牙周治療前后齦溝液中Ⅰ型膠原羧基端肽及鈣衛(wèi)蛋白的研究

【摘要】:目的: I型膠原羧基端肽(pyridinoline crossed-linked carboxyterminal telopeptide of type I collagen,ICTP)是骨吸收的特異性標志物,研究表明齦溝液中ICTP的含量可反映牙周炎患者牙槽骨的破壞程度。鈣衛(wèi)蛋白(calprotectin,S100A8/A9complex)被認為是炎癥性疾病的標志,其在齦溝液中的高表達與牙周炎的炎癥程度密切相關(guān)。本研究的目的是通過檢測齦溝液中這兩種成分的含量在牙周基礎(chǔ)治療前后的變化探討它們作為牙周炎診斷和療效評價的生化指標的可行性。 方法: 選取14例慢性牙周炎患者作為實驗組,8例健康人作為對照組,,每人選擇2顆磨牙的單個鄰面位點為研究對象。采用吸潮紙尖收集治療前、基礎(chǔ)治療1周后同一位點及健康牙的齦溝液,稱取齦溝液的重量,記錄牙齦指數(shù)、牙周袋深度、附著喪失水平,采用ELISA法檢測齦溝液中ICTP和S100A8/A9的濃度,分別對三組結(jié)果及臨床指標進行統(tǒng)計學(xué)分析。 結(jié)果: 1.牙周炎組治療前齦溝液中ICTP、 S100A8/A9濃度分別大于對照組濃度,差別具有統(tǒng)計學(xué)意義(P0.01)。 2.治療前齦溝液中ICTP、 S100A8/A9濃度與牙齦指數(shù)、牙周袋深度、附著喪失均呈正相關(guān)關(guān)系(P0.05)。 3.經(jīng)基礎(chǔ)治療1周后,齦溝液中ICTP、 S100A8/A9濃度較治療前明顯降低,但仍高于對照組,差別具有統(tǒng)計學(xué)意義(P0.01)。 4.牙周炎組治療后牙齦指數(shù)、牙周袋深度、附著喪失均小于治療前,差別具有統(tǒng)計學(xué)意義(P0.01)。 5.治療前、后齦溝液中ICTP與S100A8/A9的濃度均呈線性正相關(guān)關(guān)系(P0.01)。 結(jié)論: 慢性牙周炎患者齦溝液中ICTP S100A8/A9濃度明顯高于健康人,且與臨床指標呈正相關(guān),經(jīng)過牙周基礎(chǔ)治療后二者濃度明顯下降,治療前后ICTP與S100A8/A9的濃度均呈明顯正相關(guān)。該結(jié)果表明齦溝液中ICTP. S100A8/A9的含量可作為牙周炎診斷的輔助指標,并能有效評價基礎(chǔ)治療的療效。
[Abstract]:Objective:. Type I collagen carboxyl terminal peptide pyridinoline crossed-linked carboxyterminal telopeptide of type I collagenesis (ICTP) is a specific marker of bone resorption. Studies have shown that the content of ICTP in gingival crevicular fluid can reflect the degree of alveolar bone destruction in patients with periodontitis. Calprotectinine S100A8 / A9 complex is considered a marker of inflammatory disease. Its high expression in gingival crevicular fluid was closely related to the degree of periodontitis. The purpose of this study was to investigate the changes of these two components in gingival crevicular fluid before and after periodontal basic treatment. And the feasibility of biochemical indicators for efficacy evaluation. Methods:. Fourteen patients with chronic periodontitis were selected as the control group (n = 8). The single adjacent surface of 2 molars was selected as the control group. Gingival crevicular fluid (GCF) of the same site and healthy teeth was measured after 1 week of basic treatment. The gingival index, the depth of periodontal bag and the level of attachment loss were recorded. The concentrations of ICTP and S100A8 / A9 in gingival crevicular fluid were measured by ELISA method. The results and clinical indexes of the three groups were analyzed statistically. Results:. 1. The concentrations of ICTP, S100A8 / A9 in gingival crevicular fluid before treatment in periodontitis group were higher than those in control group (P 0.01). 2. The concentrations of ICTP, S100A8 / A9 in gingival crevicular fluid were positively correlated with gingival index, periodontal pocket depth and attachment loss before treatment. 3. After one week of basic treatment, the concentrations of ICTPand S100A8 / A9 in gingival crevicular fluid were significantly lower than those before treatment, but still higher than those in the control group (P 0.01). 4. The gingival index, the depth of periodontal bag and the loss of attachment after treatment in periodontitis group were lower than those before treatment, and the difference was statistically significant (P 0.01). 5. Before treatment, the concentration of ICTP and S100A8 / A9 in gingival crevicular fluid was positively correlated with that of S100A8 / A9. Conclusion:. The concentration of ICTP S100A8 / A9 in gingival crevicular fluid in patients with chronic periodontitis was significantly higher than that in healthy subjects, and was positively correlated with clinical indexes. ICTP was positively correlated with the concentration of S100A8 / A9 before and after treatment. The results showed that the contents of ICTP. S100A8 / A9 in gingival crevicular fluid could be used as an auxiliary index for the diagnosis of periodontitis and the curative effect of basic treatment could be evaluated effectively.
