù)重建的病例資料,所有患者行舌癌根治術(shù)后即刻采用頦下島狀肌皮瓣轉(zhuǎn)移修復(fù)舌缺損。對(duì)術(shù)后3個(gè)月、術(shù)后1年的吞咽功能進(jìn)行評(píng)分,通過(guò)GUSS吞咽功能評(píng)估表對(duì)患者修復(fù)后的吞咽功能進(jìn)行評(píng)價(jià)。采用SPSS21.0軟件,對(duì)術(shù)后3個(gè)月和術(shù)后1年的吞咽功能指標(biāo)評(píng)分進(jìn)行t檢驗(yàn),P0.05為差異有統(tǒng)計(jì)學(xué)意義。結(jié)果:14例頦下島狀肌皮瓣轉(zhuǎn)移修復(fù)舌缺損術(shù)后,皮瓣壞死者0例,皮瓣成活率100%。術(shù)后3個(gè)月吞咽功能平均評(píng)分為14.31±0.57,GUSS評(píng)價(jià)示有些吞咽困難;術(shù)后1年吞咽功能平均評(píng)分為18.08±0.52,GUSS評(píng)價(jià)示輕微吞咽困難。經(jīng)配對(duì)t檢驗(yàn),術(shù)后3個(gè)月的吞咽功能評(píng)分與術(shù)后1年比較,差異有統(tǒng)計(jì)學(xué)意義(t=4.864,P0.001)。結(jié)論:應(yīng)用頦下島狀肌皮瓣修復(fù)重建舌缺損能基本恢復(fù)舌癌術(shù)后患者的吞咽功能,在一定程度上滿(mǎn)足舌缺損修復(fù)重建的臨床需要,其應(yīng)用價(jià)值仍可進(jìn)一步拓展。
[Abstract]:Objective: to evaluate the changes of swallowing function in 3 months and 1 year after operation. To investigate the clinical application of submental island myocutaneous flap in reconstruction of tissue defect after tongue cancer operation methods: from December 2014 to December 2015, 14 cases of tongue cancer were collected after radical mastectomy in Department of Stomatology, 4th Hospital of Hebei Medical University. Cases of tongue reconstruction with submental island myocutaneous flap, Tongue defect was repaired by submental island myocutaneous flap transfer immediately after radical resection of tongue cancer. The swallowing function of the patients was evaluated by GUSS swallowing function evaluation table. SPSS21.0 software was used to evaluate the swallowing function of the patients. There was a significant difference in the score of swallowing function between 3 months after operation and 1 year after operation. Results 14 cases of submental island myocutaneous flap were transferred to repair tongue defect, and there were 0 cases of flap necrosis. The average score of swallowing function was 14.31 鹵0.57g USS at 3 months after operation and slightly dysphagia by paired t test. The score of swallowing function in 3 months after operation was significantly different from that in 1 year after operation. Conclusion: the reconstruction of tongue defect with submental island myocutaneous flap can basically restore the swallowing function of patients with tongue cancer after operation. To a certain extent, it can meet the clinical needs of tongue defect repair and reconstruction, and its application value can be further expanded.
